The purpose of Team Based Care is to provide coordinated, high quality, and paent-centered care.” (IOM—Best Pracce Innovaon Collaborave, 2012). Collaboraon is defined as the communicaon, coordinaon and cooperave efforts of health care professionals around care delivery. Team based care is a systems approach to care delivery and is not a regulatory construct. The team concepts of well communicated care, coordinated informaon sharing and cooperave partnerships among paents and various health care professionals have always been the foundaon of nursing pracce. It is the belief of AANP that in systems ulizing coordinated care models, such as team based care, the following principles apply: The paent is the center of the health care team. Health care teams consist of paents and their health care providers. The health care team does not belong to a single provider, system or health care discipline. The health care team is dynamic, with the needs of the paent direcng who best can lead the team at any given point of me. Characteriscs of the health care team include: o Paent idenfied and supported goals o Mutual trust among all parcipants o Effecve communicaon o Measurable processes and outcomes in the provision of health care services All members of a health care delivery team should pracce to the fullest extent of their educaonal preparaon in order to provide high quality care for paents at the appropriate me and in various sengs to meet the paents’ needs and desires. The role of nurse praconers on the health care team will vary, but may include: AANP acknowledges that health care laws must be modernized and barriers removed for health care systems to gain full benefit from all team members and reach the potenal of coordinated team care models. This includes reforming scope of pracce laws to conform to the Naonal Council of State Boards of Nursing APRN Model Pracce Act and Model Rules and regulaons. As in any team, its ulmate success rests on all members performing at their full potenal. Research has demonstrated that ulizing NPs at the top of their ability in all sengs and health systems improves access, decreases cost and enhances high quality outcomes. AANP further believes: The contribuons of all health care professions are valuable and essenal to posive paent outcomes Coordinated care delivery comes in many forms Flexible frameworks are required for innovaon and creaon of emerging models to provide high quality care for diverse populaons of paents, families and the community Nurse praconer led teams are one of the available coordinated care delivery models for team based care For addional informaon regarding the parcipaon of nurse praconers as members and leaders of health care teams and the full potenal of the nurse praconer workforce, go to www.aanp.org.
