  • Interactive Survey System (ISS)

  • What Is the ISS?The ISS is the web-based PCMH practice self-assessment or survey.The practice uses the ISS (also called the Survey Tool) for:Entering responses for each item (Standards/Elements/Factors) Attaching documents and providing text to support the responses

  • Using the ISSFor each Element, you MUST:

    Add a Document

    Link the Document

    Add Supporting Text (if necessary)

    Upload the Document

  • Using the ISS Demonstration

  • CautionsIn order to upload documents, you must first complete and submit your PPC-PCMH APPLICATION (different from the Survey Tool). This was purchased when you obtained your Survey Tool. There is a separate log-in process. If you forgot to order one of these free applications when you got your survey (or cant find the email with your log-in info), you MUST contact NCQA and get one ASAP.

  • Cautions (continued)After you have added, linked, and uploaded your supporting documents AND completed your survey, you must submit your survey. The application process is not complete until youve submitted:Your completed APPLICATION (demographic practice info)Your completed SURVEY (PCMH answers & supporting documentation)

  • What Is the Record Review Workbook?Elements PPC 2C, 2D, 3D, 4B Require medical record abstraction of dataNeed % of patients meeting the element (based on a numerator and a denominator)

    Two methods to collect and submit patient dataMethod #1 - report from the electronic system (EHR)Method #2 - Record Review WorkbookExcel workbook included in the ISSTool to identify a sample of patients and abstract data needed for Elements 2C, 2D, 3D, 4B
