CAD/CAM CuSTOM AbuTMENTSNew Image uses a variety of implant systems to scan and design custom abutments. Using these systems allows us to match the appropriate abutments to your implants in order to retain your warranty. Manufacturers require their abutments to be placed on their implants. We can design and mill custom CAD/CAM abutments from Biomet 3i(*,**), Nobel Biocare - Procera(***), Zimmer and Astra Tech/Atlantis.

Policies

INFECTION CONTROL - OCCuPATIONAL SAFETy AND HEALTH ADMINISTRATION (OSHA)It has been determined that many tasks within the dental laboratory are classified as Category I or Category II hazards as defined by OSHA. We are committed to a proactive role in providing our employees, dentists and their patients with protection from the dangers of cross-contamination. A charge of $1.75 is added to each case to help defray the costs of our comprehensive compliance program.

PAyMENT TERMSAll work done by Lab is due in full upon receipt of each monthly statement. Lab offers a two percent (2%) discount for payment in full by check within ten (10) days of statement date. Unpaid balances over thirty (30) days from statement will be charged an additional 1.5% interest per month (18% annually). Customer agrees to provide a valid credit card on account with Lab at all times and any unpaid balances after 61 days will be automatically be charged to Customer’s credit card. If Customer declines to provide a valid credit card on account, or if the credit card on file is not valid and payment is rejected, all accounts will be classified C.O.D. and Customer agrees to pay all balances on C.O.D. basis plus outstanding balances, if any.

Credit Cards: Visa, Mastercard or American Express are accepted by New Image Dental Laboratory.C.O.D.: $10.50 Shipping Charge, $35.00 Charge on all returned checks.

PRODuCT REMAKE POLICyAll New Image products carry a 100% satisfaction guarantee for the appropriate term of each product. This covers the full replacement cost of all materials and workmanship of the prosthetic produced according to the dentist’s written prescription. When a removable product fits the model produced from the dentists’ impression and does not fit intraorally, New Image shall not be responsible for the full cost of refabrication. In this case, if all models and product are returned to New Image, the charge for product remake will be 50% of the original price. If models are not returned to the laboratory, the cost of remake will be 100% of the original price.


Implant Components

Componentry can be ordered by the laboratory for your convenience. However, be advised any rush shipping and handling fees will be added to the final cost of the components when invoiced. The laboratory would be happy to provide you with a list of parts should you want to order them directly from the implant manufacturer.

New Image Guarantee

THIS PROgRAM IS DESIgNED TO ACCOMPLISH THE FOLLOWINg:

1. Assist you with case presentations (How many times have you been asked, “How long will this restoration last?” or “Is this appliance guaranteed?”)

2. Assist you in keeping your restorative and removable appliance patients on a regular dental maintenance program. To keep the Warranty valid, the crown and bridge patient must adhere to a semi-annual dental maintenance program (cleaning and exam). The full or partial denture patient must have the prosthesis relined as recommended by the dentist, in addition to the 6-month recall.

3. Increase the traffic or patient load for the dentist using this service as a tool for his recall program. This program is available on crown and bridge cases. It is self-explanatory to the patient as details are on the back of the Warranty.*

3. The maintenance schedule on this Certifi-cate must be documented by the attending dentist each visit to validate this Warranty.

4. Full or partial dentures must be relined as recommended to the attending dentist by New Image Dental Laboratory, Inc.

5. Within the limitations of this Warranty, the prosthesis, written work authorization and the Warranty Card must be shipped together to New Image Dental Laboratory, Inc.

In order to give you quality service, New Image needs adequate time for fabricating, inspecting and delivering your work. The schedule refers to full working days in lab Monday thru Friday. Pick-up and delivery days are not considered working days. Pick-up and delivery service is available throughout metro Atlanta for a $4.00 fee per case. Out of town cases should be sent to us by either Priority Mail and UPS Ground (at no charge) or United Parcel Service (UPS) airpack (Second Day Service - Blue - $7.95, Next Day Service - Red - $9.95). All out of town cases are returned via United Parcel Service (UPS) for a $6.00 per case fee.
