This free program provides support to older adults, aged 65 and older, who are living at home and who have complex care needs. Clients registered in this free program receive home visits from Community Health Worker, Joellen McHard, and Registered Nurse, Meredith Cullen. The PCO team can provide early intervention, assessment and monitoring of health conditions and link people with supports and services in the community. They can respond to immediate needs, provide ongoing monitoring and support, offer advice and education, as well as provide screening for home and personal safety risks. Clients are able to access services intensely or less so, as their individual needs require. By working in collaboration with other health professionals, geriatric partners, community agencies and the Community Care Access Centre, the aim of this program is to reduce the number of emergency room visits, prevent hospital admissions, reduce isolation and allow seniors to live in their own homes for as long as safely possible. Registered Nurse Meredith recounted how the PCO team was able to assist a client in reducing their risk of falling. Upon visiting with a client, the PCO team used a “falls risk checklist” and determined that the senior had a high risk of falling. The team offered to assist the client in finding reliable transportation so that they could attend the free, 12-week falls prevention exercise program offered at the North Lanark Community Health Centre. Community Health Worker Joellen was able to secure transportation for the client through Lanark County Transportation and also helped reconnect the senior with members of their local church congregation. “It’s a great privilege to be invited into a client’s home and to be given the opportunity to make even a small difference in someone’s life” says Meredith. The Primary Care Outreach to Seniors

(PCO) program has received funding from

the Champlain Local Health Integration

Network (LHIN) to expand services

throughout Lanark County and is now up

and running at the North Lanark

Community Health Centre.

This free program provides support to older adults, aged

65 and older, who are living at home and who have

complex care needs. Clients registered in this free

assessment and monitoring of health conditions and link

They can respond to immediate needs, provide ongoing

monitoring and support, offer advice and education, as

well as provide screening for home and personal safety

risks. Clients are able to access services intensely or less

so, as their individual needs require.

By working in collaboration with other health

professionals, geriatric partners, community agencies and

the Community Care Access Centre, the aim of this

program is to reduce the number of emergency room

visits, prevent hospital admissions, reduce isolation and

allow seniors to live in their own homes for as long as

safely possible.

Registered Nurse Meredith recounted how the PCO team

was able to assist a client in reducing their risk of falling.

Upon visiting with a client, the PCO team used a “falls risk

checklist” and determined that the senior had a high risk

of falling. The team offered to assist the client in finding

reliable transportation so that they could attend the free,

12-week falls prevention exercise program offered at the North

Lanark Community Health Centre. Community Health Worker

Joellen was able to secure transportation for the client through

Lanark County Transportation and also helped reconnect the

senior with members of their local church congregation. “It’s a

great privilege to be invited into a client’s home and to be given

the opportunity to make even a small difference in someone’s

life” says Meredith.

Many people hit the gym or pound the pavement to improve cardiovascular health, build muscle, and of course, get fit, but working out has above-the-neck benefits, too. Regardless of age or fitness level (yup, this includes everyone from mailbox walkers to marathoners), studies show that making time for exercise provides some serious mental benefits. Get inspired to exercise by reading up on these unexpected ways that regular exercise can benefit mental health, relationships, and lead to a healthier and happier life overall.

Reduce stress: Rough day? Take a walk or join a fitness class for a quick workout. One of the most common benefits of exercise is stress relief. Working up a sweat can help manage physical and mental stress.

Boost happy chemicals: Heading out to a weekly exercise class during the winter months can be tough, but it’s worth the effort! Exercise releases endorphins, which creates feelings of happiness and euphoria. Studies have shown that exercise can even alleviate symptoms among the clinically depressed. For this reason, doctors recommend that people suffering from depression or anxiety (or those who are just feeling a little blue) pencil into their calendars plenty of exercise. In some cases, it can be just as effective as antidepressant medication in treating depression.

Prevent cognitive decline: It’s unpleasant but it’s true – as we get older, our brains get a little....hazy. While exercise and a healthy diet isn’t a cure for Alzheimer’s, they can help shore up the brain against cognitive decline that begins after age 45. Working out, especially between ages 25 - 45, boosts the chemicals in the brain that support brain function and prevent degeneration.

Exercise is For More Than Physical Fitness . . .

Alleviate anxiety: The warm and fuzzy chemicals that are released during and after exercise can help people with anxiety disorders calm down. Hopping on the track or treadmill for some moderate-to-high intensity aerobic exercise can reduce anxiety sensitivity.

Boost brainpower: Various studies have shown that cardiovascular exercise can create new brain cells and improve overall brain performance and memory.

Increase relaxation: For some, a moderate workout can be the equivalent of a sleeping pill, even for people with insomnia. Physical activity around five to six hours before bedtime raises the body’s core temperature. When the body temp drops back to normal a few hours later, it signals the body that it’s time to sleep.

Even moderate exercise can have positive effects far beyond the gym. Gaining self-confidence, getting out of a funk, and even thinking smarter are some of the motivations to take time to exercise on a regular basis.
