Nirav D. Shah, M.D., J.D., was appointed as Director of the Illinois Department of Public Health in January 2015 after dedicating his career to improving access to quality healthcare. As an attorney and former public health economist, Director Shah has advised professionals around the nation and world about cost-effective health systems and methods to enhance the delivery of efficient healthcarein policy and practice.

Elizabeth Salisbury-Afshar, MD, MPH, FAAFP, FASAM, is a physician certified in family medicine, addiction medicine, and preventive medicine. Originally from rural Taylorville, IL, she completed her undergraduate degree at the University of Illinois Champaign-Urbana, Medical Degree at Rush University in Chicago, and Masters in Public Health at Johns Hopkins in Baltimore, MD. Dr. Afsharis currently the Medical Director of Behavioral Health for the Chicago Department of Public Health where she contributes to the HealthDepartment’s work in mental health, substance use prevention and treatment, and violence prevention.

PRESENTER INFORMATION:Nirav D. Shah, M.D., J.D., was appointed as Director of the Illinois Department of Public Health in January 2015 after dedicating his career to improving access to quality healthcare. As an attorney and former public health economist, Director Shah has advised professionals around the nation and world about cost effective health systems and methods to enhance the delivery of efficient healthcare in policy and practice. A native Chicagoan, Director Shah received both his medical and law degrees from the University of Chicago. Director Shah lived and worked in Phnom Penh, Cambodia, as the Chief Economist for the Ministry of Health of Cambodia, where he focused on issues related to public health system efficiency. He also served as an epidemiologist for the Ministry, investigating and managing disease outbreaks across Cambodia.

Elizabeth Salisbury-Afshar, MD, MPH, FAAFP, FASAM, is a physician certified in family medicine, addiction medicine, and preventive medicine. Originally from rural Taylorville, IL, she is currently the Medical Director of Behavioral Health for the Chicago Department of Public Health where she contributes to the Health Department’s work in mental health, substance use prevention and treatment, and violence prevention. Prior to serving in this role, Dr. Salisbury-Afshar was the Medical Director at Heartland Health Outreach, the healthcare for the homeless provider in Chicago. She continues to volunteer a half day a week in an addiction clinic at Heartland Health Outreach, where she provides primary care and treats patients with opioid use disorder using medication assisted treatment.

Michael Blain, MD, is employed by and is Chief Medical Officer for Christopher Rural Health Planning Corporation (CRHPC), located in their Eldorado, IL and Shawnee-town, IL offices. Since CRHPC is a Federally Qualified Health Center (FQHC), he serves patients who may not be able to afford medical care. Dr. Blain provides full-time emergency medicine coverage at Harrisburg Medical Center. He also serves as Medical Director for both the Egyptian Public Health Department and the Gallatin County Wellness Center, a school-based health clinic managed by the Egyptian Health Department. In 2010, Dr. Blain received certification to prescribe Suboxone for opioid addiction. Currently, over half of his practice is dedicated to addiction medicine, specifically opiate dependence.

John A. Peterson, MD, completed both undergraduate and graduate training in Internal Medicine at the University of Illinois at Urbana-Champaign. Until 2016, he was engaged full-time in emergency medicine ending his practice as Medical Director of the emergency and hospitalist program at a critical access hospital in south central Illinois. Dr. Peterson is Immediate Past President of the Illinois Society of Addiction Medicine. He is Board-Certified in both Internal Medicine and Addiction Medicine. He served a large methadone clinic in Downers Grove, IL for 10 years. As an early adopter of buprenorphine medication-assisted therapy, he has treated over 800 opioid disorder patients through his private specialty clinic in Champaign, IL. Currently, he is the Medical Director of Chronic Pain Management at the Danville, IL Veterans Administration Hospital. He works with a comprehensive collaborative team of psychologists, physical therapists, interventionists, pharmacists, advance practice nurses, and physician assistants. Dr. Peterson has taken the lead in organizing the VA’s medication-assisted therapy for opioid use disorder as well as developing a fellowship in addictive diseases through the residency program of the school of medicine.

Garry Moreland, RPh, is the co-owner, Moreland and Devitt Pharmacy, and chief pharmacist at Sarah D. Culbertson Memorial Hospital, Rushville. He is a member of several professional associations including the NCPA, Illinois Board of Pharmacies (2001-2005), the American Pharmacists Association, and IPHA where he served as president and chairman of the board from 1998-99.

Thomas Huggett, MD, MPH earned his Bachelor of Science with Distinction in Biochemistry from the University of Wisconsin; Doctor of Medicine from the University of Chicago Pritzker School of Medicine; and Masters of Public Health from Johns Hopkins University School of Hygiene and Public Health. Working on the west side of Chicago since 1995, Dr. Huggett is a family physician with a passion to provide quality healthcare to residents of underserved communities. Collaborating with other community partners, he works to implement community-based and clinical interventions to improve the community’s health.

Stacy Sattovia, MD, MBA, is a hospitalist and Associate Professor of Clinical Medicine at Southern Illinois University (SIU) School of Medicine in Springfield, IL. She attended medical school at SIU School of Medicine and completed an internal medicine residency and chief resident year at University of Iowa Hospital and Clinics. She serves as the Director of Patient Safety and Quality Improvement for the Department of Internal Medicine and the medical director for the SIU Office of Continuing Professional Development.

Paul Bonucci, MD, MBA, has been an authorized buprenorphine prescriber and treating physician to opiate-dependent patients since 2013. Dr. Bonucci earned his MD degree at Northwestern University and has an MBA from the University of Chicago. He completed his residency at Cook County Hospital and is board certified in Emergency Medicine. In addition to serving as the Emergency Department Medical Director at Illinois Valley Community Hospital in Peru since 2009, he also founded an Urgent Care Center in Princeton in 2011, which was acquired by Perry Memorial Hospital in 2016.

Kevin VanKanegan, DDS, is a native Chicagoan and obtained his DDS degree from the University of Illinois at Chicago, College of Dentistry in 1993. He and his wife, Mona, operated a private practice in Oak Park, IL for 8 years. Subsequently, he obtained a Master of Health Professions Education degree from the Department of Medical Education of the University of Illinois at Chicago. Dr. Van Kanegan was a faculty member at University of Illinois at Chicago, College of Dentistry for 14 years and currently serves as the Clinical Program Director at Midwestern University, College of Dental Medicine-Illinois in Downers Grove.

Jeanne Holdren, APRN, Richard L. Roudebush VA Medical Center, is responsible for medication management and counseling for chronically mentally ill veterans. She has achieved two post-graduate nursing degrees and is now working on her doctorate degree in nursing. She is also a member of Sigma Theta Tau, Inc., an International Honor Society of Nursing.
