Introduction

There is a recent upsurge of number of accidents in the waste management and recycling

industry in Hong Kong, including struck by moving plants or vehicles, falling from plants or vehicles

and use of unsafe plants, etc. Ecospace Limited had decided to hire Denny Lee Safety Consultant

Limited to study the major hazards in the industry, and recommend different measures to reduce the

number of accidents occurring in the industry.

Ecospace Limited is contractor of 

Environmental Protection Department who

managing and operating Chemical Waste

Treatment Centre at Tsing Yi. Ecospace

 provides a collection service using 20 to 200

litre capacity containers for chemical waste.

A fleet of licensed chemical waste collection vehicles ranging from 5.5 to 16 tonnage lorries to bulk 

tankers and barges conveys the chemical waste. All chemical waste collected are delivered to the

CWTC for treatment. Cranes and folk-lift trucks were used to unloading and moving containers

within the facility.

Who might be harm?

Person might be harm such as employee, public visiting the site, pedestrian or other road

user, employee at client site.

What type of accident might happen?

Common type of accident might happen:

Struck by moving plants or vehicles. For example, hit by moving parts of plant,

traffic accident cause by vehicle.

Falling from height. For example, fall from plant,

fall from driver seat, fall from vehicle during

loading or unloading, fall from loading bay, fall

from top of vehicle during maintenance.

Slips or trips. Slips or trips due to poor 

housekeeping.

Use of unsafe plant. Accident due to poor design

of plant.

Chemical hazard. Accident due to spill of chemical or 

improper handling of chemical.

What cause accident to happen?

Accident might happen due to:

The vehicles themselves. Are they safe and suitable for their

use? Are they properly maintained? Do they need to be replaced with new, safer 

vehicles?

The routes or roadways used by the vehicles. Are they safe and suitable for the type

and number of vehicles using them? Are they properly maintained? Have you

considered nearby obstructions, curbs or edges?

What drivers are doing. Are there pressures on operators that might encourage them

to work less safely? For example, do they have to rush to complete their work on

schedule? Is there a risk of drivers becoming over-tired? Are they working safely, eg

when getting into or out of vehicles, during loading or unloading, and are they

observing routes and speed limits? Look for ‘short cuts’ that drivers may be tempted

to use in both routes and safety procedures.

What other people are doing. Are other workers, customers, or members of the

 public kept clear of workplace vehicles wherever possible?

Safety Management System

In order to minimize the likelihood of hazard being happened, it is necessary to implement the

safe system of work. All employees, contractors, maintenance personnel and visiting drivers must

understand their responsibilities in the safety management system. Include safety responsibilities in

 job descriptions and in training, raise safety issues in day-to-day contact with employees, and display

safety notices and risk assessment results. Decide which methods best suit the company and monitor 

their use to make sure they are properly followed.

Driver Management

Company should have a policy and procedure in place to address the requirements in

recruiting new drivers and to recruit the best driver available. Driver must obtained related

license as required by the legislation, such as driver license of specific type of vehicle, load

shifting machinery certificate, or lifting appliance certificate.

Driver’s behavior and performances is largely dependant on its remuneration and

motivation. As such, the salary package shall not structure in a way encouraging drivers

violations, to work or drive beyond the specific hours.

A system in place to ensure that the training needs of drivers are identified and satisfied in

an appropriate and adequate manner so that operations are carried out competently and safely.

The driver shall be trained based on the characteristics of the work activities. The training shall

 be delivered by approved trainer and shall be properly assessed and recorded.

The company should have a formal system and procedures to monitor and promote safe

driving behavior and performance. Unsafe behavior and traffic violations shall be counseled and

disciplinary actions taken on repeated offenders.

Drivers shall not operate a vehicle while under the influence of alcohol, drugs, narcotics or 

medication that could impair the operator’s ability to safely operate the vehicle.

Journey Risk Management

A Journey Risk Management plan is in place taking into account on routing of hazardous

substances. To identify risks and restrictions on the road that might be imposed by tunnels,

  bridge regulations. Within the constraints imposed above, select the routes that offer the

minimum traffic hazards. Process in place to identify temporary road hazards, including

engagement of the drivers in sharing these hazards and mitigation actions.

Vehicle / Equipment Management

A system shall be in place to ensure all equipment are appropriate for the products to be

stored, handles and transported. All equipment must comply with the relevant legal

requirements. The equipment must be maintained in good conditions and “fit for purpose”. Put

in place a system to capture and report defective equipment, which include the criteria in

correcting these defects.

Plants should consider the user safety at the design stage or 

modify it to be safe for use. Such as provide adequate access path and

 proper guardrail to prevent people to fall from height.

Guidance on the minimum protective clothing and safety

equipment required to be carried on all heavy goods vehicle

transporting company products, and the provision of such equipment.

Planning and Emergency Response

Documented procedures or standards should be prepared for daily activities, defining how

they should be conducted weather by the company’s own employees, or by others acting on its

 behalf to ensure technical integrity and of transfer knowledge effectively.

A Journey management system should be operated to ensure each journey is necessary,

 properly organized and supported.

The company should maintain procedure to ensure that facilities and equipment which it

designs, procures, operates, maintains and inspects are suitable for the required purpose and

comply with defended criteria.

Any changes in the personnel, vehicles, processes and procedures of land transport in the

company have the potential for adverse effects on health, safety and the environment. Changes

which may be critical to the management of safety of road transportation should be reviewed

 prior to implementation.

The company should have an emergency plan for responding to any accident. This emergency

 plan should contain:

Actions to be taken in case of different types of emergencies

Individual responsibilities

Arrangement for handling incoming emergency calls

Arrangements for 24 hours coverage.

Training requirement of responsible personnel.

The specific arrangements required by individual customers.

A list of different parties to be informed with their contact details.

Monitoring

The company should maintain procedures for the internal recording and reporting and

investigation of incidents which affected or could have affected safety performance so that the

relevant lessons can be learned and appropriate actions taken to avoid its reoccurrence.

Company need to ensure that information from active and reactive monitoring is used to

identify situations that create risks, and do something about them. Priority should be given

where risks are greatest. Look closely at serious events and those with potential for serious

harm. Both require an understanding of the immediate and the underlying causes of events.

Investigate and record what happened find out why. Refer the information to the people with

authority to take remedial action, including organisational and policy changes.

Audit and Review

Planned and systematic audits of transport operations together with management reviews

of performance should be established and maintained as an integral part of the transport

operations.

Monitoring provides the information to let company review activities and decide how to

improve performance. Audits, by company’s staff or outsiders, complement monitoring activities

 by looking to see if the policy, organisation and systems are actually achieving the right results.

They tell the company about the reliability and effectiveness of your systems.

Audit protocols should be established which ensure that adequate resources, personnel

requirements and methodologies are in place for the audit, together with procedures for reporting

audit findings and tracking the implementation status of audit recommendations.

Contractor Management

Contractor Management Programme is to provide the right people, equipment and

supportive environment so that the contract driver is an engaged, safe driver, making a

difference on the road.

Conclusion and Recommendation

Risk can be eliminate or lower the likelihood from happening, by using engineering control,

administrative control, and personal protective equipment.

Engineering control remove the hazard, either by initial design specifications or by applying methods

of substitution, minimization, isolation, or ventilation. Engineering controls are the most effective

hazard control methods, especially when introduced at the conceptual stage of planning when control

measures can be integrated more readily into the design. They tend to be more effective than other 

hazard controls (administrative controls and personal protective equipment) because they remove the

source of the hazard or reduce the hazard rather than lessen the damage that may result from the

hazard. They are also less dependent on the chemical user who, unfortunately, is subjected to all of 

the frailties which befall humans.

Administrative control consist of managerial efforts to reduce hazards through planning,

information and training (e.g. hazard communication), written policies and procedures (e.g., the

Laboratory Chemical Safety Manual, Hazard Communication Program), safe work practices, and

environmental and medical surveillance (e.g. work place inspections, equipment preventive

maintenance, and exposure monitoring). Because they primarily address the human element of 

hazard controls, they are of vital importance and are always used to control chemical hazards.

Personal protective equipment (PPE) includes a wide variety of items worn by an individual to

isolate the person from chemical hazards. PPE includes articles to protect the eyes, skin, and the

respiratory tract (e.g. goggles, face shields, coats, gloves, aprons, respirators). In some situations,

PPE may be the only reasonable hazard control option, but for many reasons it is the least desirable

means of controlling chemical hazards. PPE users must be aware of, and compensate for these

undesirable qualities. PPE does not eliminate hazards but merely minimizes damage from hazards.

The effectiveness of PPE is highly dependent on the user. PPE is oftentimes cumbersome and

uncomfortable to wear. Each type of PPE has specific applications, advantages, limitations, and

 potential problems associated with their misuse and those using PPE must be fully knowledgeable of 

these considerations. PPE must match the hazards and the conditions of use and be properly

maintained in order to be effective. Their misuse may directly or indirectly contribute to the hazard

or create a new one. The material of construction must be compatible with the chemical's hazards

and must maximize protection, dexterity, and comfort.

Risk Management is not a one-off process; it is continual process of planning, implementing,

monitoring, and reviewing. Reviewing provide information and feedback for re-planning and

modification of the management system.
