Introductione offer a national network of more than 200 urgent admission beds, to meet the acute needs of adults requiring rapid access to mental health services including those who may need an

intensive care environment.

Our range of emergency admission services for adults include hospitals which can provide psychiatric intensive care, acute wards, older adult specific services and those for people with a learning disability or autism spectrum disorder.

We offer a one hour response time to referrals, with a decision on a placement no more than an hour after we receive adequate clinical information. Where a bed is not immediately available in the preferred service, we can cross-refer to our national network of hospitals with the agreement of the referrer. All of our services have 24 hour medical cover and admissions can occur 24 hours a day.

We’re focused on length of stay and making sure people move as quickly and safely as possible from our services to the most appropriate and least restrictive environment for them. People can step down through the pathways available within Cygnet services, for example from PICU into an acute ward, or onto a specialist service for ongoing treatment needs.

Our myPath Care Model

myPath is our unique overarching care model which serves to monitor service user engagement levels, manage their records, assess individual progress and formulate a personalised and dynamic care plan with measurable targets. Within our PICU and acute services, myPath is structured to support short term admissions for those individuals who have a mental illness and also require preparation for independent life within the community, or for moving into mainstream rehabilitation.

For each individual, a daily risk assessment is completed, which enables dynamic responses to individual presentation within any 24 hour period enabling immediate support to be provided. Each individual is offered a minimum of 25 hours meaningful activity, which is evidenced and documented within an individualised activity timetable based around understanding mental health needs and reinforcing daily living skills. All individuals’ care pathways are reviewed in full by the multi-disciplinary team on a weekly basis and guided by a dynamic care plan which is closely linked to our specialist outcome tool, the Global Assessment of Progress (GAP). In our PICU and acute services, the GAP allows us to measure an individual’s progress during their admission, plot the positive developments and changes in well-being whilst formulating plans over potential future risks for longer term management. In applying myPath, the team around the individual is able to be responsive in their approach and use myPath to determine length of stay, ensuring that people move as quickly as possible to the most appropriate and least restrictive environment.

myPath is also specifically adapted to care for older adults and people with learning disabilities and autistic spectrum disorder.

PICU

Our national network of PICU services cater for emergency admissions for men and women experiencing an

acute period of mental illness which cannot be managed in an open acute inpatient setting. All admissions are detained under the Mental Health Act. Referrals come to us from all areas including acute services, prison, or via the criminal justice system.

The focus of our PICU services is stabilisation, and moving service users as quickly as possible to the most appropriate and least restrictive environment, whilst avoiding the need for readmission. This is achieved through the provision of a short term intensive assessment and treatment programme, designed to support service users to regain a sense of control and order in their lives so they can ultimately return home.

Service users come to us in crisis, and often present with very complex needs and increased risks that compromise their physical or psychological well-being, or that of others. We can also assess and transfer urgent prison referrals swiftly.

Our teams work closely and collaboratively with service users, their families and the referrer to provide a seamless pathway from referral to discharge.

Care and treatment in our PICU services is person-centred and delivered by a comprehensive multi-disciplinary team. There is emphasis on the use of relational security, safety, recovery programmes and pharmacology in the management of risk, challenging behaviour and enduring mental health disorder. Training, coaching and supervision of staff enable the clinical team to remain knowledgeable and skilled in the provision of modern, appropriate, compassionate and safe care to our service users.

We provide a full programme of therapy and meaningful activity across our services, including on and off-ward therapies and 1:1 and group sessions. Therapeutic interventions and activities offered by our services include:

Our PICU and acute wards work closely together to form comprehensive acute care pathways across the group. Working closely with commissioning teams, we are able to offer pathways out of PICU where clinically appropriate and therapeutically beneficial, across our comprehensive network of specialist services, including acute services, mental health rehabilitation and specialist personality disorder services.

Acute

Our acute inpatient psychiatric services provide a safe and stabilising environment for men and women who are in crisis,

experiencing an acute episode of mental illness and requiring an emergency admission. Service users may be informal or detained under the Mental Health Act (service users must be detained for admission to the enhanced acute services at Cygnet Hospital Stevenage).

The ethos across our services is about assessing and treating service users in the least restrictive environment and planning for discharge in a robust and timely fashion. With a focus on stabilisation, we support service users to manage their mental health, reinforce daily living skills and prepare for independent life back in the community.

Our teams work closely and collaboratively with service users, their families and the referrer to provide a seamless pathway from referral to discharge. Our approach is recovery focused and supported by comprehensive multi-disciplinary teams, contributing to shorter lengths of stay. Our short-term psychological interventions are based on a CBT approach and we also provide specialist substance misuse support and psychoeducation, alongside a comprehensive occupational therapy programme.

Due to the robust environment on our services at Cygnet Hospital Stevenage, here we are able to actively support service users at the challenging end of the acute spectrum, without requiring admission to a PICU service. As part of our corporate Reducing Restrictive Practice strategy, Safewards has been implemented on Branwell ward and Sanctuary Ward.

We work closely with NHS referring teams to keep lengths of stay as short as possible and to ideally discharge people home, or move them back to a lesser restrictive environment.

Older AdultsAcute Admissions / Intensive Support

Our specialist intensive support services for older adults are able to accept emergency admissions for men and

women experiencing acute and enduring mental illness. Service users may be informal, detained under the Mental Health Act or subject to DoLS. Referrals to the service are based on individual presentation rather than age or diagnosis.

The aim of our acute admission intensive support services is to support individuals through their period of crisis and to move on to the least restrictive environment as soon as possible. We have successfully supported people to transition into supported living, nursing home placements, alternative hospital provision and back to their own homes. We can offer transitional support for those who may not be ready for a nursing or care home environment.

Many service users admitted to our services have complex mental health presentations, organic disorders and physical health problems. They may have a long history of requiring support for their mental health and may now also be experiencing a level of cognitive impairment.

Our comprehensive multi-disciplinary teams include RGNs as well as RMNs, meaning we are able to effectively support service users who require a high level of intervention relating to their physical health. We are able to support individuals with urology problems, epilepsy, diabetes, PEG feeding, pain management, and those who may require end of life care.

We are also able to support individuals with dementia and related conditions, including vascular dementia, Alzheimer’s disease, Korsakoff’s syndrome, Pick’s disease and Lewy body dementia.

Learning DisabilityAcute Admissions / Intensive Support

We provide a number of specialist services able to accept crisis admissions for men and women

with a diagnosis of learning disability and associated mental health needs, who may display behaviours which challenge. These intensive support services can accommodate individuals with high dependency needs and requiring short-term admission for assessment and stabilisation.

Our services work proactively to reduce length of stay in these specialist intensive support beds and for service users to transition back to lesser restrictive services or discharge back into the community where possible.

Our comprehensive, well-established multi-disciplinary teams provide a range of

therapeutic interventions carefully selected, graded and adapted to meet individual needs, and underpinned by a Positive Behaviour Support approach. We work together with service users to provide a holistic, comprehensive assessment and care plan, focusing on the identified needs of the individual.

There is emphasis on the use of relational security, patient safety, recovery programmes, and pharmacology in the management of risk and challenging behaviour.

Our services offer adapted environments including multi-sensory spaces and with a focus on communication tools appropriate to supporting the needs of this client group.

Autism SpectrumAcute Admissions / Intensive Support

We provide intensive support and crisis admissions services for men with autism, complex needs and who may

display behaviours that challenge.

The services are specifically designed to meet the needs of this client group. At Larch Court, Cygnet Hospital Colchester, this includes self-modulation sensory facilities, with a programmable sensory menu to accommodate personal preferences, strengths and abilities of each resident. Our staff team reflects the differing needs of each of our residents, with high levels of staff support provided (1:1 or 2:1, 24 hours a day).

Service users are typically referred from community services, secure services, or inappropriate / non-specialist hospital placements.

The aim of the services is to achieve three key targeted outcomes:

> Increase in pro-social presentation

> Progress to future out of hospital placements within lesser restrictive environments
