• To be aware of and respond to the health rights and needs of the culturally and
demographically diverse peoples and communities of New Mexico.
• The Foundation makes grants, contributions, and program-related investments to
fulfill its Mission of improving the health status and access to health care
services and advocates for health policies that address the health needs of all in
New Mexico.

Annual Grant Cycles close: Friday, April 30, 2021(at midnight)
1. Con Alma Health Foundation Annual Grant Cycle
Awards grants to eligible nonprofits whose efforts help ensure that New Mexico’s diverse communities
have full access and opportunities to lead healthy lives through support of health systems strategies
that address Social Determinants of Health (gaps, barriers, needs, and priorities)
2. Northern New Mexico Health Grants Group Grant Cycle
Awards grants to eligible nonprofits that promote the health and wellness of the diverse peoples of Los
Alamos, Rio Arriba and northern Santa Fe Counties
Joint initiative of Con Alma Health Foundation and the Hospital Auxiliary of Los Alamos Medical Center

Northern NM Health Grants Group
• Geographic Reach/Impact: Region Specific to Los Alamos, Rio Arriba, & northern Santa Fe Counties
• Single Review Process
• Notification of Denial: October or November
• Request Type * (choice) of Project,
General Operating/Core, or Technical Assistance Support
• Focus/Criteria outlined in NNMHGG GUIDELINES
Common to both• Grants to Federally recognized 501c3s or
Fiscal Sponsor (with 501c3 & financial capacity)
• Must benefit people living in NM
• Health defined broadly
• Single-year implementation: Jan 1 to Dec 31
• Open to all eligible applicants (based on
guidelines), including current and past grant recipients of Con Alma and NNMHGG
• Request Amount * (choice) of $7,500,
$10,000, $15,000, or $20,000
• Organization’s operating budget
• Project Budget (when applicable)
• Tribal Letter of Support (when applicable)
• Notification of Award: October or November
• Eligibility & Restrictions (similar)
• Access to Application Quiz & Form via Online Grant Portal
