On behalf of all staff, I would like to welcome you to our Health Service where we are continuingthe mission of the Sisters of the Little Company of Mary.

Calvary Health Care Bethlehem has been providing services to our local community for over 75years and are known for our specialist palliative care service and our state-wide role in caring forthose with a progressive neurological disease.

Our care is patient-centred and focused on the whole individual. Our compassionate approach andthe expertise of our interdisciplinary teams are coordinated to ensure that we deliver our patientssafe, high quality care and provide support to families.

The Purpose of this handbook is to explain how the hospital works and answer some of thequestions you may have. At Calvary Health Care Bethlehem, our priority is to promote your qualityof life and to do all that we can to meet your needs.

Welcome to Calvary Health Care Bethlehem.

In preparation for your inpatient stay with us, please find below some key items which you will need.

What you will need

Personal items• Toiletries: soap, shampoo, deodorant, brush/comb, razors (if required) and tissues

• Clothing: comfortable clothing for day and night clearly marked with your name

• Medications: for medical, nursing staff and pharmacist to review

• Money: for minor expenses such as newspapers or vending machines ($20-$30 as a guide. Please do not bring large sums of money into the hospital)

• Ear phones: if you are bringing any audio visual items, also can be used with TVs

• Continence equipment

• Walking aids: including walking stick, wheeled frame, wheelchair if currently used

Documentation• Medicare card

• Private Health Insurance card (if applicable)

• Pension details

• DVA (Department of Veterans’ Affairs) card (if applicable)

• Any referral letter from your GP or specialist

• List of current medications and allergy information

• List of key contacts (e.g. family members, friends) and their phone numbers

Access

Calvary Health Care Bethlehem has threeinpatient wards.

Main Wing

St Joseph’s and St Luke’s Wards Levels 1and 2 Ph: 03 95962853.

Families can use the Ambulance Bay at the frontof the hospital for patient drop-off/pick up. Ifassistance is required, please enquire at mainReception. There are 2 disabled car parksopposite the Ambulance Bay.

St Teresa’s Ward - Ph: 03 9595 3336 or 03 95945 3434.

During business hours (Mon – Fri 8:30am to 5:00pm)access to St Teresa’s Ward and the AmbulatoryClinics (the Neurological Ambulatory Service andCommunity Palliative Care Clinic), is availablefrom the car park entrance on Ludbrooke Avenue(located on the North side of the building nearPhysiotherapy and the Day Centre).

The visitors’ car park is located on Saturn Streetopposite the main hospital entrance. Parkingrestrictions apply in local streets, please checksigns for details. Saturn Street Car Park is closedfrom 10pm – 6.30am. Ward Support are availableto unlock the car park during this time if required.

Access – After Hours

After-hours access is through the main reception.

Accommodation forfamily/friends

In special circumstances, there may be limitedaccommodation available for family or friends tostay within the hospital. Queries should bedirected to the Nurse Unit Manager or WardSecretary.

There are also a number of hotels, self containedapartments and Bed and Breakfasts nearby.Please ask the Ward Secretary for details.

Appointments - clinic

Appointments are made under Medicare and canonly be made with an up to date referral from thepatient’s General Practitioner (GP) or specialist. Itis the responsibility of the patient to arrange theirown referral.

Patients are also required to provide up to dateinformation about themselves, including theirGeneral Practitioner contacts, Medicare cardnumbers, and carer details, their availability,residency status or any interpreter requirements.

Because Outpatient appointments are in highdemand, we require advance notice from you ifyou are unable to make your appointment.Unfortunately missed appointments may result ina patient being added to a waiting list.

Alcohol

Patients are permitted to consume alcohol attheir doctor’s discretion provided it does notinterfere with their medication or place anyone atrisk. Please consult with your treating Doctor.

Coffee and Tea

Coffee and Tea making facilities are available forpatients and visitors in the Day Rooms on eachward of the hospital. To access these facilities,please speak to the Ward Secretary or consult themap in the middle of this handbook.

Community Palliative CareServices

The Community Palliative Care Service is a teamof specialist palliative care nurses, allied healthand medical specialists that attend to patients inclinic or in their home. Specialist CommunityPalliative Care Nurses are available 24 hours aday, 7 days a week for patients who reside withinthe hospital service area – in the cities of Bayside,Port Phillip, Glen Eira, Stonnington and part ofKingston.

The aim of the Community Palliative Care Serviceis to provide the best possible care for patientswho wish to be cared for at home. Please discusswith your unit manager.

Computers

Computer access is available in the Informationand Resources Room (Level 1) and on St Teresa’sWard in the Jacaranda (Day) Room (please seemap). For internet access please consult staff onyour ward.

Counselling

Grief counselling and support is available forfamilies either on site or at home. Please discusswith nursing staff for referral.

Banking

There is no automatic teller machine (ATM) in thehospital. The nearest ATMs are located on MartinStreet (near the Gardenvale Train Station) and theservice station on the corner of Hawthorn andNorth Roads.

Cafeteria

The cafeteria is open Monday to Friday between7.30am and 2.30pm, with:

• Hot and cold food and drinks available for purchase (during above hours only)

• Free tea and coffee making facilities

• Fridge, microwave oven and sandwich machine available

• Vending machines providing light snacks anddrinks.

Chapel

You and your family are welcome to spend timein the chapel which is open 24 hours a day. Thechapel is on the ground floor past the mainreception desk. Catholic mass is celebratedmonthly. Check dates and times on the chapeldoor. Please consult your Ward Secretary orreception for directions.

Chemists

The closest chemists are located on the corner ofHawthorn and Kooyong Roads, and on thecorner of the Nepean Highway and GardenvaleRoad.

Clothing

Patients are required to bring sufficient clothingfor their length of stay with us. Please bring

Day Centre

The Palliative Care Day Centre is a warm, friendlyenvironment that caters to palliative care patientsvisiting from the community. The Centre offers avariety of programs, including cooking, art andmusic therapy, gardening, tai chi and games toenhance, challenge and promote optimum,physical and mental well being in a warm,friendly and social environment. Free morning teaand lunch are provided along with activities. ThePalliative Day Care Centre is situated on theground floor of St Teresa’s ward opposite theNeurological Ambulatory Service and is openWednesdays and Fridays between 10.00am-3.00pm. Inpatients wishing to engage in similaractivities are welcome to do so with our residentDiversional Therapist via your nursing staff. (SeeDiversional Therapy below)

Day Rooms

There are Day rooms located on each ward forpatients, families and visitors to meet and relax.The Day rooms have nearby tea and coffeemaking facilities, couches and televisions. Thesefacilities are also available in the Cafeteria (onLevel 1 of the main building).

Dental

Emergency Dental work can be arranged via thehospital at the patient’s expense. Please discusswith your treating Doctor or Nursing Staff ifrequired.

Discharge

Upon discharge, all patients will be provided withtheir own Patient Discharge Plan. The planidentifies where, when and by whom a patientwill be discharged, the support they will requireincluding follow-up, and any equipment they

may need once home. Our DischargeCoordinator will be able to arrange for adischarge letter with relevant Nursing, Medicaland Allied Health information, to be forwarded toa patient’s specialist and community serviceproviders. Please discuss with Nursing Staff.

Diversional Therapy

Our Diversional Therapist works individually withpatients to design and facilitate tailored leisureand recreation programmes.

Activities are designed to support, challenge andenhance a patient’s psychological, spiritual,social, emotional and physical wellbeing, and areheld on site in patient rooms, in ward day roomsand in the day centre space. Please discuss withNursing Staff for referral.

Donations

There are many ways to support the work ofCalvary Health Care Bethlehem. You may chooseto provide a one-off or regular donation, supportour latest fundraising appeal, give an in-memoriam gift, or leave a bequest in your Will.Your donation enables us to purchase specialisedequipment, and develop new initiatives whichimprove patients’ quality of life. Equipment Hire Service

CHCB has access to a comprehensive range ofequipment that is available for loan or hire toassist patients living at home. Equipment will beprovided following assessment from CHCBtherapists or community nursing staff.

Electrical Appliances

For the comfort of all patients, earphones mustbe used with any audio-visual equipment. Allrooms are equipped with ceiling mountedtelevisions with headphone capability. Televisionviewing is available free of charge.

Electrical appliances brought into the hospitalmust be tested by an electrician and have acurrent certificate of electrical safety. Pleasediscuss with the Ward Secretary or Nursing Staffprior to bringing into hospital.

Falls Prevention

Calvary Health Care Bethlehem aims to minimisethe risk of patients falling, especially while theyare in the unfamiliar surroundings of the hospital.We believe that preventing falls is a team effort.When admitted to Calvary Health CareBethlehem, your risk of falling will be assessed.All patients will be provided with writteninformation to reduce the risk of a fall.

Florist

There are a number of florists located within thelocal area. Please ask Reception (dial 9) fordirections. Vases are available on each ward.Please ask the Ward Secretary or Nursing Staff forassistance.

Gardens

Calvary Health Care Bethlehem has a number ofgarden spaces for the use of patients and theirfamilies/visitors. They are ‘all-ability’ accessiblefrom the walkway connecting the main hospitalwith St Teresa’s Ward (please see map) andinclude a special sensory garden. Self-cateredbirthdays and special events can be celebrated inthe gardens by anybody that wishes to do so.Please consult with the Nurse Unit Manager orWard Secretary.

For security reasons the gardens are closed after6:00pm.

Hairdressing

A hairdresser visits the hospital every Friday.Bookings can be made via the Ward Secretary.

Health and Safety

Calvary Health Care Bethlehem is committed tocontinuously improving the management andstandards of workplace health and safety.

We welcome feedback on health and safetyissues that a patient or family member believeswarrants our attention. Anonymous feedbackcan be provided at any of a number of sources inthe hospital, including Reception, the Cafeteriaand Wards – please see “Suggestions &Complaints” for details.

Hospital Staff Identification

All Calvary Health Care Bethlehem staff wearname badges with the CHCB logo, their nameand job title. Any Students or Agency NursingStaff also wear an identification badge displayingtheir name and the organisation/agency to whichthey belong. Please consult with staff if you haveany concerns.

Information and ResourcesRoom

The Visitor Information and Resources Room isavailable for patients and visitors to use everyweekday and by appointment on weekends. Theroom is located on Level 1 of the main building(next to the Cafeteria). The room offers acollection of books and information pamphlets inmany languages. Access to a computer, Internet,TV and DVD player is available. The room alsofeatures a small children’s play area withchildren’s books available.

Interpreter Service

Calvary Health Care Bethlehemoffers interpreters for patientswho require them. Aninterpreter service is available free of charge forpatients and their families. Any Information of amedical or personal nature that is discussedthrough an interpreter will be treated with totalconfidentiality.

This interpreter symbol indicates where you askfor help to organise an interpreter.

Laundry

Please bring enough clothes for your stay inhospital. If you are unable to make arrangementsfor your clothing to be laundered, please discussthis with Ward Staff.

Leave Passes

Day leave is available if the patient’s conditionallows. Please discuss any leave requirementswith the Nursing or Medical staff to organise itwell in advance. Belongings may be stored on theward whilst patients are on leave.

Legal Documents

Members of the hospital staff and volunteers arenot permitted to witness the signature of anylegal documents. Families or solicitors shouldmake arrangements for their own witnesses.

Mail

Mail is received every day Monday to Friday.Please consult with the Ward Secretary if youhave mail that you wish to send.

Meals

Inpatient Meal TimesBreakfast: 8.00 - 9.00 am

Lunch : 12.00 – 1.00 pm

Dinner: 5.00- 6.00 pm

Families are welcome to order a meal from thekitchen at a small cost. Please arrange throughthe Meal Monitor (Ext 3420) or, after hours, viaReception (dial 9).

• Our catering provides for Kosher, Halal and other special Dietary requirements. These aremade known to the kitchen during the patient admission process.

• If you wish to bring your own food into hospital please discuss this with staff.

If you bring food, it must be clearly labelled anddated, and placed in the fridge available on theward. For health and safety reasons, unlabelledfood, and food that has been stored beyond 3days, will be thrown away. Visitors are asked toplease record all food items brought in, in theFood record book, located in each ward pantry.

Medical Information

Patient Information is provided to relative(s)nominated by the patient - over the phone, atfamily meetings or in writing. If required, amember of the Nursing Staff or Social Work teammay be able to arrange a family meeting foridentified relatives to discuss the patient’s medicalcondition with the Medical and Nursing team.Please discuss with the Nursing Staff.

Medications

Please bring all your medications to hospital,including prescribed and complimentary/naturalmedicines, for medical, nursing and pharmacystaff to review.

Money

A small amount of money is useful fornewspapers, the Cafeteria and unexpected smallexpenses. There is no automatic teller machine(ATM) in the hospital. (For nearest ATMs, seeBanking)

Newspapers and Magazines

Daily newspapers can be ordered via the WardSecretary on each ward at a small cost.

Next of Kin

Person ResponsiblePlease ensure the correct phone numbers andaddresses are given to the staff for yournominated contact person or next of kin. If apatient’s relative or carer will be away for anyreason, please advise a member of staff. It isessential that alternative contact details beprovided.

Nurse Call System

There is a nurse call button close to your bed.This means you are in close contact with nursingstaff at all times. Please consult with Nursing Staffto familiarise yourself with this.

Parking

The main visitors’ car park (Gate 7) is situated offKooyong Road in Saturn Street, opposite themain hospital entrance. Parking is free of charge.

For those visiting St Teresa’s Ward, limited parkingincluding Disabled parking, is also availableduring business hours via Ludbrooke Avenue(Gate 2 - located on the North side of thebuilding near Physiotherapy and the Day Centre).Parking restrictions apply in local streets, pleasecheck signs for details. Some disabled parking isavailable at the front entrance off Saturn Street(refer to signs).

Pastoral Care

Calvary Health Care Bethlehem has a strongcommitment to caring for the physical, emotionaland spiritual wellbeing of patients. Pastoral carersvisit the wards daily for the support of patientsand their families. There is also a pastoral teamavailable for home visits. Pastoral care includes,but is not limited to, spiritual and religious care. Itis concerned with the whole illness experienceand helping people find their own meaning in it.Pastoral care can arrange visits from a minister ofyour own faith if you wish. Please discuss withNursing Staff for referral.

Podiatry

A podiatrist visit can be arranged via the hospitalat the patient’s expense. Please discuss withNursing Staff or the Ward Secretary.

Prayer and Reflection

There are a number of spaces available forreflection and prayer. The gardens offer a quietrelaxing space and you and your family arewelcome to spend time in the chapel which isopen 24 hours a day. Reflective writings areavailable in the chapel for people to read.

The chapel is on the Ground Floor past mainReception desk (please see map).

Ministers of religion visit regularly. Pastoral carecan arrange visits from a minister of your ownfaith if you wish (dial 3436).

Private Patients

When you choose to be admitted using yourPrivate Health Insurance, your health insurance orVeteran’s Affairs Gold Card will pay a fee thathelps us provide extra services and equipment.You will not be charged a gap fee. Please feel freeto discuss this option with the Ward Secretary.

Based on our values of Hospitality, Healing,Stewardship and Respect, the management andstaff of Calvary support the Charter of the Rightsand Responsibilities of the Person Receiving Care.It recognises, protects and promotes the rights ofeach person receiving care, the partnership withhealth care providers and ensures that personaldignity and autonomy are upheld at all times.

• Know all treatment/care options and to participate in decisions regarding treatment/care (informed consent)

• Be treated with respect and not be discriminated against

Smoking

Calvary Health Care Bethlehem is committed topromoting a healthy and safe environment forpatients, visitors and staff. Smoking will not bepermitted anywhere within the boundary of thehospital buildings or grounds, as of 31 December2014. Patients will be given the opportunity andresources to quit smoking. Patients who areindependent and self-caring, and their visitorswho wish to smoke, must do so outside thehospital boundary. Please dispose of smokinglitter responsibly.

Suggestions and Complaints

Feedback from patients and families about anyaspect of the service at Calvary Health CareBethlehem is greatly appreciated and is vital inassisting us in our efforts to continually improveour health service. To provide your feedback, aSuggestions and Complaints form and box is heldat Reception and in the Cafeteria. There are alsoforms available on each of the Wards, and thePhysiotherapy Department. Please enquire withstaff.

Supermarkets

The nearest Supermarket is Coles Supermarket,located at 445-457 Glenhuntly Road, (approx.1.5 kilometres from the hospital – please seemap).

Televisions

Ceiling-mounted digital televisions are availablein each room and are provided free of charge.Foxtel is available in the Jacaranda Room on StTeresa’s Ward.

Telephones

You may bring a mobile phone. To use thehospital phone, dial 9 for reception.

Relatives and friends are asked not to call duringmealtimes. (See ‘Meal Times’)

Toiletries

Patients are required to supply their owntoiletries: tissues, shampoo, soap, toothbrush,comb/brush, shaving equipment etc.

Toilets

Visitor’s toilets are marked on the large maplocated at the centre of this handbook.

Volunteers are an important part of CalvaryHealth Care Bethlehem. They are trained to assistyou in a number of ways. They can providesupport in the form of relaxation, gentle foot andhand massage, games and social support. TheVolunteer Service also supplies a BiographyProgram that enables patients to tell their story.

OCT – Looking below the surfaceIt is natural for your eyes to go through changes as you age. With OCT scanning technology, we can often identify eye health problems before you experience anynoticeable symptoms eg. Macula degeneration andGlaucoma

This is important as early detection and treatment of many eye diseases can greatly reduce their severity andprogression. 1. Check your Blood PressureHigh blood pressure often gives no warning signs and it isnecessary to have it checked regularly by your doctor.
