History of University of Perpetual Help System Dalta
Dr. Antonio Laperal Tamayo, founder and his wife Dr. Daisy Moran
Tamayo, co-founder together with their friend, Ernesto Palanca Crisostomo,
established on February 5, 1975 in Las Piňas City the Perpetual Help College of
Rizal, now the University of Perpetual Help System DALTA Las Piňas.
The first school building consisting of four floors (the present hospital) was
constructed in a remarkable record-breaking period of only four months, which
ordinarily takes one year to construct. The school building was created to house
the College of Nursing. Around 700 nursing student were enrolled and a few
were referrals from the Perpetual Help School in Manila. Perpetual was indeed
on its way to becoming a premier university south of Metro Manila.
The organizational structure of the former Perpetual Help College of Rizal
has interwoven within its fabric a closely-knit group of administrators and
educators. The administrative expertise of Mr. Crisostomo, the educational and
supervisory proficiency of former Pamantasan ng Lungsod ng Maynila acting
President and the Perpetual – Biñan First Chancellor Benjamin L. Intal and
former Bureau of Private Schools Regional Director Teofilo Gallardo were the
forefront keys in the formulation of the institution. The other members of the
founding group are the University’s First Chancellor Dr. Grace R. De Leon whose
loyalty stood by and supported the founders in hurling the challenges of
organizational growth; University Registrar Dr. Perla R. Sope, and consultants

ADMU’s Emiliano Armas of the Ateneo De Manila University and the De La Salle
University’s Oscar Bautista, Col. Jose C. Moran, father of Dr. Daisy, a Certified
Public Accountant and former President of the Continental Bank, facilitated the
needed funds of the project through his friends and the Development Bank of the
Philippines, where he served as its first manager of branches and agencies.
In 1997, the Commission on Higher Education granted University status,
thus the change of name to the University of Perpetual Help.
The University of Perpetual Help System DALTA is one of the subsystems
complementing the University of Perpetual Help System.
The other subsystem is the University of Perpetual Help System
JONELTA. University of Perpetual Help System JONELTA was founded and
established by Dr. Jose G. Tamayo, as its first President and Chairman of the
Board, with Co-Founder, Dr. Josefina Laperal Tamayo, as its Vice-Chairman and
Treasurer. With both co-founders now deceased, the University of Perpetual
Help System JONELTA is presently led by the first born of the founders, Dr.
Antonio L. Tamayo. He is currently the Chairman of the Board and Chief
Executive Officer of UPH System JONELTA. Incidentally, he is also the
Chairman of the Board and Chief Executive Officer the UPH System DALTA.
Dr. Tamayo earned his post-graduate courses in Hospital Administration
at George Washington University in Washington D.C., supplemented with special
studies on Hospital Management from Ateneo de Manila University, and a Ph. D

in Organizational Development from the Southeast Asian Interdisciplinary
Institute.
On June 10, 2003, the helm of leadership of the University of Perpetual
Help System DALTA was transferred to, Dr. Daisy Moran Tamayo as Second
President.
Dr. Daisy Moran Tamayo is a holder of a Master’s Degree in Nursing from
New York University, USA. She is also a doctorate degree holder in
Organization Development from the Southeast Asian Interdisciplinary Institute.
Complementing the leadership and management functions of Dr. Antonio
L. Tamayo and Dr. Daisy M. Tamayo are their two sons, Anthony Jose and
Richard Antonio who are both outstanding cum laude graduates of the University
of the Philippines.
Anthony Jose M. Tamayo assumed presidency of the UPHSD campuses
on January 2010. He is a Certified Public Accountant, MBA degree holder from
Kellogg School of Management, Northwestern University and MA in Education
degree holder from Harvard University.
Likewise, Richard Antonio M. Tamayo was appointed as President of the
University of Perpetual DALTA Medical Center. He is currently finishing his
Master of Business in Hospital Administration at Ateneo de Manila University.

In subsequent years, the UPHSD established two branches as well as
several agri-businesses which later incorporated into the DALTA Group of
companies. The units of DALTA Group are located in Las Pinas City; Molino,
Cavite; and Calamba City in Laguna. It also has a Medical Center in Las Piňas
City.
The University of Perpetual Help System DALTA– Molino was established
and inaugurated in May 1995 with an initial enrolment of seven hundred
students. Just a year after, the University of Perpetual Help System DALTA –
Calamba was established in inaugurated initially with 360 enrollees.
The University of Perpetual Help System DALTA subscribes to the
institutional philosophy that national development and transformation is
predicated upon the quality of education of its people. It is committed to the
ideas of teaching, community service and research, with “Character Building is
Nation Building” as its guiding principle. It is continuing in its efforts to achieve its
objective of maintaining and sustaining excellence in education. It has instituted
an extensive network of national and international linkages in Higher Education
and Business. It has established its indelible Perpetualite identity as it steadfastly
enshrines its Perpetualite values and aspirations.

Philosophy
The University of Perpetual Help System DALTA believes and invokes
Divine Guidance in the betterment of the quality of life through national
development and transformation, which are predicated upon the quality of
education of its people. Towards this end, the Institution is committed to the
ideals of teaching, community service, and research as it nurtures the value of
“Helpers of God”, with “Character Building is Nation Building” as its guiding
principle.
Vision
The University of Perpetual Help System DALTA shall emerge as a
premier university in the Philippines. It shall provide a venue for the pursuit of
excellence in academics, technology, and research through community
partnership.
The University shall take the role of a catalyst for human development. It
shall inculcate Christian values – Catholic in doctrine as a way of strengthening
the moral fiber of the Filipino – a people who are “Helpers of God”, proud of their
race and prepared for the exemplary global participation in the arts, sciences,
humanities, and business.
It foresees the Filipino people enjoying a quality of life in abundance, living
in peace, and building a nation that the next generation will nourish, cherish, and

Goals of College of Engineering
The College of Engineering is geared towards the development of well-
rounded engineers who are capable to meet the changing demands of industry
and who are committed to the proper utilization of the nation’s patrimony. It aims
to produce students who are intellectually and technically prepared for their role
in nation building and responsive to the needs of industry in the general,
specializes and highly skilled areas.

INTRODUCTION
On the job training or OJT is one method by which students is given a
chance to apply the theories and computations that they have learned from the
school. It is a program that provides opportunities for students to go through the
actual methodologies of a specific job using the real tools, equipments and
documents. In effect, the workplace becomes a development venue for a student
trainee to learn more about his chosen field and practice what he has learn from
academy.
OJT recognizes the need to train beyond that which is taught in the
classroom and also the need to upgrade and update skills throughout one’s
working life the main objective of OJT is to remove the learner from the
classroom environment to the workplace to continue learning in a manner that
will be able to be practically applied upon graduation.
History of Teodoro Construction Corporation
Established in May 15, 2001, founder Engr. Federick C. Teodoro TCC has
become a major provider in the design and construction of buildings and
infrastructures in the industry, categorized by PCAB as a Class A construction
firm. From its main office at Las Piñas City, this privately held company provides
innovative planning and strategy in the field of construction and engineering.

Part II
DUTIES AND RESPONSIBILITIES
GENERAL:
1. Oversee and render close physical inspection of the project execution,
installation, equipment and materials to ensure the Owner that the project
is completed by the Contractor in accordance with the Architect’s /
Engineer’s plans, specification and terms and conditions of the Contract
Documents.
2. Assist in coordinating the work between the Owner, the Architect and the
Contractor, in order to expedite the successful completion of the project.
3. Attend scheduled conferences as required in connection with the project
and render report on the current status and development of the work.
4. Help the electrical engineer in estimating wires and verifying the size of
wire in the project design is accurate.

PROJECT EXECUTION:
1. Conduct on-site observations and spot checks of the work with respect to
the requirements of the Contract Documents.
2. Disallow / notify the contractor regarding any materials or execution if work
that does not conform to the plans, specifications and Contract
Documents or found contrary to established engineering standards and
practices.
3. Supervise perform random samples of materials for quality control and
test them to approved Material Testing Laboratories. Results of such tests
are to be submitted to the Owner or commissioned Architects / Engineers.
4. Do not allow any change orders / extra works to be implemented until
shop drawings and notice to proceed have been issued and approved by
the owner.
5. After substantial completion of the Project, issue checklist of defective
works to be corrected, before conducting final inspection and issuance of
Certification of final Acceptance.

FIELD COORDINATION:
1. Assist when possible the Contractor in obtaining from the owner or the
Engineer / architect clarifications or additional details of information if and
when required for the proper execution for the work.
2. Evaluate suggestion for modifications which may be submitted by the
contractor and report them recommendations to the Architect / Engineer
and the Owner for final decision.
3. Be alert to the Completion Date and to Conditions of work and weather
which may cause delays and extensions in time and report them to the
owner and Architect.
4. When the project has been substantially completed, advise the Architect /
Engineer and the owner that the work is already for general inspection
procedure prior to Final Acceptance.
PAYMENT REQUISITIONS:
1. Reviews and verify with all concerned all request for partial and additional
payments as submitted by the Contractor and forward them with
recommendations to the Owner for disposition.

2. After Due review and verification, indicate in the Certificate of Final
Acceptance as issued to the owner, the final payment due to the
contractor and other pertinent recommendations for the Owner disposition.
RECORDS OF WORKS:
1. Maintain a daily log book, which include weather conditions, manpower
capabilities, equipment, materials, progress accomplishment, on-going
activities, test specimens and laboratory results.
2. Keep a cumulative record of percentages accomplishment of major
technical works, approved deviations and charges, cause of delays and
stoppages to serve as reference for bi-monthly accomplishment report.
3. Maintain a construction site a set of up-to-date plans and specifications
for reference in the execution of the work, as well as shop drawings,
change orders and additional drawings / details issued subsequent to
award of the Contract.

Part III
CONCLUSION
At the end of the on-the-job training, the trainee experienced things and
she learned from it. She gained knowledge about the work in a construction firm
especially in electrical side. The task given to her was the application of Electrical
Design 1 Lec and Lab of her course. Actual wiring in the site was a big help to
the trainee. The estimation of wire in the given project has a great connection to

the subjects. The trainees were technically given actual problems that have main
a main concern with the electrical engineering subjects. The nature of the career
course was the main focus of the training of the student will be useful if the
nature of their training have the major concern for their chosen career. The
trainee would able to use the experiences they have gone through during the
training whenever they are already in the field of specialization.
