    What Happened? Process Safety Incident:Contractor Head Injury, NLWDC

    Whilst Pressure Testing HCR & RemoteChoke Valves, via a 1502 test flange withincorrect bolts fitted, flange released witha bang (contained on remaining bolts). Onhearing the sound the I.P. stumbled &struck his head, causing a small 1 cmlaceration behind his right ear. All barriers& permits were in place.

    What Caused This?

    1. Incorrect bolts used on the flange as perAPI SPEC 6Aresulted in short boltedconnection that could not withstandtest pressure.

    2. Supervisors accepted an unsafecondition.

    3. Crew members failed to use the StopWork Authority.

    What Can I Do?1. Ensure supervisors are aware of API

    recommended practices, standards andspecifications.

    2. Enforce empowerment to STOPprogram.

    3. Never accept unsafe conditions.4. Ensure The PTW addresses the task.

    5. Discuss incident and highlight the lessonslearned with Drill Crews during Pre-tourmeetings, TBT and Weekly safety Meeting.

    6. Ensure Pressure test area is barricaded andall personnel are clear.

    7. Ensure proper bolting of flanges and that allbolts are in place and fully engaged.
