PROCEDURES FOR PHYSICAL EXAMINATION OF INCOMING FRESHMEN OF UP DILIMAN FOR SCHOOLYEAR 2014-2015 The Office of the University Registrar requires all incoming freshmen to secure a medical certificate from the University Health Service (UHS) prior to enrolment. This can be obtained after undergoing a complete physical-medical examination at the University Health Service. The steps to follow are: A. Physical-Medical Examination at the University Health Service 1. The schedule of pre-enrolment physical examination this year is until May 31, 2014, Mondays thru Fridays except holidays, from 8:00 to 11:30 a.m. and from 1:00 to 4:30 pm. The procedure will take at least 3 working days to complete. Hence, all freshmen are enjoined to undergo their physical examination at the UHS as soon as they receive their Notice of Admission into UP Diliman, and to come not later than one week before the end of May 2014. 2. Present the following documents at the Public Health Unit if student comes before April 1, 2014 (at the UHS Conference Room thereafter): a. Notice of Admission b. Consent for Physical Examination for those below 18 years old, duly signed by the parent or guardian. NO CONSENT will mean NO EXAMINATION is to be performed. c. UPHS Form No. 2 duly accomplished with a 2x2 ID Photo d. Questionnaire for Adolescent Preventive Services which the student has already answered. (UPHS Form No. 2, the Questionnaire for Adolescent Preventive Services and the Consent form are all enclosed with Notice of Admission and may also be downloaded at www.upd.edu.ph ) e. List of previous immunizations, if any, preferably signed by a physician. (Optional) f. Chest X-ray film and official result with 2 xerox copies (if student elects to have his Chest X-ray done in a reputable hospital/facility other than the UHS; must have been taken within the last 6 months). 3. After presenting the above documents, you will be directed to any of the following areas: a. X-ray Section Students who will undergo Chest X-ray at the UHS are advised to wear plain T-shirts without metal parts such as buttons or zipper. Results can be claimed after 2 working days at the X-ray Section. b. Public Health Unit for height, weight, BP determination and visual acuity testing. c. Dental Unit d. Triage (last step) for physician assignment 4. After examination by a UHS physician (of the same sex as the student), submit the accomplished forms and Chest X-ray film and results to the Public Health Unit to claim your medical certificate. If the entire procedure cannot be accomplished in one day, do not submit the unfinished forms. Keep the forms until you return to complete the physical examination. 5. Submit your medical certificate to the Office of the University Registrar for enrolment. *Please note that while the student may opt to have his Chest X-ray done in another facility other than the University Health Service, the physical examination should be done at the UHS.
