Interagency planning is utilized to provide an opportunity for individuals and families to coordinate existing services and supports, as well as identifying additional resources. We work as a team with all involved child-serving systems.

The team works to develop comprehensive recommendations and activities to support successful outcomes and well-being for the child and family (home, school and community).

Provides an opportunity to brainstorm and bring people together across systems, agencies and natural supports.

Looks at the individuals strengths, needs, hopes and opportunities for positive change. CASSP seeks to understand how the children’s behavioral health system works in our

County and can serve as a resource to family members, providers and others who need assistance with services. This includes providing information on resources and technical assistance.

CSP works collaboratively together using the CSP principles to evaluate mental health services, policies, regulations and legislation to make recommendations/advocate for change that will positively impact the lives of those with mental health challenges.

CSP is a coalition of individuals who receive mental health services, their family members and mental health providers with the Bucks County Department of Mental Health/Developmental Programs and other County Behavioral Health Partners in an equal partnership to promote recovery and excellence in the delivery of community based mental health services. A child/adolescent must meet eligibility requirements for MA through documentation of a disability. Typically, individuals come to the system by referral from schools, child-care providers, health care providers and family members.

For adults, some individuals have Medicare and Medicaid ( Dual eligible )

Magellan Behavioral Health is the identified MCO for all of Bucks County.

If MA eligible, the member/family should contact Magellan to request an assessment for services. Depending on the service requested, the member may be referred to their primary insurance.

County Funding is available for specific services and needs to be authorized by County Staff ( FSS ).

Planning for transition considers insurance eligibility and services and supports are developed accordingly.

At 18, individuals Must apply for Social Security and may be entitled to SSI!

Service Description: General case management that provide referral, monitoring and service coordination. Can assist with transition planning in a limited scope

Service Description: Eligibility-based case management service with 24/7 capacity to provide individualized, face-to-face contact with person on an ongoing basis to assist the person in gaining access to needed supports, services and to assist in the coordination of care

Access Services provides a special model for transition age youth ( ages 16-26 ) : Transition to Independence Program Model ( TIP )

CPS are trained and use their lived experiences to support individuals

We now have TAY CPS through Child and Family Focus for individuals ages 14-26

Funded through Medicaid and limited County dollars TAY CPS can assist young people and young adult to plan

for transitions

Service Description: Mental Health treatment services provided by qualified mental health professionals that include individual, group and family therapy, medication management, diagnostic evaluation and assessment.

Funding Source: Commercial insurance, County; HealthChoices; MA Fee-for-Service; Other

Population Served: Children/Adolescents/Adults

Programs created specifically for TAY through our Grant: TAY HI-FI: utilizes a national model to bring change to the lives of youth and young

adults with complex needs, including a youth support partner ( Child and Family Focus )

TAY Peer Support: A Peer Support model for individuals ages 14-26. A recovery and resiliency based model to assist individuals to identify plans and attain their goals. The model utilizes the support of a peer sharing their own lived experiences ( Child and Family Focus )

Family Support Specialist: provides support to families with youth and young adults experiencing behavioral health needs ( Access Services, Inc. ).

Employment Specialist: to work with young people ages 18-24 who are experiencing homelessness and are in Rapid-Rehousing ( Valley Youth House)

TAY RISE program: ( Reaching Independence with Support and Encouragement) A unique model of support for individuals 16-25 years old. Uses the Casey Life Skills Assessment and creates a mentoring model to gain specific skills ( Child and Family Focus )

Student Assistance Program: Screening and assessment of at-risk or identified youth in collaboration with the School District ( Council of SEPA )

intervention services throughout Bucks County Grandview Hospital- Crisis services are provided by

Penn Foundation Doylestown Hospital- Crisis services are provided

by Lenape Valley Foundation Mobile Crisis Services- Crisis services are provided

by Lenape Valley Foundation for all ages

Housing- Community Rehabilitation Residences (CRR) and Supported Living Program Description- Housing options for eligible

people with serious and persistent mental illnesses. Supervision varies from programs to meet individuals’ needs. Housing is transitional and helps individuals move on to independent living.

Funding Source- County Population Served- Adults ( must be 18 years old to

access housing services )

Bucks County Supported Employment Provides pre and post-employment help (resume building, interview

practice, job searching, interview assistance, job maintenance, advocacy with manager/employer, on-site job support, etc…) for those living in Bucks with a “Priority” Dx

Bucks County Supported Education (2 County-Funded Programs) TAP-Tuition Assistance Program

Provides County-funding (only for tuition) for upwards to a 4 credit course per semester (Spring, Summer, Fall) at Bucks County Community College for adults living in Bucks with a “Priority” Dx

Individuals enrolled in TAP are also eligible for any and all supports provided through the College Plus program

Voice & Vision’s “College Plus” Program Provides (1) individualized and group support to help maximize an

individual’s chances of success and (2) County-funding for tuition, books & materials, etc. for one course at Bucks County Community College for adults living in Bucks with a “Priority” Dx. Funding may or may not be available through other funding streams for multiple courses and for individual’s without a “Priority” Dx.

TAY First Responder Adult Professional

Question Persuade Refer (QPR) Training Approximately 1500 individuals, including Bucks County employees, have

been trained since 2016 as QPR Gatekeepers.

Mental Health First Aid (MHFA)93 individuals have been trained throughout the Bucks County in

community in FY 17/18 . Trainings will continue in 18-19!

1402 individuals have been trained since Fall of 2015 through the Now Is The Time: Healthy Transitions (NITT: HT) initiative.

Crisis Intervention Team (CIT)361 law enforcement and 495 professionals have been trained since the

start of this initiative in 2008.

Crisis Response Training94 corrections officers and 91 911 Dispatchers have been trained.

Magisterial District Judge Program The goal of the Magisterial District Judge Mobile Engagement Pilot is to divert individuals with mental illness, substance use, or co-occurring disorders out of the criminal justice system

into appropriate community supports while maintaining public safety
