This student handbook provides information about the Peter Ballantyne CreeNation administration of the Post Secondary Student Support Program_ It isissued under the authority of the Chief and Council of the Peter Ballantyne CreeNation.

All students are REQUIRED to read and retain this handbook for referencepurposes. It outlines the terms and conditions by which the student and thePBCN will work together in furthering the individual and collective educationalgoals of band members belonging to the Peter Ballantyne Cree Nation.

The Peter Ballantyne Cree Nation has adopted a Post Secondary StudentSupport Program Policy Manual which is available for review on request.

1.0 INTRODUCTION

1.1 The Peter Ballantyne Cree Nation asserts that Post Secondaryeducation is a Treaty Right and nothing in its acceptance ofadministrative responsibility for the Post Secondary StudentSupport Program (PSSSP) abridges that right nor the trustresponsibility of the Government of Canada towards TreatyIndians.

1.2 The Post Secondary Student Support Program supports Treaty /Status Indian students in pursuing post secondary studies inrecognized and authorized post secondary institutions.

1.3 The objectives of the Post Secondary Student Support Programare to encourage and support prequalified, eligible students toacquire university, technical institute, college or professionalqualifications so that they can become economically self-sufficientand develop their individual potential to further their own aims andcontribute to the progress of the Indian community, the Provinceand Canada.

2.0 DEFINITIONS

2.1 "Treaty/Status Indian" means a person whose name has beenentered in the Indian Register in accordance with the Indian Actand/or a person recognized as possessing Band membershipstatus in accordance with the laws of the Peter Ballantyne CreeNation.

2.2 "Post Secondary Education" means a program of studies, offeredby a recognized post secondary institution, for which completion ofsecondary school studies (Grade 12) or its equivalent is aprerequisite.

2.3 "Program of Studies" includes all post secondary programs, at leastone academic year in duration, leading to a certificate, diploma ordegree. Programs e.g., pre-Law, less than one academic yearwhich are prerequisites to post secondary programs of at least oneacademic year in duration are included.

2.4 "Post Secondary Institutions" are degree, diploma, and certificategranting institutions which are recognized by provincial authoritiesand include educational institutions affiliated with, or delivering

accredited programs by arrangement with, a post secondaryinstitution. The Post Secondary Office will maintain a national list of

recognized Canadian post secondary institutions.

2.5 "Public Institution" is a Post Secondary institution which receivesthe majority of its funding from Federal and Provincialgovernments.

2.6 "Private Institution" is a Canadian or foreign Post Secondaryinstitution which receives the majority of its funding from sourcesother than governments.

2.7 "Full time Students" and "Part time Students" are defined by thePost Secondary institution with the provision that full time studentsare required to carry a minimum of 12 credit hours per semester.

2.8 "Academic year" is as defined by the Post Secondary institution,but will not be less than eight months in duration.

2.9 "Semester" refers to a part of the academic year, as defined by thePost Secondary institution. Semesters usually cover the periodsfrom September to December, January to April, and May to August.

2.10 "Dependent Spouse" means a person who is married to the studentor a person who has lived with the student as a husband or wife fora period of at least one year prior to application for educationalsupport. This person is dependent upon the student and does notreceive income in excess of the level of income allowed for adependent spouse by Revenue Canada.

2.11 "Dependent" means a person other than a spouse, who isdependent upon the student as defined by Revenue Canada andwho does not receive income in excess of the level of incomeallowed for a dependent by Revenue Canada, with a residencyprovision whereby a "dependent" has to be living with and besupported by the student.

2.12 "CEGEP" is an abbreviation of College d'enseignement general etprofessionnel. CEGE Ps operate in Quebec only.

2.13 "Immediate family" includes spouse, mother, father, sister, brother,son, daughter, mother-in-law, father-in-law, step-mother, step-father, parent surrogate, grandparent, or any relative permanentlyresiding in the student's household or with whom the studentresides.

3.0 ELIGIBIUTY FOR PSSSP SUPPORT

3.1 The student must be a member of the Peter Ballantyne CreeNation.

3.2 Students who have transferred to the Peter Ballantyne Cree Nationfrom another First Nation must wait for a period of one year fromthe date of transfer before being eligible for PSSSP funding.

3.3 The student must be enrolled in a program of at least 8 monthsduration that leads to a recognized certificate, diploma or degreeand which normally has grade 12 as an entrance requirement.

3.4 Support will be provided for within the limits of funds available inaccordance with PBCN funding arrangements. If demand forfunding exceeds availability, applications will be deferred accordingto priorization rules set out in Section 4 and in PSSSP OperatingGuidelines.

3.5 Applications must have been received at the PBCN PostSecondary Centre by the deadline dates set in PSSSP OperatingGuidelines. (See Section 4.0 below)

4.0 PRIORITIES FOR APPROVAL OF APPLICATIONS

Peter Ballantyne Cree Nation reserve / community members will be givenpriority over members from other areas. This will include: DeschambaultLake, Denare Beach, Kinoosao, Pelican Narrows, Prince Albert, SandyBay, Southend and Sturgeon Landing.

Priorities for approval of applications for student living expenses shall bebased on the following categories with Group 1 being the first set ofapplications to be approved, Group 2 to be the second considered, Group3 to be the third considered and so forth.

Only applications received within the deadline dates will be considered forthe current intake; those received late will be considered early applicantsfor the next intake. DEADLINE DATES ARE: May 31' for September(Fall Semester) enrolment; November 15 th for January (Winter Semester)enrolment; March 31' for Spring or Summer school.

These categories may be modified in PSSSP Operating Guidelines inaccordance with the types of applications received for any specific intakeor to accommodate specific PBCN initiatives.

Students will be notified of any changes.

Group 1 - These Categories will be considered together.

1.1. Continuing students with minimum grade averagerequirements for program completion.

1.2. Regular grade 12 students entering regular Post Secondaryprograms (not UEP or Pre-Tech).

1.3 Students who were accepted as partially funded studentsand attended during the previous year.

Group 2 - These categories will be priorized separately within thegroup.

2. Students in professional degrees as defined by our policyand scholarship criteria.

2.1 Students entering higher program level.2.2 Combination degrees such as B.A.-LLB or B.A.-M.D.,

B.ED-M.ED.

3. Returning students, returning after a leave of absence forauthorized medical or personal reasons, with graderequirements similar to the continuing students in Group 1.

3.1 Returning to the same program.3.2 Returning to a different program.(Note: decision may be based on individual circumstances)

4. Other students e.g. mature students, who can go directlyinto Post Secondary programs.

4.1 With GED 124.2 Without GED 12

5. UEP and/or Pre-Tech students.5.1 Students with grade 12.5.2 Students with GED 12.5.3 Students without grade 12 or GED 12.

6. Students enrolled in a second professional degree andchanging disciplines.

7. Students attending private institutions.

8. Re-admitted "required to discontinue" students.

9. Students changing programs without completing anyprogram because of poor grades and poor attendance.

10. Students in programs several years with no certificate.

5.0 LIMITS OF SUPPORT

5.1 Support for travel and living expenses will be provided for fourlevels of Post Secondary education and limits are placed on theduration of support according to the level of program the student isenrolled in.

Level I

Level II

Technical Institute, CEGEP or Community College program:1 to 2 academic years depending on the institution's normalprogram duration.

University Undergraduate Programs (i.e. - B.A., B.Ed., B.Science): Academic years based on the university's normalprogram duration.

Level III a) University Professional Programs (i.e. - M.D., LL.B): Durationof support will be in accordance with the university's normalprogram duration.

b) Graduate Programs (i.e. - M.Ed., M.A., Ph.D.): Duration ofsupport will be in accordance with the university's normalprogram duration, however, reasonable limits may beestablished on the basis of individual need. All applicationswill be dealt with on an individual basis.

Level 1111

University or College Entrance / Preparation programs:maximum of 2 semesters.

5.2 Funding will be considered when a student drops out of a Level IIprogram and goes to a Level I program. If a student resumes LevelII studies, the previous time spent at Level II will be counted forsupport purposes.

5.3 Duration of assistance will be in accordance with the minimum full-time requirements of the program as defined by the PostSecondary institutions.

5.4 Students may be assisted for up to one additional academic year of8 months per level for authorized and documented medical,academic or personal reasons. In cases where the need for anadditional year is academically based, a letter from the dean or thedepartment head will be required in support of the application forfunding. Those applicants who request more than one additionalacademic year will be referred to the Funding Appeal Process priorto approval of additional funding.

5.5 Level III funding arrangements will be dealt with on an individualbasis and arrangements will depend upon the program andinstitution; however maximum cumulative total years of support asoutlined in 5.1 will be adhered to.

5.6 Students who transfer programs more than once require approvalof the Funding Appeal Board prior to approval of additional funding.

5.7 Students who have completed a Level II program, with or withoutsupport from this program, are ineligible for Level I programsupport.

5.8 Students who have completed a Level III program, with or withoutsupport from this program, are ineligible for Level I or Level IIprogram support.

5.9 Where students change programs within one of the levels, theacademic time used for each prograrn within this level will becounted for support purposes.

5.10 Students who were supported under the previous E-12 programand who have been in continuous attendance since that time maycontinue to have their limits of support calculated using the "studentmonths" provisions of E-12 under certain conditions identified in thepolicy

5.11 Students who become eligible for support and who have previouslycompleted a portion of Post Secondary studies without supportfrom this program will receive support for the balance of theirprogram of studies in accordance with 5.3 and 5.4.

5.12 Support will be approved for a university academic year which isnormally eight months. On-campus students may be supported inintersession / summer school if funding permits. Students for whomintersession or summer school attendance is a compulsory aspectof their program will receive normal levels of support for thesesessions. Support for students attending other Post Secondaryinstitutions, e.g. SIAST, will be for the normal academic year as setby the institution.

5.13 Tuition and book support will be provided to eligible studentsenrolled in all four levels.

5.14 Any student who quits or withdraws from classes must obtainauthorization from the Post Secondary Office prior to formalcancellation of classes. Failure to receive this authorization willresult in a one year waiting period prior to funding being approved.This is effective beginning April 01, 2002.

5.15 Any student that is required to discontinue (RTD) or is expelledfrom their program of studies by their University or Institution will besubject to a two year waiting period prior to funding beingapproved. This is effective beginning April 01, 2002.

5.16 Students must reside in the community where the institution islocated. Fiscal resources will not be provided for students whocommute. The exception to this will be programs especiallydesigned for this purpose.

517 PBCN reserve / community education boards are expected toexpend fiscal resources for the professional development orupgrading of skills of their staff. PSSSP funding will be expended ifnecessary.

5.18 Students will not be funded for a second undergraduate degreeuntil the PSSSP is out of deficit unless courses are taken part timewithout living allowances. The few students currently in this positionare grandfathered conditional upon satisfactory academic success.

6.0 TYPES OF SUPPORT AND ALLOWANCES

6.1 Tuition and Book Support

a) Includes student's fees for registration, tuition, tutorials,initial professional certification and examination fees, andreasonable cost of books and supplies which are listed asrequired by the Post Secondary institution. The Director hasthe flexibility to reasonably adjust tuition, books and suppliessupport to meet the needs of students.

b) Students considering attending private or foreign PostSecondary institutions should note that they will be eligiblefor tuition support at no higher than the rate charged by thepublic institution nearest to the student's Canadian place ofresidence which offers a comparable program at the time ofapplication.

c) Students enrolled in foreign institutions will be eligible fortuition support at the actual tuition rate charged by theforeign institution where no comparable program is availableat an institution in Canada. The onus for showing that nocomparable Canadian program is available will be on thestudent.

d) Students who receive scholarships or bursaries to attendPost Secondary institutions will be eligible for funding fromthe PSSSP as required to meet tuition, travel or living costsnot fully covered by the scholarship or bursary. Allregulations governing eligible students, program andinstitutions must be met. Each case will be consideredindividually.

e) Students are responsible for library fines, and droppedclasses and withdrawal fees. Students must make sure thatall payments are made before registration.

6.2 Travel and Relocation Support

a) Students will be eligible for travel support for one round tripper semester for the student and dependents if theirpermanent address is different from their study address, at arate established in PSSSP Operating Guidelines. The ratefor 2002-2003 will be 25 cents per kilometer for use ofprivate vehicles. Out of province students will receive travelsupport at the same rate they would receive if they wereattending the provincial public Post Secondary institution

nearest to their permanent homes offering a comparableprogram. A $10.00 meal allowance per student and claimeddependant will be provided each round trip.

b) Students on summer programming will not be eligible forsummer travel.

c) New students and graduates may be eligible for areasonable relocation allowance to move from the student'shome community to the community where the PostSecondary institution is located. This would be payable atthe start of a student's program and upon graduationproviding the student is returning to his or her originaladdress. The rate payable on each occasion would be setannually in Operating Guidelines.

d) Approval for a relocation allowance will be dependent uponthe student providing verification from his or her band officeor landlord of the change of address and a cleardemonstration of need. The student must apply for thisassistance after relocation.

e) Relocation allowance will be payable as per the followingrates for first time students to assist in relocation from theirhome reserves to the centre where the post secondaryinstitution is located. Relocation assistance will be payableto graduates of post secondary programs who are returningto their original reserve or after final graduation from theprogram. Students must apply for his relocation allowance inwriting to the PSSSP Office.

6.3 Part time Studies Support

a) When a student in attendance at a Post Secondaryinstitution studies for what is deemed less than full time bythe institution which offers the program, that student will beeligible for financial assistance for books, supplies andtuition fees only.

b) Students enrolled in correspondence or other distanceeducation courses and who do not qualify as full time or parttime students as defined above, may qualify for tuition feesand book assistance as required by the institution.

c) Part time students must follow the same admissionprocedures as full time students.

6.4 Living Allowance

a) The living allowance rate structure and entitlement will beset annually in Operating Guidelines. For the current yearthe rates are attached to this Handbook as Appendix A.

b) Living allowance rates may be supplemented to reflect theadded costs of rent in those areas/cities designated as highcost rent areas in the annual Canada Mortgage and HousingCorporation rental survey.

c) Required school registration and book allowances will bepaid to a student on behalf of dependents attending non-band schools in the community where the parent isattending a Post Secondary institution. The rates are set inOperating Guidelines. For 2002-2003 they will be $100/yearfor Division I & II students; $120/year for Division HI & IVstudents.

d) Full time students are eligible for reimbursement of childcare expenses for eligible dependents at a rate of 50% ofactual expenses to a maximum of $200.00 per montheffective July 1, 1999.

e) Required daily bus transportation allowance will be paid to astudent on behalf of dependents who must use publictransportation to attend Division Ill & IV schools in thecommunity where the parent is attending a Post Secondaryinstitution. Rates will be set at the normal monthly studentbus pass rate for the city where the student resides.

6.5 Allowances for Special Needs Students

Students with special needs (i.e. physically disabled)requiring additional assistance related to their PostSecondary programs shall have their requests consideredon an individual basis. Supporting documentation from amedical physician is required.

6.6 Contingency Assistance

a) Contingency assistance may be paid, subject to availabilityof funds, to eligible full time PSSSP students to defray costsassociated with emergency or special situations not coveredby other provisions of this policy. Contingency assistancemay be provided for situations that include, but are notnecessarily limited to, the following: graduation expenses;damage and hook-up deposits (refundable); practicum /internship assistance; program orientation; internshipseminars; emergency travel due to death or critical illness inthe immediate family; rental of specialized equipmentdesignated by the Post Secondary institution; specializedcounseling services; or other costs of an extraordinary orunforeseen nature.

6.7 Graduation Assistance and Graduation Bursary

a) Graduating Post Secondary students are eligible forassistance and bursary based on two funding levelsaccording to the length of the program and type of certificateas listed below:

It is the responsibility of the graduating student to submit awritten request to the Post Secondary Office, when apply forgraduation assistance and bursary. All requests have to beaccompanied with a supporting letter from the Institutionindicating that the graduating student has in fact met all thesuccessful requirements for convocation as per theInstitutions policies and guidelines. All requests should besubmitted 3 weeks prior to graduation day.

7.0 INCENTIVES AND SCHOLARSHIPS

The PBCN, in conjunction with the PBCN's Education Board, may, subjectto the availability of funds, provide scholarships as incentives, toencourage strategic studies, or to recognize academic or personalachievement. Any incentives and scholarships programs established byPBCN will be publicly advertised among eligible students.

Have demonstrated a high level of academic achievement (75% &over) Have demonstrated leadership commitment and dedication intheir program of studies.

b) Four (4) Personal Performance and Achievement Scholarships forUniversities:Year 1 $ 750.00Year 2 $ 750.00Year 3 $ 750.00Year 4 $ 750.00

Eligibility:Peter Ballantyne Cree Nation Post Secondary Students must be afull time student enrolled in a program of study at a University. E.g.University of Saskatchewan, University of Regina.

Have demonstrated a high level of academic achievement (75%orover).

c) Three (3) Personal Performance and Achievement Scholarships forCommunity Based Programs:

University Entrance Programs:

Human Justice - Prince Albert $ 500.00Community UEP (per course) $ 100.00

Eligibility:Peter Ballantyne Cree Nation Post Secondary Students that wereenrolled in one of the community based programs as listed above.

d) One (1) Academic Excellence Scholarship for Institutions andUniversities.

Any year $ 1,000.00Eligibility:Peter Ballantyne Cree Nation Post Secondary students that wereenrolled in an Institute or University and have achieved academicexcellence (85%).

e) One (1) Strategic Studies Scholarship

Any year $ 1,000.00Eligibility:Peter Ballantyne Cree Nation Post Secondary Students that wereenrolled in a program which will directly contribute to theadvancement of self-sufficiency of our First Nation. Programs ofstudy which are encouraged. E.g. Law, Medicine, Sciences andothers as outlined in operating guidelines.

Successful completion of academic year or program in the previousacademic year.

Total scholarships to be awarded for the previous academic year

8.0 ACCOUNTABILITY BY STUDENTS AND ADMINISTERING ORGANIZATION

8.0 Students will be closely monitored for attendance by the PSSSPstaff. All students are required to submit a DECLARATION OFFULL TIME STUDENT STATUS form to the PSSSP office by the15 th of every month for the academic period for which their fundingwas approved. Failure to do so will result in an automatic holdbeing placed on the following month's living allowance. The PostSecondary Office will distribute this form to all new and continuingstudents.

8.1 Every effort will be made by the Post Secondary Office to recoveroverpayment to students who misuse funding by not fulfilling theterms of the policy, or who misrepresent their marital, dependent orprogram status on application.

8.2 When misuse of Post Secondary funds is discovered, the PostSecondary Office will adopt the following procedures:• write a letter to the individual stating the findings;• If the student is, in fact, found to be a misuser of funding, the

Post Secondary Office will suspend that student from furtherfunding or take other corrective action.

• Arrangements, satisfactory to the Post Secondary Office, forthe 100% repayment by the student of overpayments receivedwill be a condition of any future funding.

8.3 Any student who quits or withdraws from classes must obtainauthorization from the Post Secondary Office prior to formalcancellation of classes. Failure to receive this authorization willresult in a one year waiting period prior to funding being approved.This is effective beginning April 01, 2002.

8.4 Any student that is required to discontinue (RTD) or is expelledfrom their program of studies by their University or Institution willbe subject to a two year waiting period prior to funding beingapproved. This is effective beginning April 01, 2002.

8.5 An annual report and financial review of Post Secondary funds willbe made available on request to current Post Secondary students.

9.0 FUNDING APPEALS PROCESS

9.1 Every student has a right to appeal the Directors decisionregarding funding, however, when a student application has beenrefused because available funds are fully committed, this appealprocess will not be considered.

9.2 The matter, if unresolved after discussion with the counselor, canbe appealed to the PBCN Director of Education who must reply inwriting within 7 days.

9.3 If misuse of funding is suspected, the student will be notified inwriting in accordance with subsection 8.2 above. If there is noresponse, funding will be terminated. The student may appealwithin 14 days.

9.4 If the issue is still not resolved, a third party Appeal Board will hearthe student's appeal. The Appeal Board's decision is full and final.

9.5 The student has a right to attend the appeal hearing in person orby a designate, but no professional legal counsel will be permittedat appeal hearings.

9.6 The Appeal Board, which will be appointed by the PBCNManagement Committee shall consist of: a member of the ChiefsCouncil; a Band Director or Director of Education; an Elder; amember of a Band / First Nation Education Committee; and astudent representative selected from a list of at least 3 studentssubmitted by the appealing student. No representation shall beappointed by the PBCN Management Committee to the AppealBoard from the community of which the student is a member,however, advice from the appellants' band will be obtained prior tothe Appeal Board hearing by the PBCN Director of Education orhis/her designate. An effort will be made to select Appeal Boardmembers who are knowledgeable about the PSSSP. The AppealBoard will meet within 14 days of its appointment to hear and ruleon the student's appeal.

9.7 Policies and Operating Guidelines should be strictly followed.

1 0.0 GRIEVANCE PROCEDURES

If a student, or a group of students, has a grievance regarding the PostSecondary Office that is riot about funding, the grievance procedure shallbe as follows:

10.1 The student(s) submit by registered mail a written report andsupporting documentation to the PBCN Management Committee.The students will send copies to whomever they feel requiresknowledge of the situation expecting that appropriate confidentialitywill be maintained.

102 The PBCN Management Committee has 14 days from the receiptof the report to make their decision. It is understood that a decisioncan be rendered / recommended in less than 14 days.

10.3 If the grievance is not resolved to the satisfaction of the student(s),the PBCN Management Committee must submit the grievance tothe PBCN Education Board. Their decision, which is final for allparties, must be made at their next scheduled meeting.

10.4 If, at any of these steps, the student(s) presence is requested, thestudent's travel expenses will be reimbursed or covered accordingto Operating Guideline rates.

11.0 OBUGATIONS OF STUDENTS

11.1 It is understood that the PSSSP is intended to provide support forstudents with a serious intent to succeed in Post Secondaryeducation. Expectations are that the student will attend classesregularly, submit assignments as required by their programs, andcommit themselves to their studies. Students are expected toconduct themselves so as to bring credit to their peers and tothemselves. More detailed obligations of students and theadministering organization may be spelled out in OperatingGuidelines and related documents. These will be discussed withstudents on an individual basis or at orientation sessions.

11.2 Students will be dealt with fairly and equitably under the PSSSPand are expected to fairly and equitably apply themselves to theirduties as students.

11.3 In the event that the student, through his or her deportment orperformance, indicates a distinct lack of possibility of success in hisor her studies, and if this is reinforced by program officials at theinstitution being attended, it maybe necessary to reallocatefinancial resources by terminating the student's support andreallocating funds to other students in that show a greaterprobability of success.

11.4 Students have access to the appeal and grievance processesoutlined in Sections 9 and 10 of this student handbook.

12.0 OBLIGATIONS OF ADMINISTERING ORGANIZATION

12.1 The Post Secondary Office will clearly set out obligations to and ofstudents in Operating Guidelines and related documents such asthis Student Handbook, student contracts or such other means asmay be mutually advantageous to the students and the PostSecondary Office.

12.2 The Post Secondary Office will provide orientation seminars fornew and continuing students at each of the four activity centers (LaRonge, Prince Albert, Saskatoon, Regina) during the first month ofeach semester in which there is an intake of new students.

13.0 STUDENT REGISTRY

13.1 The Post Secondary Office will maintain documentation withrespect to student identifying information, academic record andsupport provided. Disclosure of academic information requires theinformed consent of the student. Personal information is subject tothe terms of privacy provisions.

13.2 The Post Secondary Office will maintain a student registry forstatistical submission purposes to DIAND Education Branch inOttawa as required by the Post Secondary education data andinformation system.

14.0 POLICY REVIEW

1 4.1 Student representatives will be asked to participate in PolicyReview activities. This involvement is detailed in the Policy Manualand the Operating Guidelines. Students will be asked, during theorientation sessions, to appoint or elect a student representative(s).

1 5.0 APPLICATION PROCESS

15.1 Applying to the Post Secondary Institution.

a) Applying for admissions to the Post Secondary institutionand program of choice is entirely the STUDENTSRESPONSIBILITY, furthermore, any expenses incurred willbe at the student's expense, all students are subject topolicies, provisions and regulations as set by the PostSecondary institutions. Students can obtain guidance fromtheir school counseling staff or by contacting the PostSecondary Office.

15.2 Applying for PSSSP Funding

a) Deadline Dates

Fully completed application forms and relateddocumentation must be received at the Post SecondaryOffice by the following deadline dates:

• For Fall Semester (September) commencement -MAY 31st

• For Winter Semester (January) commencement -NOVEMBER 15th.

• For Intersession and Summer School commencementMARCH 315T

All students must reapply for funding on an annual basis.

b) Only signed original applications forms will beconsidered for funding. No faxed applications will beaccepted.

c) Effective October 1999, late applications will not beaccepted.

d) Documentation required prior to final approval of thestudent's application for support will generally include:Letter of acceptance from the Post Secondary institution;copy of student's Treaty status card; copy of hospitalizationcard for self and any dependents, if applicable; transcript ofmarks for previous year (Grade 12 marks for newapplicants); transcripts of marks for any training coursestaken. Additional documentation may be required in specificcases.

e) Obtaining PSSSP funding application forms: These will bemade available through your School Guidance Counselor orby contacting the Post Secondary Office.

15.3 Notification

a) Applicants will be advised by the Post Secondary Office assoon as possible about the decision made regarding theirapplications.

b) Students who receive additional information such asacceptance letters by their Post Secondary institution or anyother relevant information such as transcript of marks shouldimmediately provide the Post Secondary Office with originalswhich will be returned to the student upon request.
