Community pharmacy contractors can now view the NHSBSA weekly dataset report to check if meeting three (Advanced Services, NHSmail and NHS website) gateway criteria of the PQS, HERE (available under the ‘PQS datasets’ section). The report can also act as a double check that all tasks have been completed correctly and if not, provides an opportunity to rectify errors accordingly. COMING SOON - Help Us Help You (HUHY) Stay Well This Winter (SWTW): November & December 2019… HUHY Stay Well This Winter is the 4th NHS England contractual public health campaign for all community pharmacy and Distance Selling Pharmacy (DSP) contractors. From the week commencing 23rd October 2019 all community pharmacies in England will receive a pack containing resources to support the mandated “Help Us Help You” winter campaign. This campaign will be running on TV, online and social media, as well on billboards across the country. The campaign will encourage the public to seek advice from their pharmacy team for their common ailments and help you support people to make the right decision and get the right treatment and/or advice at the right time this winter. Please ensure that you are displaying the “Help us help you stay well this winter” campaign material during November and December 2019: The pack will also include non-mandatory campaign materials such as “exemption checking” and “NHS 111 services” that you may also find helpful.

All these resources will arrive in the same box via the Healthcare Distribution Association delivery

to your pharmacy. If you have not received your pack by 15 November, visit the PHE Campaign Resource Centre,

Fluenz Supply… • Vaccine supply for the 2019/20 children’s flu programme: AstraZeneca is delaying their delivery of some batches of the childhood nasal spray flu vaccine (Fluenz Tetra®) due to be delivered to PHE in November. Message for Community Pharmacists from the NHS England & NHS Improvement Learning Disability Programme Team… We know that many of you will already be actively administering the flu vaccination to people with a learning disability as part of your day to day work. Following feedback from our user groups, the learning disability programme would like to provide you with resources that may help you engage more people with a learning disability. We know that around 1 in 3 deaths of people with a learning disability are caused by respiratory problems and the number of respiratory related deaths increases during winter. Having the flu jab can help avoid preventable deaths for people with a learning disability during this period.

Pharmacists have played a crucial role in helping protect vulnerable groups from flu by administering over 1.43 million flu vaccinations last winter. We want to support you to reach people with a learning disability so we can increase the number of people who are protected. An easy read poster and leaflet https://www.gov.uk/government/publications/flu-leaflet-for-people-with-learning-disability are available to download and display in your pharmacy. We also suggest that you ask people who present themselves as having a learning disability if they are protected against flu and administer the vaccination as appropriate.

Background and Interpretation: In the 2019/20 flu season, flu vaccine is being offered to all primary school-aged children and any children from six months of age in clinical risk groups. The flu vaccine that should be offered to most children in the eligible cohort groups is a live attenuated influenza intranasal vaccine (LAIV). It contains an attenuated (weakened) vaccine virus that is also cold adapted so that it cannot cause the disease that it protects against. Only one LAIV vaccine (manufactured by AstraZeneca/MedImmune) is available and is marketed as Fluenz Tetra for the UK and EU market. AstraZeneca is delaying their delivery of some batches of the childhood nasal spray flu vaccine (Fluenz Tetra®) due to be delivered to PHE in November. This is due to an ‘invalid’ test result in its routine quality testing process which need to be repeated before these batches of vaccine are released by the independent regulator. This is not related to the safety or the efficacy of the vaccine itself, which has passed all the other tests. Although we anticipate that this issue will be resolved quickly; there will be an unavoidable delay to the release of some stock, which may have a knock-on effect on the release of vaccine through ImmForm. It does not affect batches of the vaccine already distributed, or those currently available to order and use for the 2019/20 season. From Friday 18 October, GPs have been able to order sufficient LAIV vaccine to vaccinate up to 20% of their 2-3 year olds and under-18 at-risk practice population. This will provide most practices with just under half of the vaccine doses they are likely to require for their eligible patients. Recommendations are being made below on how to prioritise the use of LAIV within General Practice. School providers have been able to order LAIV for over two weeks and school vaccination should continue as planned. PHE will be closely managing the amount of stock available for GPs to order over the next few weeks. If AstraZeneca notify us of further delays with LAIV then PHE, NHS E/I and DHSC will consider further additional measures, in order to prioritise particular groups of children. Inactivated quadrivalent influenza vaccine for children (QIVe) is supplied centrally by PHE for those aged under two years and for older children in whom LAIV is contraindicated. This vaccine has been available for ordering for some weeks. The delay does not currently impact on vaccinations for at-risk adults or those aged over 65 years which is already underway in GP surgeries and pharmacies:

Implications for PHE Centres Screening and Immunisation teams in PHE centres are likely to receive calls from GPs and other providers concerned about the delay.

Implications for PHE sites and services Other sites should be aware of the issue and refer calls to the local Screening and Immunisation team.

Recommendations to PHE centres Screening and Immunisation teams are asked to liaise with the local NHS to ensure the recommendations below are cascaded to local GPs. Please continue to liaise with local schools providers to get intelligence on stock levels for the schools programmes. Recommendations for General Practitioners GPs are requested to implement the seasonal influenza programme as outlined in the Direct Enhanced Service Specification (https://www.england.nhs.uk/wp-content/uploads/2019/03/dess-sfl-and-pneumococcal-1920.pdf). As part of this GPs are required to call in those children who are eligible for flu vaccination, and to undertake recall for those children in clinical risk groups for influenza. Because of the phasing of supplies PHE recommends planning the childhood vaccination programme using following priorities:

• Children in high risk groups aged 6 months to 2 years – these children should be called and offered quadrivalent inactivated influenza vaccine (QIVe)

• Children in high risk groups from 2 to 18 years should be prioritised and offered LAIV (unless contraindicated).

o those aged 2-3 years and age 11-18 years should be called in and offered LAIV or a suitable quadrivalent influenza vaccine (QIV)

o those of primary school age (4-10 years) will be invited through schools, but should be vaccinated with LAIV or QIV if they choose to present in general practice

o Where a practice does not have LAIV available, vaccination of children in high risk groups should not be delayed and a suitable QIV should be offered as an alternative.

• Healthy children aged 2-3 years should be called and offered LAIV as the practice receives stock. Two year olds who are receiving vaccine for the first season are a higher priority than 3 year olds.

Responsible ordering will help support the direction of stock to those with greatest immediate need as it becomes available. Practice staff are asked to only order vaccine needed for the forthcoming week, even if this is below the maximum quota and to avoid stockpiling. Close adherence to the vaccine storage in the cold chain is essential to avoid vaccine wastage. Further details on eligible groups can be found in ‘The Green Book’: https://www.gov.uk/government/publications/influenza-the-green-book-chapter-19 An information leaflet is being developed for parents to explain which children are eligible for vaccination and where this will be delivered and will be posted shortly on the PHE website.

Implications and recommendations for local authorities

• Directors of Public Health (DsPH) should be aware of the potential interruption of the supply of Fluenz Tetra and the possible delay in the flu vaccination programme for children in their local authority area.

• DsPH should be aware in the event of a delay in the delivery of stocks of Fluenz Tetra, a prioritisation protocol will be implemented to ensure the best use of the supplies.

The launch date for the NHS Community Pharmacist Consultation Service (CPCS) has been set as 29th October 2019.

This date has been agreed by NHS England and NHS Improvement (NHSE&I), the Department of Health and Social Care (DHSC) and PSNC as part of the ongoing work to implement the first phase of the service, which will see NHS 111 call advisers referring suitable patients to community pharmacy.

To help pharmacy contractors and their teams to prepare to provide the service, which will connect patients who have a minor illness or need an urgent supply of a medicine with a community pharmacy, the draft service specification has been published on the NHS Business Services Authority (NHSBSA) website; the final version will be published by NHSE&I once it has completed its journey through their internal governance process.

Pharmacy contractors can now register to provide the service via the NHSBSA Manage Your Service (MYS) portal. This includes claiming the Transition payment supplement, which will be £900 for those who claim by 1st December 2019.

PSNC and NHSE&I will be holding a joint webinar for pharmacy teams and others interested in learning more about the service and how to implement it; the date and time of the webinar will be announced shortly. Information for parents of children and young people up to 18 years of agefluvaccination

WINTER 2019/20The

Flu vaccination is recommended for:

• children aged two or three years old (on 31 August of current flu season)

• all primary school-aged children and

• children and young people from 6 months up to 18 years of age with a health condition that puts them at greater risk from flu.

Most of these children and young people will be eligible for the live attenuated influenza vaccine (LAIV) given either in school or at the general practice. This year the delivery of LAIV from the manufacturer has been delayed and this means that deliveries to schools and general practices were later or did not start until after 21st October. Supplies of the injected vaccine for children are not affected by this delay.

Infants 6 months to under 2 years old.‘At risk’ infants under 2 are offered the injected vaccine so are not affected by this delay.

Two and three-year olds.Your child’s GP surgery should contact you about getting them vaccinated. If you haven’t heard from your GP by mid-November, contact them directly to make an appointment.

Primary school-aged children.Some vaccination sessions have already started and the rest will be held during this term or in January. Your local healthcare team will contact you via the school.

If your child is at primary school and has a health condition that puts them at increased risk from flu and you don’t want to wait, you can ask your child’s GP surgery to provide the vaccine if you prefer.

‘At risk’ young people aged 11 to 17 years of age.The surgery should contact them about getting vaccinated. If they haven’t heard from their GP by mid-November, they should contact the surgery directly.

Can I go elsewhere if my GP practice or school doesn’t have the LAIV in stock?Only schools and GPs will have supplies of LAIV and will schedule appointments for when vaccine is available.

What if my child is considered ‘at risk’?If you know that your child is considered ‘at risk’ and the surgery does not have any nasal vaccine you can ask your surgery to give your child the injected vaccine instead.

Updates on flu vaccine delivery and recommendations Public Health England have been receiving emails regarding a few specific issues and so have provided information as follows based on the information currently available from the National team.

1. Further delays with vaccine delivery

We have been informed that the supply of QIVe flu vaccine from Sanofi Pasteur remains a concern and will result in minor changes to confirmed delivery dates for some primary care providers. Sanofi Pasteur is currently making direct contact with all affected providers to confirm revised delivery dates. QIV flu vaccines are still available to order from several manufacturers and are recommended/ reimbursed for the NHS flu programme. The National team are developing a patient facing leaflet that providers can use to support the programme this year, particularly where practices are required to reschedule clinics. This will be made available shortly on the PHE webpage and will be circulated to you all once published.

2. Pharmacies receiving stock ahead of practices

We are aware that some practices and chain pharmacies have already received vaccine stock. Whilst ideally all would receive at the same time, please note that overall only around 10% of the currently vaccinated eligible patients receive their vaccination in a pharmacy setting. Uptake in patients in an ‘at risk’ group under age 65 is below 50%, suggesting there are significant cohorts of patients to be vaccinated which could take place in either setting.

3. Recommended vaccines for next year

For vaccination of those aged 9 to less than 65 years of age in an at-risk group JCVI advises the use of the following vaccines: •Quadrivalent influenza cell-culture vaccine (QIVc) •Quadrivalent influenza egg-culture vaccine (QIVe) For vaccination of those aged less than 9 years of age in an at-risk group JCVI advises the use of the following vaccines: •Quadrivalent influenza egg-culture vaccine (QIVe) Please note the cell based quadrivalent vaccine (QIVc) is not included in the over 65’s age group. This annual flu letter is expected to follow, which will confirm which vaccines will be eligible for reimbursement next year. Practices are advised to check if ordering early that any order can be cancelled or amended based on the national flu letter recommendations.

1st October 2019 The window opens for contractors to update their NHS website profiles or for distance selling pharmacies (DSPS) to send an email to the NHSBSA Provider Assurance Team (further details will be available in the NHS England and NHS Improvement (NHSE&I) Pharmacy Quality Scheme (PQS) 2019/20 guidance). This opens at 00:00am.

Contractors should start checking that all patients with diabetes, have had foot and eye checks (retinopathy) in the last 12 months (please note, eye checks are only for patients with diabetes aged 12 or over. PSNC will issue a resource to support contractors to meet this criterion soon.

DO NOT MISS THIS START DATE: CONTRACTORS NEED TO START WORKING ON THIS QUALITY CRITERION ON THIS DATE TO ENSURE THEY ARE ELIGIBLE TO MEET THIS QUALITY CRITERION.

This will allow contractors to complete their lithium audit, within a three month consecutive period (if the contractor starts the audit when the pharmacy opens on 29th November 2019 and makes their declaration on the final day of the declaration window (28th February 2020) after the pharmacy has closed for the day.

Contractors are strongly encouraged to start their lithium audit well before this date to reduce the risk of missing this deadline and allow themselves more flexibility on the date on when they make their declaration.

This is the final day by which contractors must have started their valproate audit. This will allow contractors to complete their valproate audit, within a three month consecutive period (if the contractor starts the audit when the pharmacy opens on 29th November 2019 and makes their declaration on the final day of the declaration window (28th February 2020) after the pharmacy has closed for the day. The contractor will need to start the audit when the pharmacy opens on 1st February 2020 and if they contractor is required to complete the audit over a four week period, make their declaration on the final day of the declaration window (28th February 2020) after the pharmacy has closed for the day.

Contractors are strongly encouraged to start their NSAID audit well before this date to reduce the risk of missing this deadline and allow themselves more flexibility on the date on when they make their declaration.

3rd February 2020 The window opens for contractors to claim for a PQS payment on MYS (this opens at 9am).

Dear colleagues, Re: The transfer of excess of QIV/LAIV flu vaccine stock between providers including Primary Care, NHS Trusts and School Age Vaccine Providers on the National Immunisation Flu Programme

In July 2019, we wrote to all primary care providers regarding the delayed World Health Organisation (WHO) vaccine virus strain recommendation and the fact that subsequently Sanofi Pasteur – one of the manufacturers – delayed their delivery schedule. This letter is attached as Annex A.

Following the previous communication, we have now in addition learned of a temporary delay in the supply of LAIV flu vaccine which will have an impact on the children’s flu vaccination programme.

The Medicines and Healthcare Products Regulatory Agency (MHRA) has the role of regulating the supply of medicines. Routinely, medication can only be distributed between providers where a wholesaler licence exists, however the MHRA has confirmed that with regards to the QIV flu vaccines recommended for those aged under 65 years in clinical risk groups and for LAIV flu vaccine recommended for the children’s programme in the 2019/20 flu season, it would not prevent the transfer of QIV or LAIV vaccine under the given circumstance of ‘in short supply’ or temporary ‘no supply’ available provided that:

• the practice, pharmacy, NHS Trust or school provider that is holding the excess QIV /LAIV stock has assurance that the vaccine has been stored in the correct temperature-controlled conditions;

• confirmed record keeping of temperature monitoring is available;

• the practice, pharmacy, NHS or school provider Trust that requests QIV/LAIV is able to verify the assurances given; and

• the vaccine can be transported appropriately under the right cold chain conditions.

Providers and NHS Trusts should refer to Public Health England’s protocol for the

Community Pharmacies are already able to transfer medicines to other healthcare providers without a wholesalers licence under specific circumstances, and should refer to the relevant guidance at https://www.gov.uk/government/publications/repeal-of-wholesale-dealer-licence-exemption-for-pharmacists

Transferring flu vaccines between practices is a failsafe measure; in the first instance alternative actions should be explored if a local stock issue is anticipated.

This should include for QIV:

1. Assessing the impact of the phased delivery of QIV flu vaccine and take local action to ensure sufficient supply of the recommended flu vaccine for those under 65 years in clinical risk groups in advance of the season

2. Confirming vaccine delivery dates with the manufacturer/supplier in advance of the season and schedule clinics/appointments at a time that vaccine becomes available. 3. Following Public Health England advice that, where a patient presents for immunisation and QIVe is unavailable, providers should offer QIVc if they have sufficient stock.

4. Where vaccine is unavailable, discussing the option of receiving the flu vaccine at the local pharmacist with the eligible patient. Please refer to the Public Health England and NHS England and NHS Improvement patient facing leaflet

5. Considering the transfer of vaccines where there is excess stock and in accordance with the advice detailed in this letter. GPs transferring excess stock between other practices and pharmacies should ensure they discuss this in the first instance with the CCG Flu lead and NHS England commissioning team

This should include for LAIV:

6. Considering the excess transfer of complete packs of LAIV vaccines between various providers of the programme in accordance with the advice detailed in this letter, and where the unavailability of vaccine through the ImmForm website would otherwise cause disruption to pre-arranged clinics and sessions.
