1.0 INTRODUCTION Premium Pipe & Supply facility is located at 2703 North Cedar Street, approximately 0.6 mile west of the Intersection of Hwy 95S and Arrow Drive/1-40 Exit 107, Morrilton, AR, Conway County. The facility is used by vehicles required for transporting fluids during natural gas well drilling operations. The site presently has facilities for regular vehicle maintenance and will have washout facilities for cleaning the interior and exterior of tank vacuum trucks. Waste generated from this process will be stored in an earth pit.

The proposed storage pit has an estimated total volume of 1,556 barrels. The stored residual fluid from washing and maintaining trucks may consist of but not limited to fresh water, water based drilling mud, drill cuttings, produced water, frac water, flow-back water, bentonite clays, chemical additives, barite, foaming agents and/or lubricants. The estimated daily volume of waste produced is 100 barrels/day.

In accordance with the Arkansas Department of Environmental Quality's (ADEQ) current regulations, all facilities that engage in land application or storage of fluids generated or utilized during exploration or production phases of oil or gas operations must provide a closure plan for that facility. This plan will document closure procedures in the event closure of the facility is necessary. These procedures include;

• Removal and disposal of fence, • Removal of wastewater from the earth storage pit, • Removal of solids from the concrete waste collection pit, • Filling pits after fluid, solids, liner and concrete material is removed, • Establishing vegetative cover to near pre-development conditions.

A cost estimate for closure of the facility is provided with this plan.

2.0 C L O S U R E OVERVIEW The closure process for the facility is summarized as follows:

1) Remove and dispose of fence. 2) Fluid stored in the pits will be removed and transported to an ADEQ approved

disposal area off-site. 3) Accumulated solids, liner and concrete will be removed and disposed of in a

permitted Class 1 Landfill. 4) Site will be graded to near adjacent natural ground. 5) Establish vegetative cover on disturbed soils to minimize erosion. 6) Submit report of closure activities to ADEQ.

3.0 WASTEWATER DISPOSAL The wastewater stored in the 1,556 barrel earth pit is to be removed and transported to an ADEQ approved off-site disposal area.

4.0 SOLIDS AND LINER DISPOSAL Prior to closure of the facility, accumulated solids and pit liner would be characterized through laboratory analysis. Once characterized and approved for Class 1 landfill disposal, solids will be removed, dewatered and transported to a permitted Class 1 landfill.

5.0 CONCRETE and EARTH STORAGE PIT C L O S U R E S After all fluids, solids, liner and concrete are removed and off-site, the pits are to be filled with compacted earth.

The site is to be smoothed and graded to closely match original contours. Additional fill material will be brought in if necessary to match existing adjacent grade. Upon completion of the earthwork, all disturbed areas are to be re-vegetated in order to minimize erosion.

6.0 C L O S U R E CERTIFICATION

Periodic inspection and monitoring by a Professional Engineer or his designated assistant must be performed during the closure process to ensure that activities are conducted in a manner consistent with this plan. Upon completion of closure activities, a Professional Engineers certification that the basins were closed in accordance with the approved plan will be provided to ADEQ. The certification will include a summary report of the closure process and any "as-built" drawings and other pertinent information if needed.
