“......parent said that healthcare professionals kept saying that they would be ‘going through transitions’ and now they are accessing adult services and they are still not aware of the transitions process. ....
...I.e. there were in children’s services and suddenly found themselves in adults services. .......”
CONCLUSION
“Families said that their understanding of transitions was extremely vague and no-one could define what transitions was and what did / should happen during this period.”

Investing in Children Investing in Children (IiC) is a Community Interest
Company that listens to the wants and needs of children & young people and helps to make changes based on this.
Started working with Paed. Diabetes Teams in 2001 and T1KZ has grown considerably since then. Children & young people asked to have more peer support to help them live successful lives by learning from one another. Have several interactive sessions each month across North East England.
Investing in Children
Young facilitators support other children & young people to learn by taking part in practical activities and events. In the past 2 years 160 children & young people have taken part. Winner of 2 Quality in Care Awards 2016.
