How much do MyWellScreen℠ services cost?

• Nominal PCP office visit co-pay may apply per individual/family• HMO & Kaiser Permanente plans not eligible to participate• HSA plans will be charged $900/individual for allergy testing• Allergy Immunotherapy treatment is available to employees and

covered according to individual’s co-insurance, if applicable• Employer may consider gift card, prize, or PTO incentives to

motivate employees to participate to cover co-pay cost
