Bright Heart Health, Inc. 2603 Camino Ramon, Suite 200, San Ramon, CA 94583 OUD INDIVIDUAL THERAPIST APPOINTMENT CHECKLIST As part of individual therapy appointments, counselors need to ensure clients complete their symptomatology survey and a drug screen, if appropriate. As part of Bright Heart Health’s OUD program, clients complete a symptomatology survey to capture information related to their recovery to inform clinical decisions. The survey is based on the Brief Addiction Monitor. You can learn more about the Brief Addiction Monitor here. In addition to the survey, clients are also required to complete a monthly drug screen. Bright Heart Health uses CLIA waived saliva screens. Instead of urine drug screens, saliva screens allow the client to remain on camera throughout the entire screening process, thus reducing tampering or adulterating the sample. In addition, saliva screens have fewer invalid samples necessitating an additional screen or confirmation test. Please follow the steps below to complete both activities. Step 1: Complete your individual session and leave approximately 10 min at the end of the appointment for drug screen and survey. Step 2: Request the client complete the weekly survey. If they do not have the web address to the survey, you can provide it here - https://www.brighthearthealth.com/weekly-survey Clients should take about 5 min. to complete the survey. PLEASE NOTE: Clients must use their Bright Heart ID (MRN) to complete the survey. If they don’t know it, they can find it in their welcome email or you can find it on their medical record in Kareo. Step 3: Please have clients complete the drug survey. You will be notified when drug surveys are available. Bright Heart Health ships drug surveys to clients.
