WDS SPOTLIGHT

Many dermatologists are women, as clearlyevidenced by the large membership of theWomen’s Dermatologic Society (WDS). Women

now comprise 65-70% of dermatology residents in U.S.programs, and more than 40% of the specialty as a whole(Franki R. Skin and Allergy News Digital Network 2012).According to the AMA in 2010, 70% of all U.S. physicianswere male and 30% female, making dermatology unique asa specialty. Although greater than in the past, female chairsand chiefs of dermatology are still a minority with womenmaking up only 16% of the 107 dermatology academic chairsor chiefs (Kimball A. Arch Derm 2012; 148(7):844-846).

Despite a low representation of women as departmentchairs or chiefs in academic institutions, it is expected thatwith a growing number of women entering dermatology, there will also be increasing numbers in academic medicine.A quick look at the list of today’s women leaders is impressiveand clearly shows that the number of women chairs ofdermatology departments has increased in the past decade.

Maria K. Hordinsky, MD, Chair of Dermatology at theUniversity of Minnesota since 2001, is the current presidentof the Association of Professors of Dermatology (APD), anorganization devoted to facilitating the exchange ofinformation regarding best practices and excelling inprogramming aimed at the recruitment, retention, and

mentorship of academic faculty and future leaders. WDSPresident Amy S. Paller, MS, MD is also an active memberwho has facilitated and organized networking for womendermatology chairs and chiefs at the APD. The group, nowin its fourth year, meets annually to discuss issues specificallyrelated to women in academia.

Dr. Hordinsky commented on a career in academicmedicine, saying that “career paths vary in all professions. In dermatology, some residents and younger physiciansdesire private practice, while others choose to become partof academic practices - doing both clinical practice andteaching or research, or both.” When asked what she wouldrecommend to someone interested in academic medicine,Dr. Hordinsky replied that “finding a mentor and someoneto emulate was an important step. That individual might bein one’s own department, another department, or even atanother institution.”

As more women move into leadership roles in theacademic world, they are able to help empower the youngerdermatologists and assist them in choosing paths that mightinclude research along with clinical practice. Dr. Paller waschief of dermatology for 16 years in the department ofpediatrics before becoming chair of dermatology 10 yearsago. With a strong clinical and bench research background

MEMBER PROFILE

In hopes of increasing awareness for sun safety, WDSmember Melanie D. Palm, MD organized andvolunteered her expertise for several skin cancer

screenings on Saturdays during Melanoma AwarenessMonth in May, 2011. Under the name ‘SolSearch,’ she andher office staff organized an informational booth and shescreened patients at Lululemon and three local gyms. WhileDr. Palm did find it challenging to rally support in thecommunity and get the word out to individuals that couldpotentially benefit from free skin cancer screenings, she didfind local businesses to be very supportive of her efforts.

In May of 2012, SolSearch became more structured with aparty at a local spa in which proceeds from the entry fee, asilent auction and raffle (over $4,000) were donated to the SkinCancer Foundation. Dr. Palm held free skin cancer screeningsbefore and during the party, and several local TV affiliates andregional magazines provided coverage of the event.

SolSearch 2013, which raised over $7,000 for the SkinCancer Foundation, was held at Dr. Palm’s practice (Art ofSkin MD) at the Beachwalk Center of Solana Beach,California. Guests were treated to free skin cancerscreenings, while skin care vendors were stationed on theterrace of Art of Skin MD to educate shoppers on ways toprotect against sun damage. The event culminated with anevening cocktail party that included raffle prizes, a silentauction and live music. In just a few short years, SolSearchhas grown to a full-fledged party with several dermatologistsproviding skin screenings, and fundraising efforts havenearly doubled in just one year!

Plans for 2014 are already underway for an event that Dr.Palm expects will be bigger and better, with morefundraising and more patients screened. “By increasingcommunity awareness,” she says, “we’ll continue to makethis a celebration of healthy skin.”

For WDS members who want to help raise awareness forSkin Cancer/Melanoma Awareness Month, Dr. Palm offersthe following advice:

• Consider volunteering at a local WDS skin cancerscreening event. The Skin Cancer Foundationsponsors a nationwide “Road to Healthy Skin Tour”that dermatologists may volunteer for as well.

• Start local and small, and soon the event grows –especially if you are passionate about the cause. Dr.Palm encourages members to provide similar events intheir community during the month of May. Skincancer is top of mind to many in San Diego due to ourclimate; even here, however, there are still plenty ofopportunities for patient and public education.

• Developing your own philanthropic cause takes a lot oftime and consideration, and supportive staff makes ahuge difference. Dr. Palm credits her wonderful team, who all work tirelessly for SolSearch each year. Regularmonthly planning meetings at her office start 6-7months before the event, with weekly meetings takingplace during the last month.

• Trying to balance event preparation with workresponsibilities can be tricky. A lot of the work andpreparation for SolSearch occurred after hours orduring weekends, but the sacrifice was definitely worththe end result: detecting and preventing skin cancers, raising public awareness, and fundraising success forskin cancer research.

Looking back on the implementation and growth ofSolSearch, Dr. Palm notes “I think it’s a wonderful wayto give back to the community. It reminds me of why Ibecame a physician and dermatologist: to help patientsand those in need. It has been a wonderful way toconnect with other physicians and develop relationshipswith local businesses.” n

Melanie D. Palm, MD WDS members are finding creative ways to raise awareness for sun safety in their own communities. Here, we profile the recentefforts of Melanie D. Palm, MD on behalf of Melanoma Awareness Month and what has become the annual ‘SolSearch’ event inSan Diego, California.

READS & RECOMMENDSBy Felisa Lewis, MD and Jennifer Sorrell, MD

Lean In By Sheryl SandbergOn the heels of a TED talk gone viral("Why we have too few womenleaders"), Sheryl Sandberg, COO ofFacebook, has written Lean In, amanifesto for a new generation offeminists. Her observations of genderinequality in the workplace are well-grounded in statistics and studies, whileanecdotes from her own experiencehighlight her assertion that not only society but womenthemselves have held their gender back from makingsignificant progress in "free choice."

The themes are neither new nor ground-breaking, butSandberg’s messages of ‘Sit at the table,’ ‘Make your partnera real partner,’ and ‘Don't leave before you leave’ (amongothers) are well-taken and she illustrates her points byrecounting personal experiences with surprising candor. While offering specific steps women can take to combineprofessional achievement with personal fulfillment,Sandberg also demonstrates how men can benefit bysupporting women in the workplace and at home.

This book is truly thought-provoking, and will inspiredebate for both genders about how to effect meaningfulchange for the individual, with the goal of evolving societalnorms about women. Quoted no less than three times atthe WDS MELD Forum, Sheryl Sandberg clearly has a lotof fans among WDS members.

Gone GirlBy Gillian FlynnWhat would it be like to be married toyour worst enemy but know in yourheart that he or she was the best personfor you? At the onset of this novel, NickDunne comes home to an empty houseon his 5th wedding anniversary todiscover his wife, Amy, missing. Notsurprisingly, Nick is the prime suspect inher disappearance as Amy’s diary tellsthe tale of a failing marriage in a classic "He said, She said."

Nick struggles to prove his innocence whileincriminating evidence piles up against him and, as secretsare cleverly exposed, the question of who's the victim and

who's the manipulator gets turned on its head. If you are afan of reading the book before seeing the movie, this shouldbe next on your list. The film version of Gone Girl, featuring Ben Affleck as Nick Dunne, is scheduled forrelease later this year.

Little BeeBy Chris CleaveWhile struggling to reconnect in theirmarriage in the face of infidelity anddepression, Sarah and AndrewO'Rourke travel on a spontaneousvacation to Nigeria. Wanderingoutside the confines of their resort, thecouple encounters Little Bee and herolder sister from the nearby ruralvillage. As the couple comes to realizethe sisters were fleeing from soldiers who had slaughteredtheir entire village for its underlying wealth of oil, the girlsand the O'Rourke’s find themselves in a life and deathconfrontation with these brutal men.

These events are the background to the heart of thenovel, which starts off some time after this life-alteringvacation. Little Bee has managed to escape Nigeria andmake her way to a British immigration center, where shedresses unfashionably and binds her chest to make herselfundesirable to the center's lustful guards. Her acquiredQueen's English helps with her illegal release and shemakes her way to the O'Rourke home in Kingston-upon-Thames. Little Bee arrives just after Andrew's untimelysuicide, and the novel takes us through the process ofhealing and the development of an unlikely friendshipbetween Sarah and Little Bee. This novel is historicallyinteresting, deeply touching and a fast-paced read. n

Dermatopathology: Apocrine HidrocystomaBy Michelle Tarbox, MD

Apocrine hidrocystoma presents clinically as asolitary translucent cystic nodule, often with abluish hue, typically on the face, most commonly

around the eyes (figure 1). The translucent nature of theselesions can be enhanced by side lighting. Histologically, apocrine hidrocystomas are characterized by a large cysticspace within the dermis, lined by a single layer of columnarcells demonstrating decapitation secretion, “pig snouts” andsurrounded by a peripheral row of myoepithelial cells(figure 2).

Apocrine hidrocystomas can be associated with a rareform of ectodermal dysplasia known as Schöpf–Schulz–Passarge syndrome (SSPS). SSPS is characterized byhypotrichosis, hypodontia, nail dystrophy, a palmoplantarkeratoderma, peri-ocular hidrocystomas, and palmoplantareccrine syringofibroadenomas. You can remember thissyndrome with the help of the Passarrrrge pirate, a salty old

I recently had the honor of beingmentored by Pearl E. Grimes, MDat The Vitiligo and Pigmentation

Institute of Southern California.Demonstrating the utmost care, respect, and compassion for her patients, Dr.Grimes embodies what it means to be awell-rounded physician. Patients who hadoften been seen by several other dermatologists and thoughtnothing could be done for their cosmetically disfiguringconditions were reassured and treated with compassion by Dr.Grimes. She often commented on the cosmetic stigmaassociated with the conspicuous condition of vitiligo, and Ifound her upbeat personality and confidence to be veryreassuring for her patients.

Dr. Grimes alsoworks as a solo practitioner and I was impressed by thegraceful yet authoritative manner in which she worked withher ancillary staff. She frequently discussed her passion forThe Coalition for At-Risk Youth (CARRY), a non-profitorganization dedicated to the enrichment of foster and at-risk youth, and I came to admire Dr. Grimes for the manyroles she balances: - academician, physician, researcher, entrepreneur, philanthropist, fundraiser and mother.

While the time I spent with Dr. Grimes was indeliblyenriching, I am even more excited about what lies ahead incultivating a lifelong mentorship with such a spectacularwoman. My deepest thanks to Elizabeth McBurney, MDand Erin E. Boh, MD, PhD for their recommendations, andto Medicis for sponsoring the WDS Mentorship Award thatmade this opportunity possible.

that was of benefit to her, she is now one of two femaledepartment chairs out of 20 at Northwestern. “It isinteresting being surrounded by male colleagues in theboard room,” said Dr. Paller. “It makes me realize on aconstant level how much inequity there is in academics.”Dr. Hordinsky’s experience is similar, as one of three womendepartment chairs at her university.

As the specialty of dermatology grows and changes, it isexpected that there will be more women in leadership roles. The current roster of leaders is very impressive, and allserve as role models to women moving up in the ranks ofacademic medicine.
