• Patient is admitted to the ICU with respiratory failure:– Respiratory rate 28 breaths/min, peripheral oxygen

saturation 95% (6 L oxygen/min via mask)• He has been hospitalised for 4 weeks because of

haemophagocytic syndrome (diagnosis by bone marrow smear)

• Received therapy:– HLH4 protocol (etoposide, dexamethasone, cyclosporin A)

• History of multiple periods of fever of unknown origin
