ASSESSMENT NURSING DIAGNOSIS RATIONALE GOALS AND OBJECTIVES INTERVENTION RATIONALE EVALUATION SUBJECTIVE CUES: Mother: Risk for infection related to inadequate primary defenses (broken skin specifically referring to exposed umbilical cord) The stump of the umbilical cord is an excellent medium for bacterial growth.
