Pre-Employment Requirements Checklist
upon hire up to first week of employment
Complete the following pre-employment requirements and put them in a long brown envelope. Submit to HUMAN RESOURCES according to the given
deadline. Items in bold should be downloaded from the Preboarding Kit on your MicroResumes dashboard. Items in red should be provided by HR.

SSS E-1 Form
1. This form is needed to register for the government’s Social Security System (SSS). The SSS
entitles you to the following social security benefits: sickness, retirement, maternity, disability,
death, funeral, and employee’s compensation.
2. If you have previous working experience, then you should already have your SSS ID or
completed SSS E-1 Form. Just submit to us a photocopy of either your SSS ID or your completed
SSS E-1 Form.
3. If you are a fresh graduate or have no previous working experience, you will need to print and fill
out the SSS E-1 Form and have it processed in any SSS office.
4. Submit to us the photocopy of the processed SSS E-1 Form.
5. Instructions on filling out and processing this form can be found on the second page of the file
named SSS E-1 Form in the preboarding kit.
6. Reminder: You will be the one to process this form in the SSS office, not HR.
BIR Form 1905
1. This form is used to transfer your records from your previous Revenue District Office (RDO) to
MicroSourcing’s RDO.
2. If you have previous working experience, print and fill out the form then submit it to your old RDO.
BIR will have it stamped.
3. If you are a fresh graduate or have no previous working experience, you don’t need to print and
fill out this form.
4. Submit to us the stamped copy of the BIR Form 1905.
5. Instructions on filling out this form can be found on the third page of the file named BIR Form
1905 in the preboarding kit.
6. Reminder: You will be the one to process this form in your old RDO, not HR.
PhilHealth Form PMRF
1. This form is needed to register for the government’s healthcare program with PhilHealth.
PhilHealth reduces the amount of expenses upon confinement or acquisition of selected health
services. It also entitles you and your dependents to inpatient and outpatient services in
PhilHealth accredited facilities.
2. If you have previous working experience or you already have an existing PhilHealth number, print
and fill out the form. Check the box “For Updating” and write your PhilHealth number on the
boxes above the purpose.
3. If you have no previous working experience or you don’t have an existing PhilHealth number, also
print and fill out the form. Check the box “For Enrollment.”
4. Submit to us 2 copies of the completed PhilHealth Form PMRF with the necessary attachments.
5. Instructions on filling out this form can be found on the second and third pages of the file named
PhilHealth Form PMRF in the preboarding kit.
PAG-IBIG Consolidation Form
1. This form is used to consolidate PAG-IBIG contributions into the PAG-IBIG office where
MicroSourcing remits contributions.
2. If you have previous working experience, print and fill out the form. Indicate the names of your
previous employers and the inclusive date of your employment with them. Bring the form to any
PAG-IBIG branch to transfer your consolidations from your previous employer.
3. If you are a fresh graduate or have no previous working experience, you don’t need to print and
fill out this form.
4. After having the form processed in any PAG-IBIG branch, submit to us the completed PAG-IBIG
Consolidation Form.
5. Reminder: You will be the one processing this form in the PAG-IBIG office before submitting it to
HR.
Authority to Deduct Form
1. This form is for payroll purposes. We will use this to enroll you for a bank account where your
salary will be credited.
2. Print the form. Fill out Employee Name.
3. Under Deduction Type, check Others and write Metrobank on the space provided.
4. Under Total Amount, write P100.
5. Under Inclusive Pay Periods, check Non Recurring.
6. Under Pay Period, if you were hired from 1-15th of the current month, write 30
th of same month. If
you were hired from 16th-30
th of the current month, write 15
th of next month.
7. Above Employee’s Signature over Printer Name/Date, affix your signature over your printed name
and date.
8. Submit to us the completed Authority to Deduct Form.
HMO Dependent’s Enrollment Form
1. This form is to enroll you for a health insurance plan with our HMO provider. Enrollment ensures
coverage for your and your dependents’ health-related needs.
2. Print the form. Fill out the Employee Information section.
3. Under the Dependent’s Data section, the guidelines on filling it out are:
a. If the Benefits section of your employment contract says “Standard HMO”, only the
employee (you) are covered. You don’t need to fill out the Dependent’s Data section. But
if you have a Standard HMO and you still wish to enroll a dependent, you can include one
but the premium for that will be deducted from your salary every 15th and 30
th of the
month.
b. If the Benefits section of your employment contract says “Covered up to 1 additional
dependent,” fill out the Dependent’s Data section.
c. If you are Single, you can only enroll your parents first as dependents before your
siblings. Parents should not be beyond 65 years old and siblings should not be beyond
21 years old.
d. If your parents already have HMO and you wish to enroll your siblings, attach a
photocopy of your parents’ HMO card.
e. If you are Married, you can only enroll your spouse first before your children.
f. If your spouse already has HMO and you wish to enroll your children, attach a photocopy
of your spouse’s HMO card.
g. If you are a Single Parent, you can enroll your children.
h. If you are enrolling your spouse as a dependent and he/she is working abroad, you need
to attach proof that he/she is working abroad.
4. Under the Room & Board section:
a. Check the box beside Small Private under the Room & Board section.
5. Submit to us the completed HMO Dependent’s Enrollment Form.
Insular Life Application Form
1. This form is to enroll for a life insurance plan with our life insurance provider.
2. Print and fill out the application form.
3. Since the application is for a group insurance, you can designate as many beneficiaries as you
want.
4. Submit to us the completed Insular Life Application Form.
Waiver: W2 BIR Form 2316
1. This form is needed as an attachment to your BIR Form 2316.
2. Print and fill out the form. If you have no previous employer within the year, or your previous
employment was abroad, or if you are a fresh graduate, check box 1.
3. If you are a rehire of MicroSourcing, check box 2.
4. If you have a previous employer within the year, check box 3. Then choose amongst options a, b
or c.
5. Submit to us the 2 copies of the completed Waiver: W2 BIR Form 2316.
Affidavit of Non-Employment
1. This form is used to complement the Waiver: W2 BIR Form 2316.
2. You only need to print and fill out this form if you checked box 1 in the Waiver: W2 BIR Form
2316.
3. You don’t need to have this form notarized.
4. Submit to us 3 copies of the completed Affidavit of Non-Employment.
