• Face to Face Documentation must be identified as “Face to Face” on MD Progress Note

• Encounter Date must be included• Patients Medical Condition related to the need

for Home Oxygen • Document: I certify that Home Oxygen is

medically Necessary for the Patient due to….I have discussed the need for Home Oxygen with the patient/caregiver

• Progress Note Must Contain:– Home Oxygen per Nasal Cannula/Mask at 2Liters

per Minute continuous/Nocturnally

• Other Documentation Required to Complete Referral is the Oxygen Saturation or Blood Gas Labs done at Office, Hospital, or Clinical Lab– Refer to Next Slide for Medicare Qualifying
