ClinicalPracticeLearningEnvironmentTheclinicalexperienceisasignificantlearningcomponentwithinthenursingcurriculum.Itprovidesnursingstudentswithrichopportunitiestousethetheoryandskillslearnedintheclassroomandlabsettings.Itisalsotheplaceinwhichnursingstudentsseetheartandscienceofnursingappliedandbegintodeveloptheiruniquestyle.Theclinicalplacementexperienceisthesynthesizerfornursingeducation,affordingstudentstheopportunitytodevelopclinicalreasoningskills,nursingskills,timemanagementskillsandbecomesocializedintotheroleoftheprofessionalnurse.Asaresultofcompletingthenursingprogram,studentswillworkwithchildren,adults,andolderadultswithvariousdiagnosesandconditions,inacutecareinpatientsettings,long-termcarefacilities,andcommunitymentalhealth.Inallsettingsthegoalremainsthesame,thestudentprovidingsafe,competentnursingcare,basedonevidenceandbestpractice.Eachsettinghasagency-specificrulesandguidelinesandstudentsareexpectedtofollow.Nursingstudentsareguestsattheclinicalsites.SafetyisoftheupmostimportanceandnostudentshallengageinconductdetrimentaltotheCollegecommunityorclinicalagency.TheconductofeverystudentshouldreflectwellonWCCCDandinnocaseshouldthatconductincludebehaviorprohibitedbypolicy,regulationorlaw.Conductshallbedeemeddetrimentalif:

notassignedtothem.• Confidentialityandrespectforclientprivacymustbemaintainedatalltimes.• Entriesintothepatientrecordsmustbeaccurateandtruthful.• Studentsinteractprofessionallyandcooperativelywithclients,staff,visitors,otherstudents,

andclinicalfacultyatalltimeswhileinclinicalsetting–onclientunit,inconferenceroomor anyareaoftheagency.

• Anydisplayofstudentbehaviordeemed“unprofessional”or“uncooperative”willresultin studentbeingdismissedfromtheclinicalsite.Clinicalfacultywill:

NursingAdministrationwillmeetwiththestudenttodetermineeligibilitytoremaininthe NursingProgram.Intheeventtheincident(s)occurduringaweekendand/oreveningclinical, theclinicalfacultywillfollowtheaboveguidelines,however,willcontactNursing Administrationbyphoneonthedayoftheoccurrence(weekendoreveningclinical)and submitformonthenextbusinessday.

• Studentswillreportanysafetyconcerns,errors,orunexpectedoccurrencestotheclinical facultypromptlyforguidanceandresolution.

• Studentsdonotleavetheassignedareawithoutthepermissionoftheclinicalfaculty.• Studentsleavepromptlywhendismissedbytheclinicalfaculty,anddonotreturnfor

unsupervisedpatientcareactivities.• Studentswillnotremoveanydocumentsfromaclient’smedicalrecord.• Electronicdevicesarenotallowed(cellphones,cameraphones,pagers,etc.).

Studentsareexpectedtoshowincreasingclinicalproficiencythroughouttheprogram.PatientSafetyPatientsafetyisamajorconcerninhealthcareorganizations.TheJointCommission2014NationalSafetyPatientGoalsidentifyelementsofperformanceforthefollowingcriticalaspectsofcare:

• Identifypatientscorrectly.• Improvestaffcommunication.• Usemedicinessafely.• Usealarmssafely.• Preventinfection.• Identifypatientsafetyrisks.• Preventsmistakesinsurgery.

Eachclinicalperformanceevaluationtoolidentifiespatientsafetygoalsandexpectedperformancecompetenciesspecifictoyourclinicalagencyandexperience.Clinicalfacultywillholddiscussionwithstudentsthroughoutthecourse.Studentsareexpectedtobefamiliarwiththesafetygoalsastheyapplytothesettingandassignedclients.Forin-depthinformationabouttheNationalSafetyPatientGoalsandtheJointCommission,referto:http://www.jointcommission.org.LatexAllergyandSensitivityLatexallergyandsensitivityhasgrownasahealthcareconcerninrecentyears.Sinceanumberofproductsusedinhealthcarearemadeoflatex,(gloves,syringes,tubing,etc.),itisimperativethatallapplicantstoahealthcareprogramareawareofthisconcern.WCCCDNursingProgramdoesnotprovidea100%latex-freeenvironmentinlaborclinicalsettings.





IndividualswithlatexallergyandsensitivitymaynotbeabletomeetthestudentlearningoutcomesrequiredtocompleteWCCCDNursingProgramorsuccessfullyfindemploymentinhealthcare.Researcherssuggestthatearlyrecognitionanddiagnosisoflatexsensitivitymaypreventtheevolutionofthesensitivitytomoreseveresymptoms.Ifastudenthasalatexsensitivity,he/shemustindicatesuchontheformlocatedintheHandbook.Theformwillbeplacedinstudent’sfile.SeerequiredforminHandbook:“LatexAllergy/SensitivityWaiverStatement”.

Itisthestudent’sresponsibilitytonotifyeachnursingfacultyonthefirstdayofeachnursingcourse(includingnursingclinicalskillslabandsimulationlabfaculty)thatthestudenthasa

Employment,SkillParameters,andStudentClinicalPracticeStudentswhoarecurrentlyemployedinthehealthcaresettingmaynotuseskillsperformedintheirworkrolewhileintheWCCCDNursingclinicalsetting.Studentmustworkwithinthestudentroleunderthedirectionoftheclinicalfaculty.

AllianceforClinicalExperience(ACE)andProgramRequirementsACEplacementisaconfidential,collaborative,web-basedsystemthatallowsmemberhealthcareandeducationalinstitutionsadatabaseforstudentclinicalplacements.TheACEsystemmaintainsthenecessarymedical/healthinformationthatisrequiredforastudenttoworkinahealthcarefacility.ACE-MAPPisanonlinecourseandassessmentareaforthosetraininginthehealthcareprofessions.Asanursingstudent,youarerequiredtopayayearlyprogramfeeandmaintaingoodstandingwithintheACEnetworkforthedurationofyourenrollmentinthenursingprogram.TheACEprogramfeeincludesthreecomprehensiveonlinecoursesandassessmentsinOSHAstandards,HIPAArequirementsandsafetyprecautionswithregardtoBloodBornePathogens.Knowledgeofthesethreesubjectsisarequirementtobeginallclinicalrotations,anduseoftheACEsystemwillfulfillthisrequirementforaoneyearperiod.Youwillhavetheabilitytotakethecoursesandassessmentsatyourconvenienceandviewthestatusofyourcourseworkandclinicalrequirementsatanytime.

TheclinicalsitewillnotallowstudentstobeginclinicalatasiteorcontinueinasiteifACE-MAPPonlinecoursesandassessmentsarenotcompletedorhaveexpired.

Itisthestudent’sresponsibilitytomaintaincompliance.AllACEMAPPrequirementsmustbecompletedbythedesignateddateorthestudentwillbe

removedfromtheclinicalgroup.

ActivitiespropertoregisterednursingrequirealicensefromtheMichiganBoardofNursing.TheperformanceofsuchnursingactivitieswithoutalicenseisanillegalviolationoftheMichiganPublicHealthCode,subsection:Nursing.However,asastudentnurse,

studentsmaypracticeunderthedirectionofclinicalfaculty.





ACEMAPPandNursingProgramRequirementsTheACEsystemwillalsoserveasawebaccessiblechecklistofnecessaryclinicalandprogramrotationrequirementsthatinclude:

1. WCCCDNursingProgramHealthRequirementsStudentsmustprovideacurrenthealthhistoryandphysicalexaminationformwhenstartingtheNursingProgram.Thephysicalexaminationmustbesignedbyaphysician,nursepractitioner,orphysician’sassistant.TheoriginalcopyismaintainedintheNursingOffice.

• Physicalexaminationsarevalidfortwoyears,providedthereisnochangeinthestudent’s

healthstatus.NursingAdministrationwillrequirefurtherwrittendocumentationfromstudent’shealthproviderregardingpositivefindings.Itisthestudent’sresponsibilitytoobtainthewrittendocumentationuponrequestofNursingAdministration.

• Astudentwhoispregnant:1)mustsubmitawrittenstatementfromhealthcareproviderverifyingthestudentcancontinueinnursingcourses(clinical,lab,theory)withoutrestrictions.2)Followingdelivery,thestudentmustsubmitawrittenstatementfromhealthcareproviderverifyingthestudentcanreturntonursingcourses(clinical,lab,theory)withoutrestrictions.BothstatementsmustbereceivedbytheNursingOfficepriortoreturntocourse(s).NursingAdministrationwillreviewandrequestadditionalinformation,ifneeded.

• Studentswhoexperienceillnessorinjurythatcausesabsencemustprovidewrittendocumentationfromtheirhealthproviderverifyingthestudentcancontinueinnursingcourses(clinical,lab,theory)withoutrestrictions.ThisstatementmustbereceivedbytheNursingOfficepriortoreturningtocourse(s).

• LatexallergiesaretobedocumentedbyaphysicianandsubmittedtotheNursingoffice.Additionally,studentsmustsigntheformprovidedintheNursingStudentHandbook,“LatexAllergy/SensitivityWaiverStatement”.Theformandphysician’sstatementareplacedinthestudent’sfile.

• Anyphysical/learningdisabilitymustbeclearlydocumentedontheapplicationandphysicalexaminationformprovideduponadmissiontotheNursingProgram.StudentswithdocumenteddisabilitieswillbereferredtoWCCCDStudentServicesforpossibleaccommodations.Itisthestudent’sresponsibilitytofollow-upanyreferrals.

2. ImmunizationsEvidenceofabsenceoforimmunitytoinfectiousdiseasesisrequired.Specificimmunizationinformationorevidenceofantibodiesmustbeprovidedfor:

• Measles,Mumps,andRubella(MMRimmunization).• ChickenPox• T-Dap-Tetanusinjectionorevidenceofrecentboostermustbeestablished,mustprovidedate.• Tuberculosis clearancemustbe submittedannually in the formo f anegative purified

protein derivative (PPD) test (for those studentswho havehada positive test, verificationby chest x-raywithwritten report every2 yearsthatthereisnotactivedisease).

• HepatitisBimmunizationseries(signeddeclinationmustbeonfileifseriesisnotyetcomplete),orevidenceofimmunitytoHepatitisB,orasigneddeclinationtothenursingofficeandclinicalinstructor.

• Radioallergosorbent(RAST)allergytestingspecifictolatexsensitivity.• Fluimmunization.





3. BasicLifeSupport(BLS)BLStrainingevidencemustbekeptcurrentduringthetimethestudentisintheNursingProgram. Aphotocopyofa currentBLS trainingcardmustbesubmit ted to theNursingOfficeandprovided to the clinical instructor foreachclinical rotation. TheacceptableBLScourses are the:a)HealthCareProvider Course t h rough the AmericanHeartAssociation.

4. CriminalBackgroundCheckStudentmustprovidecriminalbackgroundcheckinformationfromCertifiedBackground.comTheSchoolCodeforthewebsiteis:ay20.ThisbackgroundcheckisrequiredtobesubmittedoncestudentisofferedandacceptsadmissiontotheNursingProgram.Thebackgroundcheckiscompletedannuallyoncestudententerstheprogram.

5.UrineDrugScreenStartingfall2014,studentsmustprovideaurinedrugscreeneveryyear.ApositiveurinedrugscreenmayresultindismissalfromtheNursingProgram.NursingAdministrationwillrequirefurtherwrittendocumentationfromstudent’shealthproviderregardingpositivefindings.Itisthestudent’sresponsibilitytoobtainthewrittendocumentationuponrequestofNursingAdministration.Allparticleslistedbelowmustbetested.Urinedrugscreenincludesthefollowing.Alllisteditemsmusthavenumericalvalues/results:

EvaluationofProgress

Evaluationofstudentperformanceisrecordedbytheinstructorintheory,clinicalandlaboratorysettings.Writtentestresults,clinicalperformancenotes,andlaboratorypracticeareutilizedbytheinstructortodocumentevidenceofastudent’sprogressandlearningneeds.Weeklyclinicalperformanceformsaremaintainedforeachclinicalstudenttodocumenttheextentofachievementofexpectedbehaviors(clinicalcompetencies).Thedate,timeandplacearenotedwithadescriptionofthebehaviorobserved.Studentsshouldexpecttheclinicalfacultytoprovideongoingevaluationthroughthecoursewithafocusonamidterm(formative)andfinalevaluation(summative).GradingScaleThefollowinggradingscaleappliestotheNursingProgram.Studentsmustachieveaminimumof75%intheorycourses:

A 93%-100%B 84%-92.9%C 75%-83.9%D 70%-74.9%E ≤69.9%

• Numericalgradesarebasedonthetheorycomponentofthecourseandwillbeconsistently

recordedtothehundredthplace,butwillneverberoundedup.Forexample,69.9%willnotberoundedupto70%.74.9%willnotberoundedupto75%.

• “Extracredit”assignmentsand“extrapoints”onanassignment(s)arenotallowedineithertheory,lab,orclinicalcourses.

• Forcourseswithaclinicalcomponent,studentsmustachievea“Satisfactory”gradeinclinical/labANDatleast75%inthetheorycomponenttosuccessfullycompletethecourse.Unsuccessfulperformanceinclinical/labortheorywillmeanunsuccessfulperformanceinthecourse.ThisisacoursefailureandstudentcannotprogressintheNursingProgramifthisoccurs.

QuestionsAboutExamandAssignmentGradesFacultywillscheduletimeafteranexamtoreview.Studentsshouldusethistimetoreviewtheirexamandaskquestionsaboutexamitems.Itisthefaculty’sdecisiontocompletethereviewinagroupsessionand/ormeetingswithindividualstudents.RequestingFinalCourseGradeChangesStudentsareencouragedtoestablisharelationshipwiththeirinstructorsandtodiscusstheirprogressregularly.Ifyoufeelyouhavereceivedafinalcoursegradethathasbeencalculatedincorrectly,contactthecoursefacultyimmediatelytodiscusshowthegradewascalculated.Ifunabletoreachthefacultyorfacultyhasnotresponded,contactNursingAdministration.

1. Thestudentmustcontacttheinstructorafterthegradeisissuedtodiscussanychangeingrade.Youmustinitiatecontactwithin90calendardaysoftheendofthesemester(orsession)duringwhichyouwereenrolled.Ifyouareunabletocontactyourinstructor,contacttheCampusInstructionalAdministratorforassistance.

2. Ifthestudentisdissatisfiedwiththeinstructor’sresponse,thestudentmayappealtheinstructor’sdecisionbyobtainingaStudentStatementGradeAppealFormfromtheStudentServicesAdministrativeOfficeatanyWCCCDcampus,whowilladvisestudentsontheirRights&Responsibilities.

3. TheStudentInformationandCourseInformationsectionistobefilledoutcompletelyandreviewedbyinstructionalstaff.

4. ThestudentmustcompletetheStudentStatementGradeAppealFormaccordingtothedirectivesandprovidenecessarydocumentation(copies)andanypertinentinformation.

5. ThestudentmustreturnthecompletedpackettotheappropriateInstructionalAdministratorasdirectedonthebottomoftheform.

6. Uponreceiptofthecompletedpackage,theInstructionalAdministratorwillrequesttheinstructortocompletetheInstructor’sStatementGradeAppealForm.

7. Uponreceiptofthecompletedpackageor,uponfailuretoreceivetheInstructor’sStatementwithin14calendardaysofthemailingdate,theInstructionalAdministratorshalleithersustaintheinstructor’sgradeorrecommendagradechangetobecarriedoutbytheinstructor.Thisdecisionshallbemailedtoboththestudentandtheinstructor(SeeGradeAppealStatement–InstructionalAdministrator).Failuretoreceivearesponsewithin14daysfromtheinstructordoesnotconstituteorimplyagradewillbechangedwithoutproperdocumentationfromthestudent.

8. ArecommendationtochangeagraderequiresthattheinstructorcompleteandsubmitaChangeofGradeformtotheInstructionalAdministratorwithin14calendardaysafterreceivingthewrittennoticeofthestudentgradeappeal.Anadministrativechangeofgradeismadewhentheinstructorfailstocomplyorisunabletocompletetherequest.

9. EithertheinstructororthestudentmayappealthedecisioninwritingwithintenworkingdaystotheGRADEAPPEALCOMMITTEE,c/oViceChancellorofEducationalAffairs.

10. Ifthegradeisappealed,theappealshallbeheardbyagradeappealcommitteegenerallycomprisedof:a. Onefacultymemberinthediscipline(ifpossible)butnottheinstructorwhosegradeisin dispute.Facultyareappointedbythefacultycollectivebargainingunit.b. OnerepresentativeappointedbytheappropriateStudentServicesAdministrator.c. ThePresidentoftheStudentGovernmentordesignee(ifpossible).d. ThecommitteeshallbechairedbytheViceChancellorofEducationalAffairsordesignee.

11. Boththestudentandtheinstructorshouldbenotifiedofthedate,time,andplaceofthehearing.BothshouldbeavailabletotheCommitteeshouldfurtherclarificationbenecessary.Attendanceisnotmandatory.

12. ThedecisionoftheGradeAppealCommitteewillbecommunicatedinwritingtoboththestudentandinstructor.Eitherthestudentortheinstructormayappealthecommittee’sdecisiontotheExecutiveViceChancellor.

13. IfanadministrativeChangeofGradeisnecessarythesignatureoftheappropriateViceChancellorofEducationalAffairsordesigneeisrequired.





AcademicProgressionAllstudentsmustachievea“C”orbetterinallnursingcoursesinordertoprogressinthenursingprogram.Receivingafinalcoursegradeofa“D”or“E”willnotallowastudenttoprogress.Incourseswithboththeoryandclinicalcomponents,failuretoachieveapassinggradeineithertheoryorclinicalcomponentresultsinfailureofthecourse.Clinicalevaluationsaregradedas“Satisfactory”or“Unsatisfactory”.Studentsmustsuccessfullymeetalloftheclinicalobjectives/learningoutcomestoreceivea“Satisfactory”clinicalevaluation.

TheNursingProgramisdesignedtobecompletedintwoyears.Currentsemestercoursesmustbecompletedbeforeprogressiontothecourseornextsemester.

LapseinEnrollmentAone-yearlapsesincelastenrolledinaNURcourserendersastudentineligibletoresumenursingcourses.Ifsuchoccurs,astudentcanreapplytotheNursingProgramin2years,providingthestudentmeetsadmissionrequirementsineffectatthetimeofreapplication.Thestudentunderstandsthatifreadmissionoccursafter2years,he/shewillstarttheNursingProgramwiththefirstsemestercoursesregardlessofthesemesterhe/shedepartedtheNursingProgram.Exceptionstothispolicyarestudentswhoaremilitaryreservistswhoarecalledtoactiveduty.Written,activedutyordersmustbesubmitted,uponreceipt,totheDeanofNursing.Uponreturnfromactiveduty,thestudentwillbeaccommodated,provided:1)contactwiththeNursingOfficeoccurswithin30daysofreleasefromactivedutyand2)resumptionofcourseworkoccursatthenextavailablesemesterintheacademiccycle.LeaveofAbsenceStudentsmayseekapprovalforatemporaryleaveofabsencefromthenursingprogramnottoexceed12monthssincetheirlastnursingcourse.CircumstancesregardingaleaveofabsenceneedtobediscussedwithandapprovedbytheDeanofNursingandmustbesubmittedinwriting.Therequestmustbeaccompaniedbysupportdocumentation,forexample,statementfromhealthcareprovider.TheDeanofNursingwillreviewandrespondinwriting.Astudentmustbeingoodacademicstandingatthetimeoftherequest(passingcoursewithminimumof75%,notinremediation,notreturningfromremediation,etc.).WithdrawalfromNursing(NUR)CourseCollegewithdrawalprocedures,asexplainedinthecollege’sscheduleofclasses,arefollowedrelatedtodeadlines,signaturesrequired,refunds,andgradesissuedrelativetothetimingofthewithdrawalforfullsemestercourses.For7.5weekcourses,deadlinesareproportionatelyadjusted.Astudentshouldbeawareawithdrawalfromanursingcourseisconsideredacoursefailure.FirstNURCourseFailureorWithdrawalfromNursing(NUR)CourseandRemediationProgramUponafirstNURcoursefailureorwithdrawalthestudentwillbeplacedonprobationandwillentera7.5weekRemediationProgram:





• StudentsarerequiredtoattendaRemediationOrientationwhichwillexplaintherequirementsfortheprogram.TheorientationisscheduledapproximatelyoneweekaftertheendofthefailedcoursesemesterandtheNursingOfficewillcontactthestudentwiththedate,timeandlocationoftheorientation.AttendanceattheRemediationOrientationismandatory.Ifastudentdoesnotattendtheorientation,itwillbeconsideredasecondcoursefailureandthestudentisdismissedfromtheNursingProgram.

• TheRemediationProgramrequiresstudentstomeetwiththeirassignedNursingFacultyMentor.Eachcoursehasaplandesignedtoaddresstheacademicissuesthatimpactedastudent’sperformance.

• StudentswillnotcompleteanynursingcourseswhileintheRemediationProgram.• StudentsarerequiredtosuccessfullycompleteallaspectsoftheRemediationProgramin

1. Explanationofcontributingfactorsrelatedtopastfailuresorwithdrawals.2. Explanationofplannedorexecutedresolutionofcausativefactors.3. Supportingdocumentationthatoutlinesaplanforsuccess.

Theformswillbereviewedandthestudent’sfilewillbeforwardedtotheCollegeWideRe-EntryCommitteeforreviewanddecision.TheCollegeWideRe-EntryCommitteemayeitherapproveordenyre-entry.





Ifre-entryisapprovedbytheCommittee:

a. StudentwillentertheRemediationProgram,whichisscheduledduringspringandfallterms.AftersuccessfulcompletionoftheRemediationProgramrequirements,studentwillbeplacedintheNURcoursethatwaspreviouslyfailed.Placementisbasedonspaceavailability.

CHAPTERNINE:GUIDELINESANDPOLICIESGuidelinesandpoliciespresentaframeworkforexplainingandaddressingissues.PolicesarepresentedhereaswellasotherchaptersoftheHandbook.Becomefamiliarwiththepoliciesandproceduresthatgovernsyourbehavior.BackgroundCheckConsistentwithSection20173ofMichiganPublicHealthCodeandtherequirementsofclinicalrotationpartners,acriminalbackgroundcheckisrequiredforallstudentsintheNursingProgram.Acriminalbackgroundcheckisconductedinordertoverifythattheindividualhashadnofelonyconvictionswithinthelast15yearsandthattheindividualhashadnomisdemeanorsinthelast10yearsinvolvingabuse,neglect,assaultandbattery,criminalsexualconduct,fraud,ortheft.Onceastudentisadmittedtotheprogramandundergoestheinitialbackgroundcheck,he/sheisrequiredtoimmediatelyinformtheDeanofNursingifhe/sheissubsequentlyarrestedfororconvictedofanyofthecriminaloffensescoveredbyMichiganPublicHealthCodeSection20173,setforthabove.Individualswhodonotpassacriminalbackgroundcheck,orwhoaresubsequentlyarrestedfororconvictedofcriminaloffensescoveredbyMichiganPublicCodeSection20173willnotbeplacedinclinicalcomponentofanycourse.Insuchevent,thestudentwillberemovedfromanyclinicalcomponentorplacement.Completionofallclinicalcomponentsforcourseswithatheoryandclinicalcomponentisagraduationrequirement.Adegreewillnotbegrantedtothosewhodonotsuccessfullycompleteallrequiredclinicalcomponentsandotherprogramrequirements.MichiganPublicHealthCode20173:http://legislature.mi.gov/doc.aspx?mcl-333-20173a

MedicationCalculationCompetencyStudentsmustsuccessfullycompleteadosagecalculationcompetencyexaminthefallandspringinordertocontinueprogressioninthenursingprogram.AllstudentsmustsuccessfullycompletetheMedicationCalculationCompetencyintwoattempts:

• 90%requiredforYearOnestudents.• 95%requiredforYearTwostudents.

Astudent’sinabilitytoachievetherequiredscoreintwoattemptswillresultinfailureoftheMedicationCalculationCompetency.Thestudentwillberemovedfromthecourseandenterthe7.5weekRemediationProgram.UponsuccessfulcompletionoftheRemediationProgram,thestudentcanreturntothecourse.





MedicationAdministrationGuidelinesTheWCCCDnursingfacultystronglybelievesintheneedforaccuracyinaccomplishingnursingtasks.Thisneedisespeciallyapparentintheareaofmedicationadministration.StudentsshouldreviewtheJointCommission’s“TenRightsofMedicationAdministration”asnotedinthereference,PotterandPerry,Fundamentalsofnursing,2017,p.626.Studentswillbeevaluatedintheclinicalsettingbasedonadherencetothesixrightslistedbelow:

Examplesofbehaviorsleadingtominimalorunsatisfactoryclinicalperformancemayincludebutarenotexclusiveofthefollowing:

1.Violationofanyofthemedicationrights.2.Lackofunderstandingofthemedicationtobeadministered.3.Improperidentificationofpatient.4.Improperinfusionofintravenous(IV)liquidsand/ormedications.5.Controlledsubstanceadministrationwithoutconsentofclinicalinstructor.6.ImproperuseofMedicationAdministrationRecord(MAR).7.Failuretoaskpatientaboutallergiespriortomedicationadministration.8.Failuretofollowfacultydirections.9.Anypotentialviolationwhichwaspreventedbyinstructororstaffintervention.

DrugandAlcoholTestingPolicyWayneCountyCommunityCollegeDistrict(WCCCD)hasaresponsibilitytomaintainasafeenvironmentforitsstudentsaswellasmaintainingsafeconditionsforpatients.Anystudentundertheinfluenceofdrugsoralcoholduringaclinicalexperiencemayposeserioussafetyandhealthrisks,notonlytothemselves,buttoallthosewhoworkwiththemandtopatientsforwhomtheyprovidecare.Theunlawfulpossession,use,orsaleofillegaldrugs,prescriptiondrugs,overthecounterdrugs,and/oralcoholintheclinicalagency,posesanunacceptableriskforunsafepatientcare.AnynursewhoisawarethatanotherpersonhasviolatedtheStateofMichiganNursingCodeandlegislativeRulesisobligatedtoreportthepersontotheMichiganLicensingBoardforconsiderationof

a. Slurredspeechb. Odorofalcoholonbreathorpersonc. Unsteadygaitd. Disorientedorconfusedbehaviore. Significantchangesinworkhabitsf. Observedbehaviorsindicativeofhallucinationg. Unexplainedaccidentorinjuryh. Sloppy,inappropriateclothingand/orappearance.i. Physicallyassaultive,undulytalkative,exaggeratedself-importance,makingincoherentor

irrelevantstatementsintheagencysetting.j. Excessivesickdays,excessivetardinesswhenreportingforclinicalorclass.k. Misseddeadlines,carelessmistakes,takinglongerthancustomarytocompleteworkl. Drugtestingmayberequiredforanystudentwhodemonstratesbehaviorofreasonablesuspicion(itemsa-qabove)intheclinicalenvironment.Drugtestingmayberequiredforanystudentwhodemonstratessuspicionofsubstanceabuseimpairmentandwhohasaccessand/ordirectresponsibilityforcontrolledsubstancesifknowndrugsofabusearemissingorotherwiseunaccountedforwhileintheclinicalagency.Thisdeterminationwillbemadeonacase-by-casebasisbytheagencymanageroradministratorandtheDeanofNursing,ortheDeanofNursing’srepresentative.Informedconsentwillbeobtainedpriortotesting.Feesassociatedwithtestingwillbetheresponsibilityofthestudent.NoncompliancewithrequestsfordrugandalcoholscreeningfromastudentwhodemonstratessuspicionofsubstanceabuseimpairmentwillbeviewedasaviolationoftheWCCCDDrugandAlcoholTestingPolicy.ThestudentmaybesubjecttodismissalfromtheNursingProgram.Thecollectionsitewillbeinastandardcollectionarealaboratoryoremergencydepartmentaspertheagencyprotocol.Non-acceptableandacceptablevaluesforlabresultswillbedeterminedaccordingtotheagencypolicies.Collectionprocedureswilladheretotherequired“chainofcustody”protocolasindicatedbytheassignedclinicalagency.





Thestudentwhoissuspectedofsubstanceabuseimpairmentwillbeescortedtothecollectionsitewiththeappropriatefacultymemberorassignedpreceptorandanagencystaffmember.Allwillremainatthecollectionsiteuntiltherequiredspecimensareobtained.Agencypolicywillbefollowedasrequired.Followinganincidentthatrequiresdrugoralcoholtesting,thestudentwillbesenthomebycaborresponsibleindividual.Undernocircumstanceswillthestudentbeallowedtodrivehomefollowinganincidentthatrequiresdrugoralcoholtesting.Thestudentwillbesenthomebycaborresponsibleindividual.Thestudent’sconfidentialitywillbestrictlymaintained.Thetestresultswillbecommunicatedtothestudent,theDeanofNursing,andthephysicianreviewingtheresultswiththestudent.RecordswillbemaintainedinaseparatefilebyWCCCDinasecuredarea.Requestsforinformationwillrequireacourtorderormaybereleasedbythestudent’ssignedwrittenconsentandliabilitywaiver.AnyviolationofthispolicybyastudentwillresultindisciplinaryactionincludingdismissalfromtheNursingProgram.





CHAPTERTEN:ETHICS,CIVILITY,ANDBEHAVIORSTANDARDSEthicalbehaviormeansthatanindividualislivingandworkinginanethicalway-byasetofstandardsthataregenerallyconsideredtoberightorgood.AsaNursingProgramstudent,itisexpectedyouwilldisplayethical,respectful,professionalbehaviorinALLlearningenvironments.FailuretodosoresultsindismissalfromtheNursingProgram.ConfidentialityTherighttoprivacyofstudents,faculty,staff,clients,familiesandotherhealthprofessionalsshouldbejudiciouslyprotectedbythoseassociatedwiththenursingprogram.Awarenessandrespectforrightsandprivilegesofothersisanexpectationofallwithinthehelpingprofessions.Studentshonor,respect,andmaintainconfidencesandprivacyofclients.Asstudentsinteractwithandencounterclients-inanyform–written,oral,orother–theymustpracticeconfidentialityunderallcircumstances.Aclient’sinitialsonlyareusedasidentifiers.Thismayvaryperclinicalsite.Talkwithyourclinicalfacultyabouthowaclient’sidentifyingdata(name,forexample),shouldbepresented/recordedasyoucompleteassignmentsanddiscussclientsduringpre/post-conferences.Whenknowledgegainedinconfidenceisrelevantoressentialtoothers,professionaljudgmentshouldbeusedbyallwhensharinginformation.Itistheresponsibilityofallthosewhohaveaccesstoinformationtoseethatsuchinformationisaccessibleonlytothosedirectlyconcernedwiththeindividual’shealthcaredelivery.Noreproductionoftheclient’shospitalrecordisallowed.Documentscontainedinaclient’shospitalrecordcannotberemoved.Onlineinformationaboutaclientcannotbedownloaded,printed,savedonanexternaldrive(flashdrive,forexample),forwarded,removedortransferredfromtheclinicalpracticesiteinanyway.Clientinformationshouldnotbediscussedoutsideoftheclinicallearningenvironment,andonlywiththoseinvolvedwiththeclient’scare.Failuretotakethisintoaccountisconsideredaninvasionofthepatient’srighttoprivacyandmayleadtothestudent’sdismissalfromtheNursingProgram.TheHealthInsuranceandAccountabilityActof1996(HIPAA)wasenactedAugust21,1996to:

• Protecthealthinsurancecoverageforworkersandtheirfamilieswhentheychangeorlosetheirjobs• Establishnationalstandardsforelectronichealthcaretransactionsandnationalidentifiersforproviders,

healthinsuranceplans,andemployers,• Givetherighttoprivacytoindividualsfromage12through18.Theprovidermusthaveasigned

disclosurefromtheaffectedbeforegivingoutanyinformationonprovidedhealthcaretoanyone,includingparents.

ReviewguidelinesforclinicalbehaviorsdescribedearlierintheHandbook.ScholasticEthicsCodeThepurposeofthiscodeistoencourageandpromotepositivelearningandethicalstudentbehavior,definebehaviorviolatingscholasticethics,specifyproceduresforthedeterminationofthefactsoftheallegedviolation,andtodefinepenalties.ItistheresponsibilityofthestudenttomaintainthehighestethicalstandardsinacademicachievementwithinthepositivelearningenvironmentprovidedbytheCollege.





GuidelinesforScholasticCodeofEthicsTheexpectationatWayneCountyCommunityCollegeDistrictisthattheprinciplesoftruthandhonestywillbepracticedinallacademicmatters.Therefore,theCollegeregardsactsofacademicdishonesty,includingsuchactivitiesasplagiarismorcheating,asveryseriousoffenses.Intheeventthatcheating,plagiarismorotherformsofacademicdishonestyonthepartofstudentsarevalidated,thestudent(s)willbedismissedfromtheNursingProgram.Thefollowingisaguidetoassiststudentsinobservingpositivebehaviorinscholasticethics:

• Studentsmustdotheirownworkandsubmitonlytheirownworkonexaminations,reportsandprojects,unlessotherwisepermittedbytheinstructor.

• Studentscanbenefitfromworkingingroups.Theymaycollaborateorcooperatewithotherstudentsduringtake-homeexaminationsonlyifspecificallyauthorizedbytheinstructorintheclasssyllabusoratthetimeoftheexamination;

• Studentsmustfollowallwrittenand/orverbalinstructionsgivenbytheinstructorordesignatedCollegerepresentativepriortotakinganexamination,placementassessments,test,quizzesandevaluations;

• Studentsareresponsibleforadheringtocourserequirementsasspecifiedbytheinstructorinthecoursesyllabus.

ScholasticCodeofEthicsViolationsStudentsenrolledincollegeassumetheobligationofconductingthemselvesinaccordancewiththehighestethicalstandards.Actionsconstitutingviolationsofscholasticethicsinclude,butarenotlimitedtothefollowing:

CheatingIntentionaldeceitduringthepursuitofacademiccoursework,tests,classassignmentsoractivitiesinanytestingarea,learningcenter,clinicalsettingortutorialsession,orinthegatheringofresearchmaterialsisconsideredcheating.Cheatingincludesbutisnotlimitedto:

• Copyingfromanotherstudent’stestpaperorknowinglyallowingyourtesttobecopied;• Usingmaterialsduringatestthatarenotclearlyauthorizedbythepersongivingthetest;• Collaboratingwithanotherstudentduringthetestwithoutpermission;• Knowinglyusing,buying,selling,offering,transportingorsolicitinganyofthecontentsofatest;• Takingatestforanotherperson,orpermittinganotherstudenttotakeatestforyou;• Bribingorattemptingtobribeanotherpersontoobtainapassinggradeorabettergradeonatestor

foracourse;• Intentionalmisconstruingfactsorincidentsrelatingtoanevaluatedexerciseorassignmentthat

wouldchangetheearnedgrade;• Electronic,internettransmissionsorwirelesstransferofdata.

Atnotimewillstudentphotographexam/quizorScantron,writecontentfromexam/quizontootherpaper,orremoveexam/quizorScantronfromclassroom.Ifsuchoccurs,studentwillreceivea“0”for

theexam/quiz,the“0”scoreiscalculatedwithfinalcoursegrade,andstudentisreferredtotheDeanofNursingforfurtheraction.





PlagiarismTherepresentationoftheworkofothersasone’sown,includingtheuseoftermpaperswrittenbyothersisplagiarism.Theuseofanother’swords,ideasorinformationwithoutacknowledgementisalsoplagiarism.Thestudentshouldseekguidancefromtheinstructoraboutacceptablemethodstobeusedtoacknowledgetheworkandideasofothers.CollusionObtainingfromorgivingtoanotherstudentunauthorizedassistanceormaterialinanycourseworkiscollusion.CompromisingInstructionalandTestMaterials/Misrepresentation/FraudUnauthorizedacquisition,tamperingwith,oralterationofinstructionaland/ortestingmaterialsfromdesks,cabinets,workrooms,classrooms,laboratories,instructor’soffices,tutoringlabs,testingareas,assessmentareas,secretarialoffices,Collegeofficesand/orotherareasiscompromisingtestmaterials.

StudentCodeofConductViolationsInappropriateand/orillegalstudentconductthatrepresentsgroundsfordisciplinaryactionincludes,butisnotlimitedtothefollowing:

• Dishonesty,suchasknowinglyfurnishingfalseinformationtotheCollege;• Forgery,alterationormisuseofCollegedocuments,records,identification,e-mailorotherelectronic

information;• Obstructionordisruptionofteaching,administrationoftheCollege,disciplinaryproceduresorother

CollegeDistrictactivities,communityservicefunctionsorotherauthorizedCollegeDistrict-sponsoredactivitiesonoroffCollegepremises;

• PhysicalactionsagainstanypersononDistrict-ownedorcontrolledpropertyorataCollege/Districtsponsoredorsupervisedfunction,oranyconductthatthreatensorendangersthehealthorsafetyofanyperson.Aphysicalactionisanactionthatresultsincontactwithanotherorwhichplacesapersoninreasonableapprehensionofharmfulcontact;

• TheftordamagetopropertyoftheCollege/DistrictorofamemberoftheCollegecommunityorofavisitortotheCollege;

• Possessionoruseoffirearms,explosives,dangerouschemicals,substances,instrumentsorotherweaponsthatcanbeusedtoinflictbodilyharmtoanyindividualordamagetoabuildingorgroundsofCollege/District-ownedorcontrolledproperty;

• FailuretocomplywithdirectionsofCollege/Districtofficialsorlawenforcementofficersactinginperformanceoftheirduties,and/orfailuretoidentifyoneselftothesepersonswhenrequestedtodoso;

• Useoftobaccoandtobaccoproducts(smoking)inunauthorizedareas;• Verbalabuseand/orabusivebehavior,includesexcessiveprofanity,threats,intimidation,

CHAPTERELEVEN:GRADUATIONPROCEDURESANDREQUIREMENTSThePinningCeremonyalongwithCommencementsignalsyouraccomplishmentoftheNursingProgram.Thefollowinginformationwillidentifythestepsyouneedtofollowtoensurealldocumentsareinorder.NursingPinningCeremonyThePinningCeremonyfornursingstudentsisheldtwiceeachyear(MayandDecember)alongwiththeRecognitionCeremonyforAlliedHealthPrograms.NursingstudentsarealsoencouragedtoattendtheFormalCommencementCeremonyforWayneCountyCommunityCollegeDistrictthatisheldannuallyinJune.ThePinningCeremonyisanimportantoccasionfornursingstudents.Thedistinctivepintheyreceivefromtheirschoolsymbolizesprofessionalachievementandtherighttopracticenursing.Itisalsoalinktoa1,000yearoldtradition.ManyyearsagoinmedievalEurope,organizationscalledguildsappeared;theseguildswereresponsibleformuchofthenationaltrainingofthetime.Theemblemswornbytheguildmembersindicatedtowhichguildaworkerwasamember.Theguildemblemsalsoindicatedservicetothecommunity.Ahospitalbadgewasgiventotheeducatednurse.Inthelate19thcenturymanyhospitalsadoptedFlorenceNightingales’ideasandstartedschoolsfortheeducationandtrainingofnurses.Outstandingstudentnurseswhoexcelledwereawardedgoldorsilvermedals.Gradually,allofthenursingschoolsstartedgivingamedalorbadgetoallgraduatesoftheirprogram.TheFlorenceNightingalelampthatiscommonlyincludedinthepinningceremonyisasymbolofthelightgivenbyknowledge.Today’snursingpinhasbecomeadistinctivemarkofnursingexcellenceandservicetoothers.Besidesidentifyingthesourceofthenurse’seducation,italsoindicatestothepatient,andothers,thatthenursehassuccessfullycompletedaparticularnursingprogramandhastheabilitytofunctionasanurse.ItistheintentoftheNursingProgramatWayneCountyCommunityCollegeDistricttoeducateandtrainhighlyqualifiednurseswithasoundknowledgeofthenursingprofession.ApplicationforDegreeFourthsemesterstudentsintheNursingProgrammustfollowWCCCDproceduresandtimelinestoapplyfordegree.Eligibilitytoparticipateincommencementexerciseshingesuponmeetingdegreecompletionrequirements.Priortothesemesterthestudentintendstograduate,heorshemust:

Itisthestudent’sresponsibilitytocoordinatealldegreeorcertificaterequirementsforcompletionwiththeappropriatecounselor,advisor,oracademicdepartment.Ifthestudentdoesnotcompletethegraduationrequirementsoutlinedinthesemesterrequestedontheapplication,heorshemustfileanotherapplicationinasubsequentsemesterandpayanewgraduationapplicationfee.TheDistrictRecordsOfficewillsendalettertothestudentacknowledgingreceiptoftheapplicationforgraduationandindicatingthatthestudenthasbeenplacedontherosterforthenextgraduationceremony.Foradditionalinformationaboutgraduationrequirements,pleasevisittheAdmissionsandRecordsOfficeattheNorthwestCampusintheWelcomeCenterorwww.wcccd.edu.GraduationwithHonorsStudentswhocompletedegreerequirementswithexceptionallyhighscholasticaveragesareeligibletoreceivedegreeswithhonors.Thosewhohaveearnedagradepointaverageof3.75-4.00areeligibletobegraduatedSummaCumLaude;agradepointaverageof3.50-3.74,MagnaCumLaude;agradepointaverageof3.25-3.49CumLaude.Incomputingthegradepointaverage,allcoursestakenatWayneCountyCommunityCollegeDistrictareconsidered.





CHAPTERTWELVE:REGISTEREDNURSELICENSURESuccessfulcompletionoftheNationalCouncilofLicensingExamination-RegisteredNurse(NCLEX-RN)forStatelicensureallowsWCCCDNursingProgramgraduatestopracticeasaRegisteredNurse(RN).Thefollowinginformationwillassistyou.ApplicationforRegisteredNurseLicensureStudentsreceiveinformationaboutapplicationforlicensureduringthefourthsemesteroftheNursingProgram.TheDeanoftheNursingcertifieseachapplicantashavingmet:

a. NursingProgramcourserequirementsfollowingcompletionofthefourthsemesteranddeterminationofgrades.

b. CompletionofrequiredNCLEX-RNreviewprograms.c. Completionattherequiredlevelofperformanceonthestandardizedexitexam

scheduledbytheNursingProgram.TheDeanofNursingwillforwardtotheMichiganBoardofNursingaCertificateofCompletionconfirmingstudenthascompletedallrequirements.RequestforTestingModificationonLicensureExaminationStudentswhorequireaccommodationsduringtestingsituationsareabletorequesttestingmodificationswhensubmittingapplicationtotaketheNationalCouncilofLicensingExamination-RegisteredNurse(NCLEX-RN).
