PATIENT PORTAL INSTRUCTIONS CREATING AN ACCOUNT On the same day of your visit to our office, please check your inbox for an e-mail from “PM Urgent Care Patient Portal” with your username, a link to the portal, and instructions to set it up. Please note that the username will be unique for each patient. You will need your phone number and the patient’s date of birth to complete the setup and it must match the information provided for the patient’s chart. The e-mail subject line will be “Portal login information from your doctor's office”. If you don’t see the e-mail, check your spam/junk folder. You must set up your patient portal in order to view your test results. IMPORTANT – the patient portal is different from your PM Pediatrics Anywhere telemedicine account. Your test results will NOT be available on PM Pediatrics Anywhere. ACCESSING THE PORTAL After completing the setup of your account, you can access the portal by visiting pmpediatrics.com and clicking the Patient Portal button on the homepage, or by downloading the Healow app to log in to your account. When prompted, use our practice code: HAFHBD. If you cannot remember your password after setting up your account, click on the “trouble logging in” button on the portal homepage to recover the portal account. VIEWING TEST RESULTS Via desktop: New labs will show on the patient’s dashboard/ Home screen. To view lab results a second or third time, click Medical Records on the left side of the portal and choose Lab/ Diagnostic Reports. Letters and test results can only be printed from desktop. Via Healow app: To view labs, click My Records on the wheel and then Results on the next screen.

    El mismo día en que usted viste nuestra oficina, recibirá un correo elec-trónico de parte de “PM Urgent Care Patient Portal” con su nombre de usuario, un link al sitio del portal, e instrucciones para configurar su portal. Favor de notar que el nombre de usuario será único para cada paciente. Necesitará su número de teléfono y la fecha de nacimiento del paciente para completar la configuración y cada uno debe coincidir con la infor-mación en la historia clínica del paciente. La línea de asunto del correo electrónico será “Portal login information from your doctor's office.” Si el correo electrónico no está en su buzón de entrada, revise su carpeta de spam. Debe configurar su portal para ver los resultados de su examen.

    IMPORTANTE - El portal del paciente es diferente de la cuenta de teleme-dicina de PM Pediatrics Anywhere. Los resultados de su examen NO estarán disponibles en PM Pediatrics Anywhere.

    Después de completar la configuración de su cuenta, puede acceder al portal visitando pmpediatrics.com y haciendo clic en el botón que dice “Patient Portal” en la página de inicio, o a través de la aplicación Healow y ingresando a su portal de esa manera. Cuando se le solicite, utilice nuestro código de práctica: HAFHBD.

    Si no puede recordar su contraseña después de configurar su cuenta, haga clic en el botón “trouble logging in" en la página de inicio del portal para recuperar acceso a su cuenta.

    A través de la computadora: nuevos laboratorios se mostrarán en la pan-talla de inicio del portal. Para ver los resultados de laboratorio por segun-da o tercera vez, haga clic en “Medical Records” en el lado izquierdo del portal y seleccione “Lab/Diagnostic Report.”

    A través de la aplicación Healow: para ver los resultados, haga clic en “My Records” en la rueda y luego en “Results” en la siguiente pantalla.
