Personalisation

Personalisation of social care means moving away from traditional provision where people are fitted into a limited set of available statutory services. It involves considering a person as an individual with aspirations, as well as needs, and a circle of family, friends and other resources and support mechanisms around them. It takes into account the individual’s personal responsibilities and their own resources, as well as those available through statutory and other services, to meet their needs in the best way possible

Personalisation

• Universal services, ensuring general support and services are available to everyone locally including things like transport, education, health, housing and access to information and advice.

• Early intervention and prevention, helping people to stay independent for longer

• Choice and control through self directed support with services that meet people’s needs rather than people fitting with the limited services on offer.

• Social capital, building a society where everyone has the opportunity to be part of the community and experience the care that can come from family and friends. Carers need to be recognised and supported in their role.

• Co-production is a key part of this agenda• Real change can only happen by engaging users and

carers throughout • Personalisation means empowering people to develop

solutions that work for them

‘ our processes will be co-produced, co-developed, co-evaluated and will enable to recognise that real change will only be achieved through the participation of users and carers at every stage. We recognise that sustainable and meaningful change depends significantly on our capacity to empower people who use our services and our staff’

Co-Production
