a poiiit where it merges with U.S. Route 41, The vehicle failed to negotiate the curve into the merge lane of Northbound U,S, Route 41.

Both right tires went up onto the raised curbing, The vehicle came off of the curb, crossed over the merge lane onto the gravel, inside shoulder, The vehicle rotated counter—clockwise and began a broadside, then began to roll.

The vehicle came to final rest approximately three-hundred and twenty-nine feet (32?') from the point of first incident,

The driver of UNIT #1 (PRATT) was ejected from the vehicle and came to final rest on the roadway at approximately two-hundred and eighty-sis feet (286') North of the point of first incident,

Reporting officer was dispatched to the area of Northbound U.S. Route 41 by the Marquardt Oldstown Dealership reference an accident with unknown injuries, While enroute, reporting officer also heard Corporal SHANK of the Lake County Sheriff's Office call his Desk reference the same incident but relating it was a serious Personal Injury accident and to -notify this agency,

Upon arrival, reporting officer found a female, white subject wearing shorts and a white tee-shirt laying in the inside traffic lane, at the merge with Route 43, A maroon Pontiac Grand Am bearing Illinois registration YD 8348 was resting in the grassy center usedias,

Reporting officer requested a Rescue squad and approached the victim, I observed an excessive amount of blood coming from the right ear and mouth of the victim, A bystander, Dreus M, BEAVLIER, 02/19/66 6027 17th Avenue, Kenosha, Uisconsin 414-657-8490 stated to reporting officer he still had a pulse on the victim. Reporting officer also noticed the strong odor of an alcoholic beverage on the victim,

Reporting officer checked the victim vehicle and found no one else in the vehicle, Reporting officer noted the following about UNIT #1, The vehicle's radio was extremely loud, at almost full volume, Reporting officer noted an unopened can of "Miller Genuine Draft" beer on the driver's side floor board, Also two more cans in the back seat along with an empty case bos for the same brand of beer.

The interior of the vehicle had a strong odor of an alcoholic beverage, The vehicle showed extensive damage that is consistent with a vehicle that had rolled over,

burnee Rescue arrived and transported the victim ta St, Therese Hospital, Rescue personnel stated the victim did have a pulse and shallow respiration, but the recovery of said victim was doubtful,

By this time, Corporal SHANK, Deputy BARRETT (Lake County Sheriff's Office) and Trooper JONES (Illinois State Police) arrived to assist reporting officer, Corporal SHANK and Trooper JONES assisted with traffic, Deputy BARRETT photographed the scene while reporting officer completed the measurements and scene diagram,

After the scene was completed, reporting officer contacted Urecker Services who responded to the scene and removed UNIT #1 to their lot at 3100 Central Avenue, Usukegan, Reporting officer falloued for purpose of Chain of Evidence, The vehicle was placed inside a locked storage building pending further investigation,

While at the scene, reporting officer was contacted by Park City Police Department Communications who related the victim had been pronounced dead at 015? Hours,

At approximately 0330 Hours, reporting officer arrived at St, Therese Hospital to observe Deputy BARRETT speaking to a group of subjects, Deputy BARRETT stated that the group had just identified my victim as Christina Jean PRATT with a date of birth of 08/06/74 and that he was conducting an interview as these subjects had been with PRATT from approximately 2030 Hours to 0015 Hours.

Reporting officer copied the information of all subjects present and noted all had an odor of an alcoholic beverage on or about their persons, The main part of the interview was conducted with David N. riATOVICH 09/02/72, Deputy BARRET! gave me a copy of the information obtained and I spoke with riATOVICH and he related the same story,

Reporting officer proceeded to the Emergency Room and spoke to the Physician on Duty, Dr, SANDS, Dr, SANOS stated it appeared PRATT had died of Extreme Internal and Head Trauma coupled with extensive blood loss, Reporting officer learned the Lake County Coroner's Office was advised by the Emergency Room staff,

Chief Deputy Coroner SHIPPER arrived and reporting officer went over the facts of the accident known thus far, PRATT's body was turned over to UHIPPER,

yHIPPER and reporting officer spoke again to Dr. SANDS and learned PRATT's Blood Alcohol Concentration, upon arrival to the hospital, was .1768,

Ue then viewed the body and reporting officer noted the following abnormalities; the left arm was almost completely severed at the shoulder, extensive abrasions and avultions on the left elbow area, extensive bleeding from both ears.

UHIPPER then took custody of the body and transported it to the Coroner's facility in Uaukegan, yHIPPER stated he would have Lake County Sheriff's Office assist with the notification of the next-of-kin and also contact us if a post mortem would be performed,

Prior to Deputy BARRETT's leaving the hospital, he stated he was enroute to the last known address of PRATT at 17585 Dartmoor Road, Grayslake, Illinois and would contact me if he was able to learn any further information or if notification was made.

At 052-5 Hours reporting officer received a call from Deputy BARRETT who advised notification had been made to PRATT'5 father in Milwaukee, Wisconsin by the Milwaukee Police Department, Also that the correct address for PRATT was 17479 Dartmoor Road 708-362-9497, BARRETT was also in possession of PRATT's driver's license and would deliver it to the police department.

Reporting officer went to Urecker Services at 3i00 Central Road, Usukegan, to inventory and photograph unit fi, A visual inspection of unit 11 showed the following: odometer reading of 46292,3 idles, Headlight switch was in the on position, the seat belt showed no signs of being used on the night of the accident, and the radio's volume knob was completely to the right (full volume),

Interior inventory of unit #i: 1, Three (3) full 12 02 cans "Miller Genuine Draft" Beer 2, One (1) empty case bos of same brand 3, Miscellaneous cassette tapes 4, One (15 yellow jacket 5, One (!) pair brown loafers

Route 43, failed to negotiate the curve into the merge lans with U, S, Route 41, Hark on the raised curbing coincide with damage to the right front wheel of UNIT #1, The vehicle went up onto the curb section then came back off and then onto the west gravel sholder, Uhile on the gravel the vehicle started a broadslide and continued on a Northerly direction then it started to roll over-after the left front rim made contact with the pavement,
