NRA 150A and 150B CNA and HHA PROGRAM REQUIRMENTS

This packet of forms and information is essential for you to obtain all the needed requirements for our program. (See Bullets Below)

At your orientation meeting you will be given an overview of the program, a class calendar, and explanation of all the requirements.

Please submit your paperwork to the Health Science office, Room 4062, in the Race Building after making copies for yourself.

If you are on the waiting list, the admissions office will automatically put you on the roster by priority of the date you registered whenever a student drops. It is strongly recommended those on the wait list come to the first day of class as those who aren’t there will have to be dropped.

Please attend the orientation meeting and the first day of class to see if you make it into the class.

Screening Checklist Use this to keep track of all your paperwork. Noticethe starred items and the need to start with PPD #1 before other immunizations.

* The Immunizations must be obtained if you don’t have official record of them, unless you have immunity shown by positive serology (blood test). For most of them you may choose to have blood drawn, however this is NOT offered at Student Health Services and is usually more expensive than the immunizations. If you choose to have a serology done, have it done IMMEDIATELY. If the results show insufficient immunity, you will need to complete the immunizations on schedule.

Lead instructor will receive results via email. Flu Shot (FALL Semester ONLY), must get by 2 weeks prior to clinical.

Some of the expenses of the CNA program may be paid for using grant funds. Therefore, you may start collecting information or getting your immunizations (MMR, VARICELLA, HEPATITIS B, & TDAP (tetanus), FLU (fall semester only); and getting your 2-step PPD (free in Student Health Services, 1st floor of the Race building).

“2 STEP PPD” FAQ

WHAT IS A PPD?

IT IS A TB TEST. YOU GET A SMALL AMOUNT OF SOLUTION INJECTED WITH A TINY NEEDLE JUST UNDER THE SKIN OF THE INSIDE OF YOUR FOREARM. IT NEEDS TO BE READ IN 48-72 HOURS, OR 1 WEEK LATER IN A 2-STEP PPD. IF THE AREA IS RED OR RAISED (INDURATED) IT MAYBE CONSIDERED A POSITIVE PPD. THIS DOES NOT USUALLY MEAN YOU HAVE TB, BUT YOU MAY HAVE BEEN EXPOSED. THE OTHER POSSIBILITY IS A FALSE POSITIVE READING.

WHAT DOES IT MEAN TO HAVE A 2 STEP PPD?

THIS MEANS YOU HAVE A PPD AS DESCRIBED ABOVE, THEN THE DAY IT IS READ YOU MAKE AN APPOINTMENT TO COME BACK IN 1-2 WEEKS FOR ANOTHER (2nd) PPD, WHICH NEEDS TO BE READ 48-72 HOURS AFTER THAT ONE IS PLACED. AGAIN NO EXCEPTIONS ON THE TIME LINE.

MY DOCTOR SAID I ONLY NEED ONE!

THE COLLEGE AND THE HOSPITALS REQUIRE A 2 STEP PPD AS DESCRIBED ABOVE.

WHERE CAN I GET THE 2 STEP PPD’s DONE?

FREE AT STUDENT HEALTH SERVICES, BOTTOM FLOOR OF THE RACE BUILDING, BUT YOU DO NEED TO MAKE APPOINTMENTS, YOU CAN’T JUST DROP IN. YOU CAN ALSO HAVE THIS DONE AT YOUR DOCTOR’S.

WHY DO I NEED THE 2 STEP PPD DONE BEFORE I HAVE THE MMR?

IF YOU HAVE THE MMR SHOT FIRST IT CAN CAUSE A FALSE POSITIVE PPD. THE 2 PPD’S NEED TO BE DONE 1st.

For ClinicalBurgundy (or wine) pants and short sleeve uniform top (burgundy or wine scrubs) - 2 setsWhite walking shoes – closed toe. All white sports shoes are allowed. alcohol wipes, gait (transfer) belt, and surgical cap. (All of these may be purchased for you).

All lab pack supplies are available for purchase at Sonoma Surgical Supply (address below) or most other medical supply stores, if not provided for you.

Clinical Placement Background Screening and Review

As a Santa Rosa Junior College District student, you will be participating in courses that include mandatory clinical placements at local hospitals or advanced life support agencies (ALS agencies). Currently a number of these hospitals and ALS agencies require that all students submit to a background screening and review before commencing clinical rotations.

The screening process requires that students provide their name, date of birth, and social security number. The information will be submitted to Corporate Screening Services, Inc.,16530 Commerce Court, Cleveland, Ohio 44130-6305, Phone Number (800-229-8606, an agency that determines, based upon the information provided, whether a student has a record of any felony or misdemeanor conviction (s) or has been barred, excluded or otherwise been declared ineligible for federal program participation. (See the attached form, The Nature and Scope of the Investigative Consumer Report).

Corporate Screening sends the results of its screening to the CNA program Director. If the results indicate any felony or misdemeanor convictions or any bar, exclusion, or ineligibility for federal programs (negative background results), the College will notify the student of the negative background results, and the student will have the opportunity to dispute the results, and the student will have the opportunity to dispute the results as provided by state and federal law. A written summary of your rights under the Fair Credit Reporting Act is attached. If the negative background results are not cleared by the student, the results may then be forwarded to the placement hospital, if applicable, and the hospital determines that a student is ineligible for clinical placement, and the student may be denied admission to or dismissed from the program.

CALIFORNIA NOTICE OF RIGHTS FOR INVESTIGATIVE CONSUMER REPORTS

1786.22.

(a) An investigative consumer reporting agency shall supply files and information required under Section 1786.10during normal business hours and on reasonable notice.

(b) Files maintained on a consumer shall be made available for the consumer's visual inspection, as follows:

 

(1) In person, if he appears in person and furnishes proper identification. A copy of his file shall also be available to the consumer for a fee not to exceed the actual costs of duplication services provided.

(2) By certified mail, if he makes a written request, with proper identification, for copies to be sent to a specified addressee. Investigative consumer reporting agencies complying with requests for certified mailings under this section shall not be liable for disclosures to third parties caused by mishandling of mail after such mailings leave the investigative consumer reporting agencies.

(3) A summary of all information contained in files on a consumer and required to be provided by Section 1786.10 shall be provided by telephone, if the consumer has made a written request, with proper identification for telephone disclosure, and the toll charge, if any, for the telephone call is prepaid by or charged directly to the consumer.

 

(c) The term "proper identification" as used in subdivision

(b) Shall mean that information generally deemed sufficient to identify a person. Such information includes documents such as a valid driver’s license, social security account number, military identification card, and credit cards. Only if the consumer is unable to reasonably identify himself with the information described above, may an investigative consumer reporting agency require additional information concerning the consumer's employment and personal or family history in order to verify his identity.

(d) The investigative consumer reporting agency shall provide trained personnel to explain to the consumer any information furnished him pursuant to Section 1786.10.

(e) The investigative consumer reporting agency shall provide a written explanation of any coded information contained in files maintained on a consumer. This written explanation shall be distributed whenever a file is provided to a consumer for visual inspection as required under Section 1786.22.

(f) The consumer shall be permitted to be accompanied by one other person of his choosing, who shall furnish reasonable identification. An investigative consumer reporting agency may

New California Law Regarding Disclosure Forms and CRAs Scheduled to Go into Effect January 1, 2011

 

The Governor signed and secretary of state chaptered SB 909 on Wednesday, a bill that will require a person who procures or causes to be prepared an investigative consumer report for employment purposes to provide a consumer with the web site address or telephone number of the investigative consumer reporting agency where the consumer may find additional information about the agency's privacy practices.

The federal Fair Credit Reporting Act (FCRA) promotes the accuracy, fairness, andprivacy of information in the files of consumer reporting agencies. There are many types ofconsumer reporting agencies, including credit bureaus that gather and sell information about yourcredit worthiness to creditors, employers, landlords, and other businesses. The FCRA gives youspecific rights, which are summarized below.
