PERSONS WITH DISABILITYTEMPORARY PARKINGPLACARD ISSUANCE

In an effort to better serve the needs ofPennsylvanians and the healthcare professionalswho care for them, PennDOT has expanded itsPersons with Disability Temporary Parking PlacardIssuance Program to allow the immediate releaseof temporary parking placards by select healthcareproviders. As a result, persons with qualifyingdisabilities can now obtain a temporary parkingplacard directly and instantly from an approvedhealth care provider.

This partnership with health care providers allowspersons with temporary disabilities to complete aplacard applications, obtain the requiredcertification of disability and receive their parkingplacard at the health care facility all in one visit.This replaces the process of a patient downloadingand printing an application, making a trip back tothe health care provider to obtain the certification,mailing the completed applicationto PennDOT and finally,waiting for the placardto arrive in the mail. If you participatein the TemporaryParking PlacardIssuance Program,your qualifiedpatients canleave yourfacility witha placardthey canbegin usingthe sameday.

WHAT TYPES OF FACILITIES CANPARTICIPATE IN THE PROGRAM?

PennDOT has identified several types of healthcare facilities who can enroll in the TemporaryPlacard Issuance Program.

They are:• Hospitals• Medical Specialist Centers• Physical Therapy Centers

HOW DOES A FACILITY ENROLLIN THE PROGRAM?

The process begins by filling out PennDOT’s FormMV-145P, “Application for Enrollment/Change inTemporary Placard Program for Health CareFacilities,” available on the PennDOT website.PennDOT will review the application to confirm allprogram requirements are met by the facility. Aftercompleting training related to placard applicationsand handling of inventory, approved facilities areassigned a site number by PennDOT and receive asupply of temporary placards and applications.

HOW DOES EACH FACILITYRECEIVE TRAINING?

When a facility’s application has been reviewedand approved, PennDOT will provide training eitherby telephone or webinar. Each newly approvedfacility will also receive a training documentcovering all aspects of the program, including:

• The Placard Application

• Issuance and Instruction to Patients

• Returning Expired/Unused Placards

• Ordering New Placards

• Securely Storing Placards

HOW DOES THE PLACARD ISSUANCEPROCESS WORK?

After the facility is approved to participatein the Temporary Placard IssuanceProgram, and receives a supply oftemporary placards and applications,several simple steps are followedto issue placards to qualifiedpatients:

1. Determine if thepatient qualifies fora temporary placard,by reviewing theeligibility requirementson the back ofForm MV-145H,“Temporary Personswith Disability ParkingPlacard Application,”available only toauthorized healthcare providers.

2. If the patient doesqualify for a temporaryplacard, the health careprovider follows these steps:

a) Complete Form MV-145H, and obtain thepatient’s signature on the form.

b) Record the placard number andexpiration date of the placard issued onthe patient’s Form MV-145H.

c) Hand the temporary placard to thepatient with information about properuse, and how to recertify if the placard isneeded for a longer period of time.

d) Email a copy of the completed FormMV-145H to PennDOT so that theplacard and patient information can beimmediately recorded in PennDOT’sdatabase.

MAINTAINING SUPPLIES, RETURNINGPRODUCTS, AND OTHER

REQUIREMENTS

Any changes to the health care facility or thepersonnel responsible for operation of theTemporary Parking Placard Issuance Program mustbe reported to PennDOT on Form MV-145P. Somechanges, such as relocating or renovating thesecure location, will require new photographs ofthe secure location to be submitted to PennDOT.
