RESTORATION STRIPING NOTES 1. THE CONTRACTOR SHALL BE RESPONSIBLE FOR LABOR AND MATERIAL INVOLVED IN THE MARKOUT, AND INSTALLATION OF ALL RESTORATION

SlRIPING/PAVEMENT MARKINGS. THE CONTRACTOR SHALL ALSO BE RESPONSIBLE FOR REMOVAL OF ALL CONFUCTING DETOUR SlRIPING, SIGNING, AND OTHER DETOUR-RELATED TRAFFIC CONTROL DEVICES PRIOR TO THE INSTALLATION OF THE FINAL SlRIPING. MARKOUT SHALL BE BY HEAVY PAINT BRUSH MARKINGS OVER A PULLED ROPE IN THE RESPECTIVE WHITE AND YELLOW COLORS OF THE PROPOSED SlRIPING. SlRIPING AND PAVEMENT MARKINGS SHALL BE IN HOT APPUED ALKYD THERMOPLASTlC IN ACCORDANCE WITH lADOT SPECIFICATION NOS. 51-005-10 AND 76-012-15. PRIMER AND SEALERS ARE REQUIRED AND SHALL CONFORM TO lADOT SPECIFICATION NO. 76-012-15.

THE CONTRACTOR SHALL CONTACT LADOT BRIDGE PROGRAM SECTION AT (213) 972-8491 AT LEAST 10 WORKING DAYS BEFORE BEGINNING MARKOUT. THE CONTRACTOR SHALL CORRECT ALL ERRORS IN MARKOUT REQUESIED BY lADOT. THE INSTALLATION OF THERMOPlASTlC STRIPING MAY PROCEED ONLY AfTER APPROVAL OF MARKOUT BY THE lADOT DISlRICT ENGINEER.

5. TEMPORARY REFLECTIVE RAISED PAVEMENT MARKERS SHALL BE MAINTAINED ADJACENT TO ANY TEMPORARY CENTERUNE, MARKOUT CENTERUNE OR SCAR OF A SANDBlASTED CENTERUNE, AS APPUCABLE, AS PER LADOT STANDARD PlAN NO. S-453.0.

TEMPORARY SJRIPING NOTES 1. REMOVE ALL CONFUCTING SlRIPING PRIOR TO INSTALLATION OF WTCP/TCP.

2. WHEN REMOVAL IS REQUIRED BY CONSlRUCTlON, CROSSWALK MARKINGS AND PEDESTRIAN SIGNAL HEADS SHALL BE REMOVED SIMULTANEOUSLY.

3. WHEN REQUIRED BY CONSlRUCTlON, THE CONTRACTOR SHALL MAKE ARRANGEMENTS WITH LACMTA, OR THE APPROPRIATE TRANSIT OPERATION, FOR THE TEMPORARY RELOCATION OF BUS STOPS/ZONE. COORDINATE RELOCATION WITH lADOT FOR INSTALLATION OF "TEMPORARY TOW AWAY NO STOPPING-BUS ZONE" SIGNS.

4. CONTRACTOR SHALL PROVIDE REFLECTORIZED CONES AT TYPE III BARRICADE SUPPORTS TO ALERT PEDESTRIANS OF THE BARRICADE SUPPORT.

DIAMETER OF THE MODULE.

1. Reason for Task Work Order:

Contract Drawings calls for the raising of an existing manhole to grade. However, upon field verification no manhole exist. Also, this project has required the Contractor to spend numerous manhours coordinating this work, obtaining LADOT approved traffic control plans and attending meetings that were not accounted for in the original proposal. 2. Description of Task Work Order:

The Contractor is to construct a new City approved manhole and be reimbursed for the additional manhours spent in coordinating this project.

This is not an authorization to proceed with the proposed change . Do not perform any work that may conflict with the proposed change without further authorization by the City .
