Definition The piriformis syndrome is a
condition in which the piriformis muscle irritates or compress the sciatic nerve, causing pain in the glutial region, posterior thigh and along the course of the sciatic nerve.
Etiology Prolonged spasm
Inflammation
Shortening
Twisting injury on fixed leg.
Clinical features Radiating pain
Pain while walking , climbing stairs and rotation of trunk.
Tenderness
Weakness of hip abduction and external rotation, knee flexion.
Sensory deficits.
Clinical diagnosis No diagnostic tests, MRI, CT, NCV to
rule out sciatica.
Piriformis test.
Direct palpation test.
Management
Medical management.
Physiotherapy management.
Medical management
NSAID.
muscle relaxants.
Local Injections with a corticosteroid or anesthetic.
Surgery

Gentle , static stretching.
