    Staff EducationStaff Education

    The pharmacy shall provide education appropriate to the

    job requirements for all personnel employed in the

    department.

    A formal orientation will be provided and documented for

    all employees.

    Supportive personnel will receive formal training in any

    specialized area and will be certified in this area before theywill be allowed to work.

    The pharmacy shall maintain records of all in-service

    programs.

    Sample medication Those not-for-sale and

    non-priced-items which are donated to thehospital. Free medical samples include

    medications, cosmetics, ready to feed liquid

    milk, etc.

    Free Medical Samples must be stored only in the pharmacy and labeled

    as Sample Medications.

    Pharmacy Supervisor is responsible for monitoring the sample

    medication and ensure proper utilization according to policy.

    Sample medication on hand may be used in an emergency situation on

    physicians order :

    When a new drug which is approved to be included into Arrayan Hospital

    formulary has not yet been procured and is urgently needed for a patient;

    As a replacement of an expired drug which is urgently needed for apatient.

    As soon as the drug procured and becomes available, dispensing of sample

    medication is prohibited.

    Prescribing and Ordering PolicyPrescribing and Ordering Policy

    concerns appropriate ordering/prescribing practice

    from all authorized licensed physicians in the

    clinics and in-patient setting.

    Medication order should include:

    date and time ordered, diagnosis, allergy history,

    drug name, dosage in metric units, weight ifpediatric patient, route of administration,

    frequency, and start and stop dates.

    To deliver medications to patient in an efficient

    and timely manner.

    To ensure that all medications are administered

    and recorded accurately.

    To prevent possible adverse drug reactions by

    monitoring patient allergies and drug interactions.

    well as encouraging the use of the hospital

    formulary of the approved drugs.

    Verbal Order - An order accepted by a pharmacist in an

    emergency situation from a physician

    Telephone Order - An order from a physician accepted

    by a pharmacist when the patients condition does notrequire the physicians presence in making a medical

    assessment such as PRN orders.

    Emergency Order- Alife-threatening situation, such as

    an acute change in vital signs or patient status that will

    result in rapid deterioration if not treated immediately

    when the registered pharmacist does not need a written

    order to dispense medication

    system for the following:

    Indications and/or contra-indications. Allergies.

    Drug-food interactions

    Drug-disease interaction

    Duplication of drug therapy.
