Embedded Counselors
• PACTS Plan includes both traditional “Ed Plan,” as required by SSSP, as well as a “support plan,” identifying personalized student needs around Tier 1 competencies
• Self-Awareness – facilitators report students demonstrated an increased awareness of internal and external factors that can impact their success
• Faculty Awareness – faculty become more aware of barriers to student success
• Data provided to the embedded Pathway Counselors; which are then used develop individualized and cohort interventions
