How soon must I report?
•Work-related fatalities-employers must report to OSHA within 8 hours of finding out about them▫These only have to be reported if they are
within 30 days of a work-related incident•Inpatient Hospitalizations, Amputation, or
Eye Loss- employers must report within 24 hours of learning about it

Inpatient Hospitalization Defined•“A formal admission to the inpatient
service of a hospital or clinic for care or treatment”
•Not required to report if inpatient hospitalization was for diagnostic testing or observation only
•If a heart attack occurs leading to an inpatient hospitalization, the employer is only required if it resulted from a work-related incident

Amputation Defined
•“Traumatic loss of limb or other external body part”▫This includes a part that has been severed,
cut off, amputated (either completely or partially)
▫Fingertip amputations with or without bone loss
▫Medical amputations resulting from irreparable damage
▫Amputations of body parts that have since been reattached

How do I report?
•By telephone to the nearest OSHA Area Office during normal business hours
•By telephone to the 24-hour OSHA hotline▫1-800-321-OSHA (6742)
•OSHA is currently developing an online means of reporting▫This is not currently up in operation yet

•Establishment name•Location of the work-related incident•Time of the work-related incident•Type of reportable event•Number of employees affected by event•Names of the employees affected by event•Contact person and his or her phone
number•Brief description of the work-related
incident

What is NOT required to report?•Motor vehicle accident on public street or
highway▫Employers ARE required to report if it
happened during a construction work zone•Occurred on a commercial or public
transportation system•Occurred more than 30 days after the
work-related incident for fatalities, after 24 hours for inpatient hospitalization, amputation, or loss of any eye

Heart Attacks, report or not?
•Employers are required to report a heart attack if it was due to work-related incident▫Such as brought on by strenuous activity
•Must report if heart attack led to a fatality•High heat is a gray area
