Outcomes and metrics

For each metric other than patient experience, please provide details of the expected outcomes and benefits of the scheme and how these will be measured.

The metrics should be seen to improve around a series of operational options, designed to impact on all measures rather than individual discreet ones. These options are part of the BCF plan and

include, Accountable care co-ordinator from most relevent agency (health, social care, housing , third sector), embed access to advice and information and support at community level through

innovative partnerships with the third sector and Housing,.An integrated single point of access for unscheduled care and hospital discharges. Social Worker presence in A and E units 7 days a

up/step down beds in the system 7 days a week, introduction of emergency floors at acute hospitals in Cumbria.

For the patient experience metric, either existing or newly developed local metrics or a national metric (currently under development) can be used for October 2015 payment. Please see the

technical guidance for further detail. If you are using a local metric please provide details of the expected outcomes and benefits and how these will be measured, and include the relevant

details in the table below

For each metric, please provide details of the assurance process underpinning the agreement of the performance plans

Strategic governance will be provided through the Cumbria Health and Wellbeing Board. Operational governance arrangements will be undertaken through NHS Trust boards, the NHS Cumbria

Clinical Commissioning Group’s governing body and Cumbria County Council’s cabinet and overview and scrutiny body as well as Cumbria Housing Executive Group.

As described in section 1C a partnership programme board has overseen the development of this draft plan. The programme board includes two members of the Health and Wellbeing Board.

Notes:

REduction in DToC Rate by 10% but backloaded to reflect most change in 2015/16. Therefore Apr-Dec target set at flat DToCs and Jan-Jun set to 5% reduction

Significant change in reporting in 2013/14 - assumed to be incorrect and expectation that reporting will return to 2012/13 levels in 2014/15.

NB. Guidance asks for total number of delays in the month but this data is not collected. Instead the totla number of days delayed in the month is collected and has been made available

nationally as a baseline. Therefore these trajectories are based on days delayed rather than total number of delays.
