• A tanker truck exploded while loading at the AGE Refinery Inc.'s loading rack.

• One truck driver seriously burned.

• One mile evacuation radius ordered.

• 100 firefighters responded providing exposure protection.

before the incident stabilization.

• AGE Refining later filed for bankruptcy.

• NuStar purchased the refinery in 2012 and made several upgrades.

2013: Building a Response Foundation

• Purchased a 1994 Freightliner fire engine.

• Built out a bunker room for ERT gear and equipment at the Brooks City Base Office.

• Emergency Operations Center (EOC) established at Brooks Administration Building – Alamo Conference Room

• Began sending ERT members to TEEX NFPA 1081 Exterior Firefighting training in College Station, TX.

2014: Building a Response Foundation • Launched HAZWOPER 40 Hour training for all Operators and ERT Members.

• Develop refinery and office evacuation routes and assembly locations.

• Began monthly training for ERT members.

• Began joint training with outside agencies (San Antonio River Authority, San Antonio Fire Department, etc.)

• Safely conduct several full-scale exercises with multiple emergency response partners.

• Better use COSA emergency response resources across an array of scenarios across many different disciplines and agencies.

What Went Well?

1. Quick incident reporting and response by CSAR

Emergency Response Team members.

2. SAFD quickly deployed counter measures.

3. Minimal impact to traffic on South Presa during

the investigation and clean up.

4. A contract Environmental Company (TUFF) was

already at the refinery for spill remediation.

What Could Be Improved?

The local SAFD Engine Company drove by the

North Staging Lot (where requested) and drove

to the refinery main entrance instead.

The Env-Evol driver should have vacuumed out

his transfer hoses before driving away from the

Frac tank.

December 5, 2015 F11 Reformer Heater Fire

Black smoke was noticed coming out from F-11 heater stack followed by fire. Incident Objectives: • Protect life • Protect property • Protect the environment • Minimize impact to the local community Initial Corrective Action: • Reformer was immediately shutdown by pulling E stops. • Snuffing steam was introduced into heater fire box. • Fire was put out with no injuries to personnel and no

environmental incident.

remediation efforts were initiated. • The source of the spill was located. • The footprint of the spill was determined. • Fence line to the adjacent property operated by

Lubizol was opened to remove contaminated soil.

What Went Well?

1. No injuries or fire.

2. Quick ERT response from Brooks.

3. SAFD not needed to be called.

4. South Presa was not shutdown.

Barnes attacked the creek with timely counter

measures (soft booms, transfer pump, etc.)

What Could Be Improved?

No Evacuation notice issued.

No hot, warm or cold zones established.

Non-ERT personal in warm zone during initial

response.

Do not mix various foam products.

Freshly cleaned bunker gear not re-assembled

and ready for emergency response.

Some ERT members missing individual response

equipment (masks, gloves, etc.)

Need a quick location within fire engine for bolt

cutters, ax and Halligan tool.
