Accessing Web Site
1. Enter the web site address and save it as a favorite site on your web browser, or
2. Save the UM SON web site on your web browser.
3. Some course tools sites for some of the classes have a URL link that will take you to the log.
You will see a screen similar to this:
Choose “Enter Patient Data Log – all students” to enter or retrieve data

Be sure to familiarize yourself with what documents are available.
Once in the module, you should see buttons at the top, and a list of patient encounter records you have entered, if any.
Data Entry
Patient visit date: When logging in, the current date will
appear in this box. Please check that the patient visit date has been entered correctly before proceeding. Enter the date as follows: month/day/year (e.g. 09/01/04).
Click on the New Record button at the top of the page to enter a patient record.
Data Entry UMID#:
Enter your student ID number as assigned to you by the university (all 8 digits). If you try to submit with an ID that is less than 8 digits, or has any character other than 0-9, you will be prompted to correct the entry.
Track: Use the pull down to select your specialty
track.
Data Entry Term:
Use the pull down to select term in which the entry is made.
Course #:This is the course for which you are
seeing this particular patient to meet the clinical requirement.
i.e. if a patient is being seen for 566, use the pull down and select 566.
Data Entry Site:
Use the pull down to select the site where a patient was seen. If your site is not listed, contact [email protected] and ask to have that site added to the list.
Preceptor:Use the pull down to select the profession
of your preceptor.

This question is not about if the client is new to the practice but whether you as NP students are able to do follow up with clients or are you always seeing clients new to you.
Use the pull down and select Yes or No.
Data Entry Patient gender:
Use the pull down to select the gender of the patient.
Parity:For midwifery students only, all others may
ignore. Default is Not Applicable.
Patient age: Use the pull down to select the patient’s age
range.
Data Entry Patient race:
Use the pull down to select the ethnicity of the patient.
Reason for client visit:Use the pull down to select the primary
reason for the visit.
Exam type:Use the pull down to select the type of
exam.
Data Entry Reason for client visit:
Use the pull down to select the primary reason for the visit.
Exam type:Use the pull down to select the type of
exam.
Visit level:Use the pull down to select the visit
level/experience with the client during the visit.

Visit time:Use the pull down to select the time spent
with the client during the visit.
Payment source:Use the pull down to select the source of
payment for the client.

important nursing diagnosis.
Nursing Diagnosis #2 and 3:Use the pull down to select the second
and third nursing diagnosis, if appropriate.

important medical diagnosis.
Medical Diagnosis #2 and 3:Use the pull down to select the second
and third medical diagnosis, if appropriate.
Data Entry # of labs requested:
Use the pull down to select the total number of laboratory tests that have been requested.
# of diagnostic tests ordered:Use the pull down to select the total number
of diagnostic tests that have been ordered.
Medications category:Use the pull down to indicate medication
category for primary, secondary, and/or tertiary medication.

number of drugs prescribed.
# OTC ordered:Use the pull down to select the total
number of OTC medications ordered.

Counseling # 1:Use the pull down to select the most
important counseling program ordered.
Counseling # 2 and 3:Use the pull down to select the second
and third counseling programs ordered, if appropriate.
Data Entry Referral:
Use the pull down to select whether or not referral was provided.
Teaching # 1:Use the pull down to select the most
important teaching provided.
Teaching # 2 and 3:Use the pull down to select the second and
third teaching provided, if appropriate.
Data Entry Procedure # 1:
Use the pull down to select the most important procedure performed.
Procedure # 2 and 3:Use the pull down to select the second
and third procedure performed, if appropriate.
Responsibility:Use the pull down to select the
student’s level of responsibility.
Your Responsibility
Enter the data after seeing patients and provide reports as requested by your instructor. DO NOT wait until the last minute to enter data, this does not leave time to troubleshoot any problems encountered.

The Log Reports Click on any button that says View
Reports.
At the top of the Reports page is a pull-down, where you can choose which course you want your report to reflect, or leave blank to see all your records.
There are also instructions on how to create an excel file, which students mostly use when they are leaving the SON and want to take their data with them.
HELP
With your computer at home Send an email to [email protected] if you run
into problems. They will contact you when possible during their business hours of M-F, 8-5.
With computers in the campus computer centers: Get to know the Computer Help line number.
They are posted in all the computer pods. Live Body assistance is available in some of the
Computer Labs.
