Il dicumarolo o warfarin (acronimo della Wisconsin Alumini
Research Fundation) venne scoperto casualmente come
prodotto della fermentazione di un un trifoglio che si trovava
nel foraggio secco somministrato al bestiame durante l'inverno
del 1933 per mancanza di quello fresco. A causa del suo effetto
anticoagulante causò una morìa di capi negli allevamenti del
Wisconsin. Venne anche scoperto che una sostanza,
denominata vitamina K era utilizzabile come antidoto per
annullare l'effetto anticoagulante. Inizialmente su usato solo
come topicida.
Durante la guerra, un marinaio tentò di suicidarsi ingerendo
una cospicua quantità di topicida e un po' di vitamina K fu
sufficiente ad evitarne la morte, sicché si cominciò ad
ipotizzarne un uso terapeutico. La comunità medica rimase
tuttavia scettica fino a quando il Presidente Eisenhower,
colpito da ―stroke‖, richiese di essere curato con il farmaco
antitrombotico più potente in quel momento: il warfarin. Il
farmaco riuscì a guarire il Presidente — successivamente
eletto per un secondo mandato — guadagnandosi così la
notorietà.

Il meccanismo d'azione del warfarin è dovuto
alla sua capacità di antagonizzare le funzioni
della vitamina K.
La protrombina ed i fattori VII, IX e X per
poter divenire attivi devono subire la
carbossilazone di alcuni residui di ac.
glutamico, al fine di generare l‗ac. Y-
carbossiglutammico.
Durante la reazione di carbossilazione la
vitamina K, che fissa e poi cede la molecola
di CO2, viene trasformata in vitamina K
epossido che viene poi riconvertita nella
forma precedente tramite una specifica
reduttasi. Questo enzima è il bersaglio
dell'azione del warfarin, il quale ne determina
l'inibizione.

riduzione dell‘HB di 2gr/dl o che rendano
necessarie emotrasfusioni

Warfarin
Le linee guida dell‘ESC 2011 sulla ―gestione delle malattie
cardiovascolari in gravidanza‖ indicano, in classe II A, di
proseguire con warfarin nel 1‘ trimestre di gravidanza se il
dosaggio necessario a mantenere valori di INR in range fosse
inferiore a 5 mg.
In caso contrario (ovvero se il dosaggio necessario fosse
superiore a 5 mg o in base a scelte cliniche ed individuali nel
contesto di ogni singola paziente, di sostituire la TAO con
eparina (a basso peso o non frazionata) tra la 6‘ e la 12‘
settimana di gestazione.

This is the largest study on very old patients anticoagulated with Vitamin K
antagonists for the prevention of venous thromboembolism and, for the major
part (75%), for the prevention of stroke because affected by atrial fibrillation.
All studied patients started the anticoagulant treatment after the age of 80
years, and the median age of studied patients was 84 years, ranging from 80 to
102 years

EPICA Study showed that the rate of bleeding complications was
low, suggesting that age in itself should not be considered a
contraindication to treatment.

Condizioni a rischio elevato di complicazioni:
scarsa collaborazione: malattie psichiatriche, stati involutivi cerebrali,
malattie degenerative cerebrali
malattie cardiovascolari: ipertensione arteriosa grave, endocardite
batterica, pericardite, scompenso cardiaco
malattie renali: insufficienza renale grave. In genere gli antagonisti
della vitamina K non richiedono un aggiustamento del dosaggio, è
possibile che in alcune condizioni sia necessario somministrare dosi
ridotte di warfarin per mantenere l’indice INR entro i valori target
malattie neurologiche: recente accidente vascolare non embolico,
aneurismi, traumi SNC
malattie gastrointestinali: rettocolite emorragica, ulcere gastriche
attive, varici esofagee
malattie epatiche: grave insufficienza epatica, ittero colestatico
malattie ematologiche: alterazioni qualitative o quantitative delle
piastrine

Warfarin
L'effetto anticoagulante può essere potenziato da:
allopurinolo, steroidi anabolizzanti, androgeni, agenti antiaritmici
(amiodarone, chinidina), antibiotici, feniramidolo, clofibrato, disulfiram,
ac. etacrinico, glucagone, anti H2, metronidazoli, sulfamidici,
antidiabetici orali, ormoni tiroidei, sulfinpirazone, cloralio idrato, ac.
nalidixico e, in particolare, da: aspirina, fenilbutazone, sulfinpirazone e
FANS.
L‗effetto anticoagulante può invece essere ridotto da:
aminoglutetimide, barbiturici, carbamazepina, colestiramina,
griseofulvina, contraccettivi orali, glucosidi cardioattivi, corticosteroidi,
rifampicina, diuretici tiazidici e preparazioni a base di Hipericum
perforatum (erba di S. Giovanni o scacciadiavoli)

Chronic kidney disease affects up to 10% of the adult

L’outcome primario dello studio era l’incidenza combinata di
stroke, emorragia intracranica ed embolismo arterioso
clinicamente significativo. Rispetto all’aspirina, il warfarin ha
ridotto del 52% l’incidenza dell’outcome primario combinato ed
è risultato ugualmente efficace sia nei pazienti con età ≥ 85
anni che in quelli più giovani. Il farmaco è apparso più efficace
anche quando sono state effettuate analisi per sottogruppi in
funzione di sesso, storia precedente di stroke o rischio basale
di stroke. Il warfarin non è apparso più efficace dell’aspirina
nella prevenzione di eventi vascolari diversi dallo stroke o nei
confronti della mortalità generale.
Sorprendentemente, il warfarin non ha evidenziato alcun
incremento significativo nel rischio di emorragie maggiori
rispetto all‘aspirina.

At 12 months, the primary end point — a composite of death from vascular causes,

P=0.22). The rate of death from any cause was also reduced with ticagrelor 4.5%, vs.
5.9% with clopidogrel (P<0.001). No significant difference in the rates of major
bleeding between the ticagrelor and clopidogrel groups (11.6% and 11.2%,
respectively; P=0.43), but ticagrelor was associated with a higher rate of major
bleeding not related to coronary-artery bypass grafting (4.5% vs. 3.8%, P=0.03),
including more instances of fatal intracranial bleeding and fewer of fatal

There remains uncertainty over optimal antithrombotic
management strategy for patients with atrial fibrillation (AF)
presenting with an acute coronary syndrome and/or undergoing
percutaneous coronary intervention/stenting. Clinicians need to
balance the risk of stroke and thromboembolism against the
risk of recurrent cardiac ischaemia and/or stent thrombosis, and
the risk of bleeding.
The optimal regimen of the anticoagulant and antiplatelet
therapies in patients with atrial fibrillation who have had a
coronary stent is unclear. It is well recognised that "triple
therapy" with aspirin, clopidogrel, and warfarin is associated with
an increased risk of bleeding. National guidelines have not made
specific recommendations given the lack of adequate data. In
choosing the best antithrombotic options for a patient,
consideration needs to be given to the risks of stroke, stent
thrombosis and major bleeding
