ALZHEIMER’S DISEASE

• Alzheimer’s is one type of dementia

that accounts for up to 70% of all

dementias.

• Alzheimer’s is NOT normal aging; it’s

a progressive, fatal, brain disease.

• There is a new case of Alzheimer’s

disease in the U.S. every 68 seconds.

• Of Americans aged 65 and over, 1 in 8

has Alzheimer’s, and nearly half of

people aged 85 and older have the

disease.

ALZHEIMER’S DISEASE

• In 2011, 70% of people with Alzheimer’s

lived at home with help from family

members.

• In Minnesota over 241,000 Minnesotans

were caring for someone with

Alzheimer’s, providing unpaid care

valued at $3.3 billion

• The physical and emotional impact of

caregiving is estimated to result in $148

million for Minnesota caregivers.

• One in seven people who have

Alzheimer’s live alone.

ALZHEIMER’S DISEASE

• Without change, the total cost of

care for Americans age 65 and

older with Alzheimer’s disease

will increase five-fold per year.

• In Minnesota alone, these costs

could reach $20 billion, most of

that coming from personal funds

and Medicaid dollars.

PREPARING MINNESOTA

Prepare Minnesota for Alzheimer’s seeks to:

1. Increase early identification and improve

ongoing care;

2. Sustain caregivers by offering them

information, resources and support;

3. Equip communities to support their

residents who are touched by the

disease;

4. Identify and invest in promising approaches

that reduce costs and improve care; and

5. Raise awareness, reduce stigma and

engage community.

EQUIPPING COMMUNITIES FOR THE IMPACT

Prepare Minnesota for Alzheimer’s has developed a Community Tool Kit to assist communities in becoming dementia capable so that they can support their residents who are touched by Alzheimer’s Disease.

DEMENTIA CAPABLE COMMUNITY

A dementia capable community is informed, safe and respectful of individuals with dementia and their families, provides supportive options, and fosters quality of life. Specifically, a dementia prepared community:• Ensures access to a variety of high-quality options

that improve mobility, housing, health care, supportive services, financial and legal planning and support, and advance care planning;

• Recruit volunteers and other community resources to respond to the needs; and

• Foster contribution, connection, and meaning for individuals with the disease and their families.

A TOOL KIT FOR DEVELOPING DEMENTIA CAPABLE COMMUNITIES

A tool kit is available and can be used for:• Convening key community leaders and

members to understand Alzheimer’s disease and other dementias.

• Assessing current strengths and gaps in dementia capability.

• Identifying community goals and planning possible ways to respond.

• Acting Together to pursue priority goals to foster community preparedness and dementia capability.

PMA 2020 is seeking “Action Communities” that will pilot the tool kit. This would entail the following activities :• Convene a community team of citizens to review the

tool kit and provide feedback as it is developed;

• Test the toolkit by assessing community needs and identifying possible goals the community might undertake to prepare for needs related to dementia;

• Take action in a focus area the community team chooses to undertake; and

• Provide feedback and participate in a survey/evaluation of the tool kit.
