once upon a timeFor decades, rigid osteosynthesis, with metal plates affixed to the bone with screws, was one of the most widespread methods for setting fractures.

This technique has some drawbacks, caused by complications due to unfavourable biological and biomechanical conditions. Multidisciplinary studies on osteosynthesis with the use of traditional plates have demonstrated that stability is closely linked to the pressure of the plate on the bone due to the traction force of the screws. Cortical screws tightened to 15Nm of force have a traction force equivalent to 750N. The result is, as an example, a plate affixed with 6 screws pressing onto the bone surface with the force of 6x750N = 4500N (Granowsky). This force is distributed over an adherence surface equal to 15% of the entire surface of the plate, typically around 1.5 cm2 (Spyra).

Such strong lateral pressure causes the disorientation of the osteocytes, as well as osteolysis and osteoporosis, consequently diminishing the fixative force. Limited contact plates can reduce the pressure of the support on the bone. Reduction of the contact surface increases pressure per cm2, further worsening the biomechanical situation.

Rigid osteosynthesis is one of the conditions for primary stability of the bone. Stabilisation can be obtained only after precise reduction of the fracture and after compression between the individual bone fragments.

The recommended compression of over 500N for each cm2 of the transversal section of the bone is damaging, as it impedes the passage of nutritive substances and metabolic products through the bone fragments. The final compressed bone does not change form, but can cause delays in consolidation.

The duration of consolidation by plates is limited. This is due to the considerable overload on the screws, between the head and the start of the threading, which are subjected to the cutting forces transmitted by the movement of the plate. This induces osteolysis of the bone tissue and a reduction of the forces holding the plate. The direct consequence of all this is the loosening and/or breakage of the screws, and the consequent failure of osteosynthesis.

innovation and technologyThe O’nil system, while morphologically similar to traditional plates, is mechanically similar to external fixators. But it is different from each of these, showing itself to be a new and original system of osteosynthesis. Solidarisation between the self-locking screws and the support is assured by the coupling that is created in the interface between the head of the screw and the tapered bore of the bushing.

O’nil is a stable system that is substantially different from a “traditional” plate. The O’nil system elastically transfers forces that act on the fracture. The structure elastically deforms under the action of such forces, returning to its initial state when the actions cease.

MultiAx• The MultiAx system offers variable angle locking

to adapt the screw angles to the different types of fractures and anatomies with possible angles of up to 15° in all directions around the central axis of the plate hole.

MultiAx system characteristicsThe MultiAx screw has the following characteristics

• It offers optimal axial stability, excellent frictional adhesion and easy grip of the screw.

• Helps avoid soft tissue irritation in bicortical use.

• Designed with threads on the lower side of the screw head, which at the time of insertion engage on the circular “lip” inside any hole on the plate, with the exception of the long hole in the shaft.

• Exclusive one-step locking: obtained by simply inserting a locking screw or a peg in the polyaxial locking range of ± 15 degrees, without the need for further steps.

Autolocking screwsimplant technique

1. After the fracture is reduced, solidarise the support onto the bone with the aid of reduction forceps.

2. Insert the drill guide into the bushing, conically coupling it. Insert the helicoidal point into the drill guide and execute a bicortical hole.

3. Pay attention during removal of the drill guide: this must be done with a clockwise rotation.

4. After removing the drill guide, determine the length of the screws with the appropriate gauge.

5. Remove the gauge and insert the screws of the selected size, without completely tightening it into the site, as shown in the figure.

6. Proceed similarly for all the other screws. Before completely tightening the heads of the screws, ensure that the lengths selected are correct.

7. Tighten the screws using moderate force.

MultiAximplant technique

1. Drill a hole in the sliding slot with the bit using the universal drill guide.

2. Determine the length of the screw to be used, with the depth gauge

3. Insert the cortical screw with the hexalobe screwdriver. Adjust the position of the plate.

4. If necessary, use the K-wire to temporarily secure the fragments.

5. Insert the universal drill guide in the hole, inclining it appropriately and drill with the drill bit.

Technique with temporary stabilisation screws

1. Remove the point and the drill guide and tighten the temporary stabilisation screws. If necessary, use the second stabilisation screw.

2. Proceed by inserting the final screws, as specified in the previous implant technique.

3. Remove the temporary stabilisation screws, measure the length of the final screws, and insert them.

4. After verifying that the length is correct, tighten the screws, conically coupling the head with the bushing.

Autolocking screwsInterfragmentary compression

2. Insert the head of the autolocking screw inside the compression device until it stops.

3. Screw the ring nut until the screw head is locked in the compression device.

1. Check that the conical part of the compression device (A) is outside the liner (B). If not, unscrew and press the ring nut (C).

4. After having drilled the hole (see technique on page 11), insert the screwdriver in the compression device and screw the screw into the desired hole. When the compression device comes in contact with the bushing head, the compression phase will begin. Keep on screwing until the desired compression level is obtained.

5. At the end of the compression phase unscrew the ring nut and retract the liner until the conical part of the compression device is free.

6. Keep on screwing until the screw head locks in the bushing cone.

Screw-bushing disassembly technique

2. Screw the cap back onto the screw/bushing disassembler.

1. If the bushing is still assembled at the head when removing a screw, insert the two components into the screw/bushing disassembly tool, as shown in the figure.

3. Insert the beater, as shown in the figure, and disassemble the bushing from the screws.

The four bores (Ø 2 mm) on the walls of the pin allow easy insertion of the cerclage cable.

After selecting the bushing in which to insert the slotted pin, insert it into the conical site and tighten it, using the appropriate screwdriver for the series.

The slotted pin is to be inserted into the bushing to perform cerclage. The slotted pin is made of Titanium Ti6Al4V and is internally hexagonally recessed.

Removal of the implant

1. Use a screwdriver to decouple the head of the screw from the bushing.
