RESIDENT RIGHTS

All Park Place Estates residents have the following rights: 1. To be treated with consideration, respect and full recognition of personal dignity and autonomy. 2. To receive care, treatment and services which are adequate and appropriate. 3. To receive respect and privacy in the resident’s medical care program. Personal and medical records shall be confidential, and the written consent of the resident shall be obtained for their release to any individual, including family members, except as needed in case of the resident's transfer to another health care facility or as required by law or a third-party payment contract. 4. To be free from mental and physical abuse. 5. To receive from the manager and staff of the program a reasonable response to all requests. 6. To associate and communicate privately and without restriction with persons and groups of the resident’s choice, including the resident advocate, on the resident’s initiative or on the initiative of the persons or groups at any reasonable hour. 7. To manage the resident’s own financial affairs unless a resident’s legal representative has been appointed for the purpose of managing the resident’s financial affairs. 8. To present grievances and recommend changes in program policies and services, personally or through other persons or in combination with others, to the program’s staff or person in charge without fear of reprisal, restraint, interference, coercion, or discrimination. 9. To be free from restraints.

PARK PLACE ESTATES ACTIVITIES

A monthly calendar of events will be made available to all residents. An event calendar will also be posted on the bulletin board by the Activity Room. Residents are welcome to participate in these activities. ADDRESS

The resident’s new address is as follows: Name Park Place Estates #_____ 900 Lincoln N.E. LeMars, IA 51031 This is the address that residents will need to give to the Post Office to ensure uninterrupted delivery of personal mail.

ASSISTIVE DEVICES

Assistive devices are allowed when operated in a safe and courteous manner. Assistive devices are canes, walkers, and wheelchairs.

BILLING

Floyd Valley Healthcare will process the billing for Park Place Estates at the end of each month. Billing will reflect the rent for the upcoming month and any resident charges incurred during the previous month, including long distance telephone charges. Payment is due by the 15th of each month and payable to Park Place Estates. If you have any questions regarding your monthly statement, please contact the Director at Park Place Estates (712) 546-6793.

Residents may make their monthly payment at the Director’s office or drop their payment in the office mailbox (located by the resident mailboxes).

BEAUTY SALON

The Beauty Salon is located on 1st floor. We provide a licensed beauty salon as well as a licensed beautician. For an appointment, please contact Glenda Homan at (712) 540-0994.

Park Place Estates residents may utilize other private sources for their personal needs. All beauticians must have a current license to use the Beauty Salon at Park Place Estates.

CASH

Residents need to keep only a small amount of cash on hand. The Activity Coordinator and staff will notify the resident, family or responsible party if there is an activity that requires the resident to have money to participate.

CHECK IN/OUT

We would like residents to sign out/in when leaving/returning to Park Place Estates. The sign in/out sheet is located at the Information Desk. This is not mandatory but it is highly recommended. We do this as a safety check for our residents.

CHILDREN

Children are welcome at Park Place Estates. We have a playground for children to enjoy while they are here visiting. We ask that children be supervised by an adult when using the playground. For safety reasons children should not run or play in the hallways or run in / out of the electric doors as this could potentially cause a resident to fall.

Children should also be supervised by an adult when in the Neighborhood Area (lobby area). The piano and computer are furnishings that need to be treated kindly. It has been the experience of Park Place Estates that the maintenance can be very costly when these items are not appropriately used.

Please remember that evening is a quiet time for our residents. Residents may be getting ready for bed as early as 8:00 p.m. and sometimes earlier. COMMUNITY FEE

A community fee is payable at the time of admission. This fee is refunded on a pro-rated basis if the resident vacates the apartment within the first 12-months of occupancy. After the 12-month occupancy period there is no refund. COMPUTER

The resident computer is located at the 2nd floor nurses station. The computer is available for the residents to use at their leisure. Computer lessons are provided by Park Place Estates at no charge for those residents who are interested. E-mail and Skype are also available to the residents.

DISCHARGE

Park Place Estates may terminate the Occupancy Agreement and assist in the transfer of a resident to an appropriate health-care setting if in the judgment of the Director the resident meets one or more of the following conditions:

1. Is bed-bound; 2. Requires routine two-person assistance with standing, transfer, or evacuation; 3. Is dangerous to self or other residents or staff, including but not limited to a resident who despite intervention chronically elopes (elope means that a resident who has impaired decision making ability leaves the program without the knowledge or authorization of staff), is sexually or physically aggressive or abusive, or displays unmanageable verbal abuse or aggression or displays behavior that places another resident at risk; 4. Is in an acute stage of alcoholism, drug addiction, or uncontrolled mental illness; 5. Is under the age of 18; 6. Requires more than part-time or intermittent health-related care; 7. Has unmanageable incontinence on a routine basis, despite an individualized toileting program; 8. Is medically unstable; 9. Requires maximal assistance with activities of daily living; 10. Despite intervention, chronically urinates or defecates in places that are not considered acceptable to societal norms, such as on the floor or in a potted plant. 11. Is unwilling to cooperate in a Managed Risk Agreement if indicated to manage any activity or behavior that may adversely affect the health, safety, or well being of resident or other residents. If Park Place Estates determines that the resident needs to be discharged as evidenced by the discharge criteria then Park Place Estates will arrange to discharge the resident to a health care facility designated by the resident. If Park Place Estates seeks to discharge a resident based on this discharge criteria and the resident disagrees with this decision, the resident has the right to file a grievance and request a review of the discharge decision.

DOORS

No nails are allowed in the door woodwork. If residents wish to hang decorations on the door please contact the Director. No door wedges are allowed to prop open a door as this is a violation of the state fire and safety codes.

ELECTRICAL GUIDELINES

No extension cords, power strips, 3-plug adapters, Scentsy pots, plug in air fresheners or space heaters are allowed in the apartments as this is a violation of the state fire and safety codes.

E-MAIL

Park Place Estates’ e-mail address is [email protected]. This e-mail address is monitored by the Park Place Estates Director on a daily basis (Monday-Friday). Family and friends may use this address to send an e-mail to the resident. The e-mail should identify the recipient by placing the resident's name in the subject area of the e-mail message. The Director will see that the e-mail is delivered. EMERGENCIES

Staff will notify families as soon as possible when an emergency arises. It is best if the resident has more than one contact person listed in their medical record. This enables the staff to make an emergency contact with family in a timely manner. Please note that Park Place Estates has all telephone numbers needed in case of an emergency (home, work, cell).

If a family member or resident’s contact person is going to be out of town or unavailable for a period of time, Park Place Estates staff would appreciate having this information. Staff strongly encourage the family or contact person to give an alternative telephone number where they can be reached.

Likewise, emergencies can happen to a family member or contact person. When this occurs, it is best for the resident to hear this news from family rather than other residents or visitors. Staff will assist families /contact person as much as possible in this situation.

EMERGENCY RESPONSE SYSTEM

All apartments are equipped with a wireless emergency response system and a “corded” emergency response system. The wireless system requires the resident to wear a pendant. Residents are encouraged to wear their pendants at all times, except while bathing. The “corded” system includes 3 call cords located in the shower, by the stool, in the bedroom and by the apartment entry. For more information please refer to RESPONSE SYSTEM, page 31.

FAMILY UPDATES

Families are encouraged to call, email or stop by the Office anytime for updates on their resident. If families would like to receive email updates from the RN please talk to the Director so that this can be arranged.

FALLS

Park Place Estates staff cannot prevent falls that may occur. Staff will do as much as possible to minimize and reduce the risk of falls for the residents. Residents are encouraged to wear appropriate footwear at all times. Residents are also encouraged to use their walkers and canes especially when outside their apartments. Park Place Estates discourages the use of rugs in the apartment due to the potential for falls. To minimize falls that could occur in the parking lot residents are encouraged to utilize the valet service offered by staff.

FAX

Park Place Estates fax number is (712) 546-8085. The fax machine is located in the Director’s office. Residents may use the fax service at no charge by contacting the Director.

FIRE SAFETY

Smoke detectors are located in each apartment, the hallways, and common areas. Park Place Estates is responsible for the maintenance of the smoke detectors. The detectors will be checked yearly. New batteries are installed at this time also.

Residents may not burn candles in their apartments.

A fire sprinkler system has been installed throughout Park Place Estates. If the fire alarm sounds, residents must leave their apartment. To exit the apartment safely, always look for smoke around the doorframe. If no smoke is visible, lightly touch the wood part of the door. If the wood is not hot, then quickly touch the metal door handle. If the handle is not hot, open the door slowly. If there is no smoke in the hallway, leave the building immediately, using the appropriate exit indicated on diagrams located in the hallways. Please proceed to the east and south parking lots. After the fire department has secured the area, residents may return to their apartment. 1. The state requires a minimum of six (6) fire drills a year. 2. Park Place Estates will conduct fire drills 10-12 times yearly. 3. Fire drills require residents to completely evacuate from the building. 4. As required by the state, at least two (2) of the fire drills per year will be

conducted at a time when the majority of the residents are reasonably expected to be sleeping.

5. All residents must cooperate and participate. These drills are to be treated as a real fire.

The Iowa State Fire Marshal prohibits the use of extension cords and outlet adapters for any reason. The use of space heaters, coffee mug warmers, scentsy pots, and plug in air fresheners are prohibited in all areas. Surge protectors (power strips) are allowed to protect equipment such as computers, TV, DVD, stereo, phone. They are not allowed for other electrical devices such as holiday decorations, lamps, microwave, fridge, lift chair, clocks, or anything with a motor. These surge protectors must be of metal construction (UL 1449).

Closet sprinklers must be kept clear. Boxes or other items may not be stacked in front of the sprinklers. A safety tour is conducted two times yearly by Park Place Estates staff to check for potential fire hazards such as blocked sprinklers, extension cords, outlet adapters, candles, space heaters, and the misuse of power strips. This safety check allows Park Place Estates to remain compliant with the rules and regulations set forth by the State Fire Marshal. FIRE EXTINGUISHER LOCATIONS (16)

FURNISHINGS

Residents will furnish the apartment with their own personal furniture items. We encourage residents to bring in at least one folding chair. This chair can be quickly and easily moved into the hall, if necessary, during times when residents are required to evacuate their rooms.

Residents need to supply the light bulbs/batteries/etc, for their personal items such as lamps, night lights, clocks, flashlights, remote controls, etc.

Resident furnishings are not required to meet “fire retardant” standards.

Park Place Estates discourages the use of rugs in the apartment due to the fall potential. However, if a resident chooses to have a rug in the kitchen or bath area we require that the rug not have a rubber backing. The rubber back and vinyl floor have a chemical reaction causing the vinyl to discolor leaving a permanent yellow stain. GARBAGE

The staff collects garbage daily. Park Place Estates will provide trash liners for the resident’s wastebaskets.

GIFT CERTIFICATES

Gift certificates are available for meals, whirlpools, beauty shop, and guestroom. Contact the Director for more information regarding the gift certificates.

GRIEVANCES

Residents have the right to express their complaints and dissatisfactions without fear of reprisal. Please contact the Director with any grievances/concerns that you might have.

GUESTS/GUESTROOM

Guests are welcome to stay in our guestroom or stay in the resident’s apartment. Residents may reserve the guestroom by contacting the Director.

The fee for the guestroom is payable at the time of departure or may be added to the resident’s monthly bill. Meals are not included in the guestroom fee. HEALTHCARE MANAGEMENT

Healthcare Management is for the purpose of meeting the resident’s personal needs. This includes assistance with ADL’s (activities of daily living) and IADL’s (instrumental activities of daily living).

ADL’s include eating, bathing, dressing, using the bathroom, and transferring. IADL’s may include ambulation, housekeeping, meal preparation, shopping, making medical appointments, accompanying to medical appointments, transportation, writing checks, medication management etc.

Prior to admission it is necessary for the prospective resident to complete a state required health assessment, functional assessment, and cognitive assessment. A healthcare professional from Park Place Estates will be responsible for completing these three assessments.

The health assessment and cognitive assessment give valuable insight into the resident's medical and cognitive well-being.

The functional assessment provides information that tells the healthcare professional what the resident is capable of doing for him/herself or the areas where the resident may need assistance. The functional assessment covers the following areas: ambulation & physical ability, fall prevention & safety, nutrition & meals, activities & socialization, bathing, dressing & grooming, toileting, housekeeping & laundry, transportation & business affairs, wellness & medications, cognition, mood, behavior, and miscellaneous.

HEALTHCARE MANAGEMENT (continued)

The functional assessment is based on a point system. Upon completion of the assessment the points are totaled. The number of total points will determine the level of Healthcare Management that the resident requires. There are four (4) levels of Healthcare Management with each level having a different fee. This fee will be reflected on the resident’s monthly billing statement.

The new resident will be re-assessed thirty (30) days after admission to Park Place Estates utilizing the same assessment tools– health, cognitive, and functional. The level of Healthcare Management could potentially change at this time.

Following the pre-admission and 30-day assessment the resident will be assessed on an annual basis or when there is a significant change in the health condition. HEATING PADS / HEATED MATTRESS PADS Park Place Estates allows residents to use electric heating pads and electric blankets/mattress pads. However these items must be in good condition and the resident must be able to use these items safely. If the staff/RN feels the resident is not using the item safely the RN will review if it can continue to be kept in the apartment.

HOUSEKEEPING

Housekeeping services are included in the basic fee schedule.

Park Place Estates housekeeping staff will clean the resident’s apartment on a weekly basis. Included in residents’ housekeeping services are dusting, vacuuming, and cleaning the kitchen and bathroom. In regard to valuable, sentimental or fragile items that require dusting we ask that family members assist the resident with this task. Park Place Estates staff will defrost the refrigerator on an as needed basis. Food or beverages that are old/outdated will be discarded. On a bi-annual basis all furniture will be moved as part of the cleaning process. Mattresses may be rotated/flipped at the resident’s request. If additional housekeeping services are required in addition to the weekly service a fee for this additional service will be reflected on the resident’s monthly billing statement. INSURANCE (Long Term Care)

Any Long Term Care insurance policy held by the resident should be presented on admission. The Director will assist the resident and family with completing and submitting Long Term Care insurance forms to the insurance provider. INSURANCE (Renter’s)

Park Place Estates carries property insurance for the building and property belonging to Park Place. Park Place Estates does not carry insurance for personal property belonging to residents and shall not be responsible for loss or damage of a resident's personal belongings (including jewelry & cash). It is advised that residents carry renter’s insurance. (Note: renter’s insurance does not automatically cover jewelry unless the insured has provided to the insurance company an appraisal for the items to be insured).

INTER-FACILITY MOVE

Residents have the option to move to a different apartment with-in Park Place Estates. Such a request can be made known to the Director at the time of admission or anytime thereafter. The Director will place the resident’s name on the In-House Waiting List. Inter-facility moves are honored prior to any new admissions to Park Place Estates.

Inter-facility moves will be considered and approved according to the needs and desires of the resident in addition to the date of the request. However, if a current resident has a physical and/or medical need requiring a 1st floor apartment and their name is not on the In House Waiting List, their need can potentially supersede the request of residents on the waiting list.

The resident’s family or responsible party will be responsible for assisting with the inter-facility move. The staff of Park Place Estates are not available for this activity. Once the inter-facility move has been approved the resident must be prepared to move within a 24-hour period to minimize occupying two apartments at a time.

The resident will be billed a one-time fee for the inter-facility move. The resident will be responsible for notifying the Post Office of their address change.

KEYS

The resident will be issued one apartment and one mailbox key upon admission to Park Place Estates. For security purposes, we ask that the residents do not duplicate these keys. The Director and staff have a key available if it is necessary to get into the apartment or mailbox. Upon vacancy all keys are to be returned to the Director. If keys are not returned there will be a replacement fee for each key.

LAB SERVICES

The Family Medicine Clinic will provide lab services on-site for the convenience of the residents at Park Place Estates on the 1st and 3rd Thursday of each month. Labs not able to be drawn on these days will be done at Floyd Valley Healthcare. The Director or RN will notify the clinic when a blood draw needs to be performed.

LAUNDRY ROOMS

There is a laundry room located on both floors. The washers and dryers are available for the resident’s use at no charge anytime during the day.

Park Place Estates will be responsible for laundering the resident’s sheets and towels on a weekly basis. Staff will be using the appliances after 4:00 p.m. to wash sheets and towels as well as any personal laundry that is scheduled.

Personal laundry services are available for a fee if the resident needs assistance. Park Place Estates provides the laundry soap if washing the resident’s personal laundry. If the resident’s preference is for different laundry soap, the resident must provide the soap. The charge for personal laundry service will be reflected on the resident’s monthly statement.

The laundry room is furnished with an ironing board and iron. Residents may take these items to their apartment to use. There is a front loading washer in the Laundry Room on 1St floor for those who would benefit from this type of washer.

LIBRARY

The Le Mars Public Library on a monthly basis brings a variety of books for residents to check out. These books are available at the front desk. When checking out a book write on the clipboard your name and date next to the book you would like to check out.

MAIL

The mailboxes are located adjacent to the Neighborhood Area (Lobby). All residents will be provided with a locked mailbox. The apartment number and mailbox number will be the same.

MAINTENANCE

Floyd Valley Healthcare provides maintenance services. Services include hanging pictures, inspecting small appliances upon admission, maintaining operation of appliances furnished with the apartment and ongoing maintenance support. If a resident wants an item hung on the wall please do not use any sticky materials. Please contact Park Place staff and they will have maintenance hang the item for you. This maintenance support does not include the repair of personal items or provision of light bulbs or batteries for personal items.

After a resident’s discharge and prior to a new resident’s admission the entire apartment is checked for appearance, cleanliness, and operation of appliances, call cords, etc.

Please notify the Director or staff of any maintenance requests that arise. MANAGED / NEGOTIATED RISK AGREEMENT

A resident may be asked to cooperate in a managed or negotiated risk agreement with the assisted living program if their decision making ability could potentially result in poor outcomes for the resident or others. Depending on the activity or behavior, the resident could be asked to find occupancy elsewhere. The Director of Park Place Estates will initiate the Negotiated Risk Agreement if indicated.

MEALS (Guests)

Having guests at mealtime is highly encouraged.

Our meals are prepared at Floyd Valley Healthcare. The amount of food prepared depends on the number of residents currently living at Park Place Estates and the number of guests. Therefore, staff need to give the Dietary staff adequate notice to prepare additional meals for guests.

Giving the staff a 24-hour notice is requested. However, staff realize that this is not always possible. If the 24-hour notice is not possible please ask the staff about a guest meal anyway. Staff will then call the Dietary Department. Depending on the menu and the resident census for that day the ability to serve a guest may still be possible.

Remember—eating a meal with your resident can be the highlight of their day! MEALS (Residents)

Park Place Estates provides 3 meals a day. Floyd Valley Healthcare will plan and prepare the meals which will include 100% of the daily recommended dietary allowances as established by the Food and Nutrition Board of the National Research Council of the National Academy of Sciences. The meals prepared will be a general diet allowing the resident to monitor their own dietary preferences (sweets intake, salt intake, etc).

Residents are expected to eat all meals in the dining room. An exception to this is made for breakfast only.

The continental breakfast is available in the Dining Room from 7 – 8:30 a.m. A hot breakfast will be served every Wednesday starting at 7:15 a.m. If a resident wishes to eat breakfast in their apartment Park Place Estates will stock their apartment weekly with continental breakfast items (cereal, juice, milk, bread). Residents may have a personal toaster and coffee pot in their apartment if so desired.

Breakfast items such as coffee, hot tea, sweet rolls, muffins, toast etc. will not routinely be delivered to the apartment. If these items are delivered to the apartment per resident’s request there will be a tray delivery charge.

Dinner is the main meal for the day and will be served at 12 noon. The dinner menu offers two choices for the main entrée.

A Select Menu is offered at the evening meal and will be served at 5 p.m.

The staff will begin serving promptly at these times. Once the staff has started to serve they will continue to serve the main course until finished. Residents who arrive late may expect to be served last. The main course will be served to all residents before staff begin to refill the beverages.

All residents will receive a monthly menu. Park Place Estates does not offer therapeutic diets. Residents will be responsible for monitoring their own dietary restrictions.

Residents should notify the Director or staff if they will be absent at mealtimes. There are no refunds for meals not eaten at Park Place Estates. Tray service to the apartment is available when a resident is ill. There will be no charge for this service at that time. However, if the resident is not ill and chooses not to eat in the dining room there will be a fee for tray service to the apartment.

Residents may keep food supplies/products in their apartment if stored in tightly sealed containers.

Snacks are not routinely provided to the residents. Snacks that are provided are those associated with a scheduled activity. Not all

activities will have snacks provided. Pop is available from the pop machine located on the back patio.

MEDICATION

Level I—Medication Management (self-administration): The resident will self-administer all medications as prescribed by their physician. The resident will be responsible for contacting the pharmacy for all prescription needs.

A current Medication Record will be maintained by Park Place Estates. The resident who self administers their own medications will be responsible for communicating to the Registered Nurse when medications are added, discontinued or changed. The Registered Nurse will provide the resident with a current Medication Record each month for their personal use. A current Medication Record is available for all physician visits and emergency room visits. Park Place Estates maintains a current Medication Record to prevent delay in treatment if an emergency arises.

The fee for Level 1 Medication Management will be reflected on the resident’s monthly billing statement. Residents will be billed for their medications by the pharmacy of their choice. Level II—Medication Management (set-up): Medications will be provided by L&M Pharmacy Care, a local long-term care pharmacy. Licensed personnel will distribute medications into a weekly medication planner. Prescription refills and new medication orders will be the responsibility of the Registered Nurse.

The resident will be responsible for self-administering their medications as distributed in the weekly planner.

Regular assessment of the resident’s ability to take their prescribed medications as ordered will be done by a Registered Nurse along with staff input. Family a/o responsible party will be notified if the resident requires more assistance in medication management and self-administration. However the resident a/o family may choose to continue to self-administer medications despite known medication omissions.

A current Medication Record will be generated monthly with this service. A current Medication Record is available for all physician visits and emergency room visits. The fee for Level II Medication Management will be reflected on the resident’s monthly billing statement. Residents will be billed monthly for their medications by L&M Pharmacy Care.

MEDICATION cont. Level I & II Eye Drop Administration / Treatments: Occasionally a resident who self-administers his/her own medications (Level I or Level II) may have difficulty self-administering their prescribed eye drops or doing their own treatments (nebulizer treatments). The resident may elect to have staff administer the eye drops a/o nebulizer treatments. Staff will be responsible for documentation of this service.

The fee for this service will be reflected on the resident's monthly billing statement. Level—III Medication Management (set-up and staff administration): Medications will be individually packaged by L&M Pharmacy in a bi-weekly medication planner. Prescription refills and new medication orders will be the responsibility of the Registered Nurse and Pharmacist.

Certified personnel will administer and document medications given to the resident.

A current Medication Record will be generated monthly with this service. A current Medication Record is available for all physician visits and emergency room visits.

The fee for Level III Medication Management will be reflected on the resident’s monthly billing statement. Residents will be billed monthly for their medications by L&M Pharmacy Care.

MEDICAL APPOINTMENTS

Residents should notify Park Place Estates staff when medical appointments are made. This allows the staff time to prepare a copy of the resident’s current medication record as well as a nursing assessment, if indicated. This will assist the physician in his/her examination of the resident.

Family members are strongly encouraged to accompany residents to medical appointments when a resident is unable to go alone. Staff is not available for this service. If it becomes necessary for staff to accompany a resident to a medical appointment, a fee for this service will be reflected on the resident’s monthly billing statement.

If possible, medical appointments should be made prior to 2 p.m. to ensure that any new physician orders are carried out in a timely manner. NEWSLETTER

Park Place Estates publishes a monthly newsletter, which is provided to each resident. The monthly event calendar is included in the newsletter. The newsletter will be mailed to family members. If family prefers to receive the newsletter through email rather than postal mail, contact the office. Addresses for family members will be obtained on admission.

NEWSPAPERS

Residents will be responsible for their own subscriptions. The LeMars Daily Sentinel is delivered to the mailbox per postal mail. The Sioux City Journal is delivered to the Information Desk. The staff will mark the papers with the resident’s name. The resident may pick up their newspaper at breakfast time or it will be delivered to their apartment. To mark the newspapers correctly Park Place Estates will maintain a current list of those residents who receive subscriptions to the different newspapers. If a resident starts or stops a newspaper subscription please notify the Director/staff so that this list may be updated. This ensures that the resident receives their newspaper without a break in service.

NOTARY PUBLIC A Notary Public is available upon request. PARKING Residents may bring their car to Park Place Estates. Resident and visitor parking are available.

The maintenance staff will assure appropriate snow removal from the parking lot and sidewalks. If a resident and/or guest will be using the parking lot for an extended period of time (especially during months requiring snow removal), please notify the Director to ensure that safe access to vehicles is maintained.

At anytime the residents may utilize Park Place Estates’ valet service. This may help prevent falls by avoiding stepping up curbs or avoid the ice, snow or rain. PETS—VISITING PETS

Park Place Estates recognizes that pets can be beneficial to the psychosocial well being of our residents. Therefore pets are permitted at Park Place Estates as long as the pet meets certain criteria and is reviewed by administration. For a copy of our pet policy please see the Director.

PRIVACY CHOICES Park Place Estates desires to protect our resident’s privacy. Upon admission all residents will be given choices regarding the following: • Placement of name on personal mailbox, • Placement of name on apartment door, • Disclosure of personal telephone number to callers, • Disclosure of protected health information to specified parties, and • Permission to take and/or use photographs, video/audio recordings of the

resident for the purpose of identification, facility publications or media promotions.

Public restrooms are located on the second floor by the elevator and on the first floor by the Activity Room.

PHARMACY PICK-UP A “pharmacy order & pick-up” by staff will be done only under the following circumstances: • A new medication (e.g. antibiotic, diuretic) or changes

in a current medication have been ordered by the physician.

• It is necessary for the medication to be started before the scheduled pharmacy delivery times.

• Calling the family to pick up the new prescription has been unsuccessful or the family has directed the staff to pick up.

• There is staff available to make the pick up.

The fee for this service will be reflected on the resident’s monthly billing statement.

POWER OUTAGE If a power outage occurs at Park Place Estates the emergency lights will automatically activate. However, the emergency lights are battery powered and will last only a short period of time– approximately 90 minutes.

Residents are encouraged to bring a flashlight when moving to Park Place Estates. The flashlight should be kept in a place easily located in the event of a power outage.

RECYCLE Park Place Estates recycles paper, junk mail, pop cans/bottles, newspapers and magazines. Residents may give recyclable materials to a staff member to dispose of properly.

RESERVATIONS / SPECIAL OCCASIONS

Residents are encouraged to use the Activity and Dining Rooms for special events (church circles, card clubs, wedding showers, baby showers, birthday parties, etc.). Residents should notify the Director or staff of their desire to reserve any of these rooms. This will ensure that the room is available when needed.

The large dining room is available for use between 1:30pm – 3:30pm and after 6:30pm. Any straightening or clean up after the event is the responsibility of the resident and/or resident’s family.

The resident is responsible for serving as host. If unable to do so, the resident needs to have someone else to serve in that capacity. The staff are not available to host events.

Park Place Estates will provide coffee and paper products for groups of 25 people or less. If the resident prefers special paper products for the occasion they will be responsible for providing them. For groups larger than 25 the resident will be responsible for the paper products and table cloths needed for serving.

There is no charge for using these rooms.

RESIDENT COUNCIL

Resident Council meetings are held on a monthly basis. The date and time will be found on the monthly event calendar. This provides an opportunity for residents to meet with the Park Place Estates Director, Activity Coordinator, and RN. This meeting allows the residents to present any issues of concern to them and to be updated on current issues by the Director.

RESPONSE SYSTEM

Park Place Estates offers a personal response system for the residents.

Each apartment has four (4) pull cords. These cords can be found in the resident’s shower, beside the bathroom stool, in the bedroom, and by the apartment entry. Each resident receives a personal response pendant on admission as part of their rental agreement. To “activate” the personal response system the resident only needs to pull the call cord located in these four (4) areas or press the button located on their personal pendant.

Staff have the option of responding by intercom or going to the resident’s apartment. Once the resident’s request has been met the response system is “deactivated” by the staff by using the intercom or by canceling the system in the apartment. The residents are encouraged to wear this pendant at all times except while bathing. The pendant is not water resistant. The resident or staff assisting with the shower will use the pull cord located in the shower if assistance is needed while bathing. If an additional pendant (for couples) is desired arrangements may be made with the Director. All pendants are to be returned to Park Place Estates when the resident leaves. If the pendant is lost or broken the resident will be responsible for the cost of replacement. Park Place Estates will be responsible for battery replacement when indicated. This wireless personal response system is considered to be an emergency response system that enables the residents to notify staff whenever an emergent situation arises within their apartment or in the hallway.

SECOND OCCUPANT

A second occupant is allowed to reside in an apartment for an additional fee. The second occupant must meet Park Place Estates’ Occupancy Criteria. SECURITY

Security for the residents is important to the staff at Park Place Estates. The front doors (electric doors) are unlocked every morning at approximately 6:00 a.m. All doors are locked every evening at 8:30 p.m. There may be times when the doors are locked earlier. The resident will always be able to exit freely in case of emergencies. However, visitors will not be able to enter Park Place Estates after 8:30 p.m. unless using the Outdoor Call Entry System.

The Outdoor Call Entry System is located in the entryway. To activate, push the button, which then alerts the staff to your presence. There is an intercom system that can be utilized to talk with the staff once they are aware of your presence.

If residents are returning after the doors are locked it would be beneficial to call ahead to notify staff. This will allow for staff to plan for the resident’s arrival and have the door unlocked.

Visitors leaving the resident’s apartment after 8:30 p.m. should exit through the Activity Room door. Please do not unlock the front doors. If visitors would like to leave through the front doors, notify staff by using the call light in the resident’s apartment and they will be able to unlock the doors for you.

SMOKING

Park Place Estates as part of Floyd Valley Healthcare is a smoke free campus. Residents and visitors may not smoke anywhere on the campus of Floyd Valley Healthcare.

SHOPPING

The weekly shopping trip is designed to give the residents an opportunity to independently purchase necessary personal items. The residents who go shopping need to be able to handle money transactions, select items with minimal assistance, and be able to get around in the store with minimal directions and personal assistance.

Park Place Estates also provides shopping services on a weekly basis to those unable to go themselves. Staff will collect the resident’s shopping list and money. The items will be purchased during the shopping trip and be brought back to the resident’s apartment along with their receipt and change. SHORT TERM CARE

Park Place Estates offers the following care options:

Recuperation Care—provides temporary healthcare for one recovering from an illness or surgery. Our emphasis is to return the individual to their home when they are able to do so.

Respite Care—offers short-term care as a break for those who need temporary relief from the demands of providing care for a friend or family member. Whether it is for a vacation, a weekend, or even during inclement weather, Park Place Estates can assist when needed.

Hospice Care—provides comfort and supportive medical care to the terminally ill residents and their families. Park Place Estates will work simultaneously with the area hospice programs to supplement care and provide a loving, sensitive, and private environment for residents and their families.

Any resident or potential resident seeking one of these care options will be required to meet the admission criteria of Park Place Estates.

SPIRITUAL OFFERINGS

A variety of spiritual opportunities are available for residents to attend on a regular basis. These opportunities may include: Rosary, Bible Study, Catholic Communion and Mass, Non-Denominational Services, and Memorial Services. The chapel area is also available for resident’s private worship. STAFFING

Park Place Estates staff will be on-site and available to the residents 24 hours a day, 7 days a week. The staff is supervised by the Program Nurse (RN). A Registered Nurse is on duty Monday-Friday from 8am – 4:30pm. A Registered Nurse is on call 24/7 when not present in the building.

The staffing ratio will be adjusted according to the resident census and the individual needs of the residents.

Well-being checks on each resident are done twice daily by the staff. Staff will check on the residents during the night if requested. Otherwise, the staff will enter the apartments during the night only when the resident activates the personal/wireless response system.

The resident may choose his/her own healthcare participants such as physician, specialists, hospital, home health agency, etc.

SUGGESTION BOX

The Office mailbox is available to the resident to use as a Suggestion Box. Residents or family may leave their suggestions at any time. All suggestions are appreciated and will be considered.

TELEPHONE

Each apartment has telephone access in the bedroom and living room area. Billing for telephone service is done by Premier Communications through Park Place Estates. Local calling is included in the Park Place Estates basic apartment fee. Long distance charges incurred by the resident will be added to the monthly Park Place billing and are the resident’s responsibility. TELEVISION

The apartments are cable-ready. Cable and hi-definition service is provided by Premier Communications. It is included in the Park Place Estates basic apartment fee. Each apartment has a black cable box that provides the hi-definition ability. Each resident will have a remote control that will be used to operate their television. This remote is programmed to each individual black box so will only work with the television in that specific apartment. Each apartment will be supplied with a fire-rated metal surge protector that meets the Fire Marshall standards. When a resident is discharged from Park Place Estates the cable box, remote control, hi-definition cable cords and metal surge protector are to remain in the apartment. If removed there will be a replacement charge to the resident.

TERMINOLOGY for ASSISTED LIVING Cognitive Assessment The cognitive assessment evaluates the resident’s short term and long term memory ability.

The cognitive assessment is completed prior to occupancy, 30 days after occupancy, as needed, and not less than annually as long as the resident resides at Park Place Estates.

Functional Assessment The functional assessment evaluates the resident's ability to carry out their ADLs (activities of daily living such as eating, bathing, dressing, toileting, transferring) and IADLs (instrumental activities of daily living such as housekeeping, meal preparation, shopping, making medical appointments, transportation, handling business affairs, handling their own medications).

The functional assessment is completed prior to occupancy, 30 days after occupancy, as needed, and not less than annually as long as the resident resides at Park Place Estates.

Health Assessment The health assessment evaluates the resident’s health as it pertains to their medical diagnoses, medications currently prescribed, and current health treatments. The health assessment is completed prior to occupancy, 30 days after occupancy, as needed, and not less than annually as long as the resident resides at Park Place Estates.

Service Plan A service plan is created for each resident after the cognitive, functional, and health assessments are completed. The service plan encompasses the services that the resident needs and/or chooses. The service plan is updated after 30 days of occupancy, as needed, and no less than annually thereafter.

The service plan is an important document requested from Long Term Care insurance companies.

THERMOSTAT USE

Each apartment is equipped with a thermostat for individual heating and air conditioning. Windows must be closed when either is in use. TORNADO

Tornado Watch - Residents will be instructed to have a chair and pillow ready if vacating the apartment becomes necessary. In the interest of comfort for the resident, please bring a folding chair when moving into Park Place. This chair can be kept in the resident’s closet. This will allow the resident to have a chair to sit on if relocation to a safe area is required. During a Tornado Watch and Thunderstorm Warning staff close doors into general areas, windows, curtains, and blinds. Residents are not required to come out of their apartment. However, if a resident desires to be out of the apartment please ask staff for assistance to a safe area. 2nd floor residents may go to the 1st floor if they would prefer.

Tornado Warning - Residents are required to relocate to a safe area when a Tornado Warning is in effect. Safe areas within Park Place Estates include the resident’s bathroom with the door closed, the laundry rooms, whirlpool room, or the hallways (away from windows).

TRANSPORTATION– PERSONAL The Siouxland Regional Transit System (SRTS) provides local transportation services. The SRTS van is available for the resident's personal transportation needs within the city limits of Le Mars Monday-Friday from 7am to 5pm. The cost for the transit van is determined by SRTS. Residents may make their own arrangements for personal transportation or staff will assist. To make contact with this service call 800-881-2076.

TRANSPORTATION—MEDICAL

Park Place Estates (PPE) will provide transportation at no cost for residents to scheduled medical appointments Monday-Friday located on/off the Floyd Valley Healthcare campus and within the city limits of Le Mars. Medical Appointments include visits to the optometrist, dentist, chiropractor, mental health clinic and/or medical services provided on the Floyd Valley Healthcare campus (lab, physical, occupational, speech therapies, outpatient services). Transportation will be arranged at the resident’s expense if traveling to medical appointment/services from providers other than previously mentioned. Appointments scheduled no later than 2:00 pm are preferred due to staffing ratios and the professional presence to carry out any new physician orders. Transportation is provided via the PPE golf cart, PPE van, or the Siouxland Regional Transit System. PPE staff will make arrangements for transportation services when necessary.

Families will be responsible for accompanying residents to medical appointments if a resident is unable to go alone. Staff is not available for this service. If it becomes necessary for staff to accompany a resident to a medical appointment, a fee for this service will be reflected on the resident’s monthly billing statement. Following a resident’s hospitalization (from Floyd Valley Healthcare or any other hospital), family will be responsible for transporting the resident back to PPE. Park Place Estates places high importance on family involvement in the discharge process of a resident from the hospital.

TRANSPORTATION (continued) Transportation to scheduled events is available. A shopping trip to area stores is scheduled weekly when weather permits. Dates and times for scheduled transportation are indicated on the monthly event calendar. Transportation may involve taking the program van or arranging the transportation with Siouxland Regional Transit System. Urgent-Out of the Ordinary Transportation is defined as providing transportation to Floyd Valley Healthcare under the following circumstances: • Based on staff assessment/judgment there is an urgent need. • Calling the family to transport has been unsuccessful. • There is staff available to make the transport. If staff is available to transport

then a fee for this service will be reflected on the resident’s monthly billing statement.

• If staff is not readily available an ambulance will be called. • The resident will be responsible for any costs associated with the ambulance. Emergency Transportation is defined as providing transportation to Floyd Valley Healthcare by the local ambulance under the following circumstances: • Based on staff assessment/judgment there is an emergent need that requires the

presence of qualified personnel to transfer the resident to the hospital. • The resident will be responsible for any costs associated with the ambulance.

VOTING Park Place Estates is located in Ward # 1 and Precinct # 11. Residents will vote at the Plymouth County Courthouse. An absentee ballot may be obtained by contacting the Activity Coordinator at Park Place Estates.

VETERANS AID & ATTENDNACE PENSION BENEFIT All veterans have the opportunity to access this pension benefit. The Aid and Attendance benefit is available to a veteran and surviving spouse. To qualify the veteran / spouse must be in incapable of self-support and in need of regular personal attendance. There are also income guidelines to accessing this benefit. The Director of Park Place Estates will assist families in accessing this benefit. Contact will be made with local Department of Veterans Affairs located in the Plymouth County Courthouse or by calling 712-546-7090

WALL HANGINGS Individuals/families may hang their own wall hangings. Please do not use any sticky materials. If needed, maintenance will also assist with hanging personal items in the resident’s apartment. Prior approval must be obtained to permanently attach items to the building, including the wall and ceiling. Items that are permanently attached may need to remain in the apartment. If removed, and any damage occurs, the resident is responsible for necessary repairs.

WEAPONS Weapons and/or contraband are prohibited on Park Place Estates property except by law enforcement officers. This includes residents, visitors, and employees. Residents are not allowed to have weapons in their apartments.

WEB SITE

The Park Place Estates web site is found at www.parkplaceestates.org. WHIRLPOOL Whirlpools are available if desired by the resident. Whirlpools offer a therapeutic benefit. There is an additional fee for this service. Revised 07-16
