What are the aims of the service?

• To provide access to support for you outside normal office hours and through planned home visits• To help you increase your confidence and independence• To help prevent a crisis occurring that may lead to hospital admission

How does the service work?The service operates from Monday to Friday from 5.00pm to 9.00pm and on Saturday and Sunday from 9.00am to 5.00pm 365 days a year.

Out of HoursEmotional and Social Support

Gofal cymru provides emotional and social support for people with mental health problems through a service funded by the Welsh Assembly Government and Bridgend Local Health Board.

Who is the service for?

• People who have a mental health problem and who live in the community within the county borough area of Bridgend.

• People who use mental health services within the county borough area of Bridgend.

The support this service can offer may include:

• You can contact us at the address on this leaflet for further information

• You can ask a friend or relative, or any other person to contact us on your behalf

• Your GP, Social Worker, CPN, Health Visitor, Occupational therapist, or support worker can also make a referral for you.

Who we are:Gofal cymru is a registered mental health charity and not for profit organisation which has been providing high quality community based mental health services for over 18 years across Wales. Gofal Cymru provides services of the highest quality which are innovative, holistic and enable service users to live independently within their local communities. We work in partnership with and receive funding from the Welsh Assembly Government, Local Authorities, the Department for Work and Pensions, Local Health Boards, the Big Lottery Fund and other organisations to develop and deliver our services.
