CLIENT PROFILE & MEASUREMENT FORM Thank you for your interest in Dress Code. We’re excited to get started on your first custom product! If you have any questions that aren’t answered in these instructions, please reach out to our customer service team, who is always available to help! THE PROCESS If you are unable to meet with a Dress Code style consultant, we’ll ask you submit your measurements to us through this form. Once we’ve processed your order, one of our style consultants will give you a call to walk you through the design of your custom product. Your style consultant will be your single point of contact throughout the entire process, and will be the one to follow-‐up with you once your custom product is shipped. In addition to your style consultant, you can always contact Customer Service with questions. OUR GUARANTEE We stand behind our guarantee to produce a perfectly-‐fitted custom garment. Finished suits occasionally require fine-‐tuning, which can be accomplished by your local tailor. We provide $75 of tailoring credit, to allow you to hone in on your fit preferences. In the rare event that you require a full re-‐make, we will gladly do so at no cost to you. By designing your custom garment to your exact measurements, we have the unique ability to build your product exactly the way it fits you best. We encourage you to take advantage of our guarantee so each custom product you wear from Dress Code is fitted and styled exactly the way you want. TIMELINE 4 weeks from the time you submit your measurements, your custom product will be ready to be shipped. We offer standard shipping through FedEx, which generally adds 3-‐7 days for it to arrive at your door. If you are hoping to wear your new Dress Code product for an upcoming event, please be sure that you leave enough time for pressing, and any final tailoring that may be needed upon delivery.

If the measurements you submit to us are so inaccurate that you will need a revised garment, your timeline can increase by another 4 weeks. This in mind, please communicate your deadlines to your style consultant so proper precautions can be taken to avoid any missed deadlines. For clients designing a suit that will be used for their wedding, our policy is that you submit your measurements to us 2 months prior to your date to account for unexpected transit delays or fit issues. RUSH ORDERS We will try to be as accommodating as possible to fulfill rush requests. Please contact customer service to discuss rush orders. If we can commit to your deadline, a 10% fee will be processed with your payment. GET MEASURED In just 15 minutes, you’ll be able to build your custom profile that can be used for any custom garment Dress Code offers. We don’t recommend that you get your measurements taken by a tailor, whose measuring techniques may be different from ours. Instead, recruit a friend to help and follow the instructions we provide in this form. We recommend that you take multiple measurements throughout a series of a few days to ensure that you’re always coming within ¼ inch of your measurements at each step. We can only build your suit off the numbers you provide, so we ask that you are certain they are accurate before you submit them to us. Before we ship your custom product to you, we always re-‐measure your garment and compare it to your profile before we ship it out to you. So long as you follow the instructions below, we’re certain you’ll end up with a perfectly fitted suit or dress shirt upon delivery. If you have any questions on any of the instructions, we encourage you to clarify with our customer service team, who is always available to walk you through measurements in more depth.
MEASURING TIPS: • Use a flexible tape measure, one made of cloth, ribbon, or soft plastic that will form to your
body as you measure. • All of the measurements we request are body measurements, not the size of the actual
garment we produce. So measure snug (but not too tight!), with room for one finger. to fit comfortably.
• While being measured, be careful not to overinflate your chest or suck in your stomach. Just stand relaxed, the way you would when you’re standing relaxed in a great-‐looking suit.
• Many measurements require you to measure horizontally across or around your body. Be sure that your tape is always parallel to the ground, and never offset.
• Once the measuring tape is in place, take a look from different angels to ensure you’re measuring accurately.

MEASUREMENTS: Full Shoulders
Your Measurement: inches What to Wear: Dress Shirt
Instructions: Measure across back of shoulders from the edge of your left shoulder bone to right. Start tape where the seam of a well-‐fitting dress shirt should be. Be sure to bring the tape up near the collar so you’re capturing the height of your shoulders in the arch you form from seam to seam.
Half Shoulder:

Instructions: Start measuring tape on top of your right or left shoulder, at its highest point near your neck (directly beneath your ear). Bring the tape down the slope of your shoulder to the edge of your shoulder bone, where the seam of a well-‐fitting dress shirt would be at the top of the arm.
Sleeve Length:

Instructions: Measure down your arm from shoulder to wrist. Start tape right at the corner of your shoulder, where the seam of a well-‐fitting dress shirt should be and measure down your arm while it’s fully extended. End tape where you like your dress shirt / suit to end.
Jacket Length:

Instructions: While wearing your suit jacket, measure from the highest point of your shoulder (start the tape right at the crest of your shoulder) near your neck, and measure straight down over your chest, beyond the waist to exact length that you would like your jacket to be.
Torso Length:

Instructions: Start tape measure at highest point of your shoulder, near your neck, and measure straight down over your chest and stomach, ending the measurement at the bottom of your belt, while wearing a pair of well-‐fitting slacks.
Back Length:
Your Measurement: inches What to Wear: Dress Shirt
Instructions: Start shoulder in the same location as you did while measuring your Torso Length, but measure down the length of your back. Keep the measuring tape straight (without bending it into the curve of your back) and record where the tape hits the bottom of your belt.

Chest:
Your Measurement: inches What to Wear: T-‐Shirt
Instructions: Measure around your chest at its widest point. Be sure to measure underneath your arms, with both arms at your sides. Keep measuring tape perfectly horizontal, and be sure it’s covering the widest points of your chest and back. Measure snug, but not tight.
Stomach:

Instructions: Measure around your stomach at its widest point. Keep measuring tape perfectly horizontal, and be sure it’s covering the widest point of your stomach.
Waist:
Your Measurement: inches What to Wear: Direct to skin
Instructions: Measure around your waistline, where you wear your slacks, as if the tape measure were the waistband of your suit pants. As tight as the measuring tape is around your waistline is as tight as the waistline of your slacks will be.
Hips:
Your Measurement: inches What to Wear: Lightweight shorts
Instructions: Measure around your hips and buttocks, at its widest point. It’s crucial that you measure around the widest point of your buttocks, at the crest. Ensure tape measure is perfectly horizontal, or parallel to the ground.
Front Chest:

Instructions:
Measure across front of your torso, over the largest part of your chest. Start measuring tape near your right armpit, where your arm meets the side of your chest, and stretch tape across to the other side of your body where your left armpit is – where your left arm meets the side of your chest.
Back (Across):

Instructions: Measure horizontally across your back, over the shoulder bones at their widest point while your arms are relaxed at your side. Start tape measure at the point where your left arm (near armpit) meets your side or back while relaxed, and stretch to that same point near your right armpit.

Neck:

Instructions: While your dress shirt is unbuttoned, slide tape measure underneath your collar and measure around your neck. This measurement should be close the neck size you wear when you buy off-‐the-‐rack.
Bicep:

Instructions: Measure around your bicep at its widest point while your arm is relaxed at your side.
Forearm:

Instructions: Measure around your forearm at its widest point while your arm is relaxed at your side.
Wrist:
Your Measurement: inches What to Wear: Direct to skin
Instructions: Measure around your wrist bone at its widest point while your arm is relaxed at your side.
Length (Outseam):
Your Measurement: inches What to Wear: Slacks / Trousers
Instructions: Start tape measure at the top of your belt and measure down the outseam of your slacks. End tape at the point that you like your slacks to hit while your shoes are off.
Rise (Crotch):
Your Measurement: inches What to Wear: Slacks / Trousers
Instructions: Start tape measure at the top of your belt in the front of your slacks. Take tape underneath and through your legs to the top of your belt on your backside. Tape measure should be snug.
Thigh:
Your Measurement: inches What to Wear: Slacks / Trousers
Instructions: Measure around your thigh at its widest point, usually up near the crotch of your pants. Measure snug, but not tight.
