Like most individual and group disability income insurance policies, MetLife disability policies also have limitations, exclusions, reductions of benefits and term under which the policies may be continued in force or discontinued. For costs and complete details of the coverages, call or write your MetLife Group Insurance Representative.

Product and service availability may vary by state and customer size.

1. Self-funded programs available for groups with 1000+. Advice-to-Pay available for groups with 200 + covered employee lives.

2. Voluntary STD is not available in states with statutory plans except NY.

3. Statutory plans, self-funded programs and CA VDP/PFL only available for 500+ lives. Statutory disability plans and paid family leave plans in NY available for groups with 10+ and in NJ, disability plans are available for groups with 100+ covered employee lives. WA self-insured voluntary plan administration and MA fully insured and self-insured coverage available to 1 life in the state. Does not include RI or DC since their programs are government run only.

4. For policies issued in NY, MetLife’s individual disability policies provide disability income insurance only. They do not provide basic hospital, basic medical or major medical insurance as defined by the New York Department of Financial Services. The expected benefit ratio for this policy is 50%. This ratio is the portion of future premiums that the company expects to return as benefits, when averaged over all people with this policy.

5. Available to private employers with a minimum of 500+ covered employees. Absence Management Solutions is only sold in conjunction with group STD (ASO or insured) and insured group LTD. The employer’s FMLA policy must require that the group disability and workers compensation insurance run concurrently with FMLA.

6. Available for employers with 2,000 or more covered employee lives. MetLife does not provide any Disease Management, Health and Wellness Programs or Data Warehousing. Employers will need to contract these services outside of their relationship with MetLife.

7. Some restrictions may apply to the EAP services. Please contact your MetLife representative for details.

Paid Family and Medical Leave:• State Paid Medical Leave applies when an employee has a serious

health condition that prevents them from working. Leaves can be taken intermittently or continuously based on physician certification as defined by each state.

• State Paid Family leave applies when the employee needs leave due to a covered event such as bonding with a child, caring for a family member with a serious health condition or military exigency. Leaves can be taken intermittently or continuously as defined by each state.
