    The Journey

    Getting a LifeA cross-government programme and part

    of the Governments Valuing People

    Nowstrategy. This strategy aimed to

    improve the lives of people with learning

    disabilities. Employment was one of thetop three priorities (along with housing

    and health).

    Getting a Life showed how people in

    local areas can work together so thatyoung people move from school or

    college into paid employment.

    Manchester pathfinder vision

    The Manchester Pathfinder focused initially onyoung people in transition aged 14-25.

    Our vision is to create a single, person

    centred, aspirational Education, Health and

    Care plan for all eligible young people, with

    flexible funding to deliver the plan.

    The plan is holistic, based on choices from the

    whole range of options, including employment,leisure and independent living.

    Preparing for Adulthood 5 Key Messages

    1. Develop a shared vision of improving life chances withyoung people, families and all key partners.

    2. Raise aspirations for a fulfilling adult life by sharing clear

    information about what has already worked for others.

    3. Develop a personalised approach to all aspects of

    support using person centred practices, personal budgetsand building strong communities.

    4. Develop post 16 options and support that lead to

    employment, independent living, good health, friends,

    relationships and community inclusion.5. Develop outcome focussed multi-agency commissioning

    strategies that are informed by the voice of young people

    and families.
