Main duties of the placement Supernummary in Intensive Care and in theoperating Theatre. In ITU, The F1 is expected toarrive on the unit in time for the start of the ICUround. They will take an active role in the wardround, examining patients, completing the dailyICU paperwork and understanding themanagement plans for the day. They will assistthe ICU resident trainee in performing dailyduties such as ordering investigations andensuring the results are followed up and writtenin the notes. They will have the opportunity togain experience in practical procedures such asinsertion of invasive monitoring lines, vascathsand airway management. They will accompanymembers of the ICU team to cardiac arrests, inreviewing critically ill patients on the ward and infollowing up ICU patients who have beendischarged back to the ward. During their timeon ICU they will gain an understanding of thekind of patient who is appropriate for ICUadmission and how to stabilise and manage acritically ill patient. They will also gainexperience in the principles of organ support andmonitoring. They will attend daily microbiologyrounds, weekly radiology meetings and weeklyICU teaching. In the operating theatres the F!will be expected to be in theatre well before thestart of the list and to have seen the patientspre-operatively with the Consultant. During thelist they will gain experience in anaesthetic care,gaining further skills in vascular access andcontrol of the airway. They will become familiarwith the anaesthetic process and also the care inthe postoperative period especially in Recovery.They will not be expected to take responsibilityof any patient under anaesthesia. During thesesessions they will receive appropiate teachingand learn the basics of regional, local andcentral nervous blockade and the maangementof patients with concurrent illness

anaesthetic rota and will be released for centralFoundation teaching and have separateteaching from Clinical Supervisor

Placement F1 CardiologyThe department The department of cardiology consists of ward
2/CCU at Southmead Hospital and ward 103 atFrenchay Hospital. There are 3 ConsultantCardiologists working at each site. There is acatheter laboratory at Frenchay hospital wherediagnostic angiography, angioplasty and pacingprocedures are carried out. The ED departmentand acute medical admissions unit are located atFrenchay hospital. Direct admissions areaccepted between working hours onto ward 2.Both sites have a cardiac investigations unit fornon invasive tests such as echocardiography,ECG monitoring and exercise stress testing.
The type of work to expect andlearning opportunities
All Foundation Doctors in hospital posts willgenerally be ward based during the ‘normal’working day and expected to deliver the dailymedical care of all the patients on their wardirrespective of specialty. Whilst on call in theacute admissions unit, the F1 will be involvedwith the generic clerking of patients beingadmitted and the ongoing care of the patients inthe unit.

the ward care of patients and the maintenanceof the patient’s medical record. They areexpected to attend the structured teachingprogrammes provided by the department andgrand rounds.

Mainly ward based from 9am-5pm apart from oncall duties.

The department The Department of Medicine for the Elderly/Care of the Elderly in North Bristol NHS Trust islargely based in specialty specific wards on bothmain sites, Wards 205 and 106 at Frenchay, andElgar Wards 3 and 4 and Malvern ward atSouthmead. All consultants work as GeneralPhysicians and Geriatricians, and each has anarea of special expertise, such as Cardiology,Parkinson’s disease, bone health &orthogeriatrics, respiratory disease, dementiaetc.

Main duties of the placement The F1 doctor will be responsible with other stafffor the ward care of patients and themaintenance of the patient’s medical records,including drug charts and discharge information.They will attend the “Board Rounds” and willhave opportunity to work with the consultants onWard rounds at least twice weekly, and also takeresponsibility for problems arising in patients onthe ward. They are expected to attend thestructured teaching programmes provided by thedepartment and the Directorate/ Academy/learning centre. The doctor will be responsiblefor such other specific clinical duties as allocatedby consultants including performing other dutiesin occasional emergencies and unforeseencircumstances.

Placement F1 Diabetes & endocrinology/ generalmedicine
The department The Department of Diabetes and endocrinologyis within the Directorate of Medicine. Staffincludes 4 consultants, including a Professor ofDiabetic medicine, 2 specialty registrars, onelecturer/ registrar, core medical trainees,diabetes specialist nurses, podiatrists anddietitians. All aspects of inpatient and outpatientdiabetes and endocrinology are dealt with. Thedepartment is currently based across 2 hospitalsites (Southmead and Frenchay hospitals) with asingle specialty inpatient ward at Frenchay.

Main duties of the placement To provide the daily ward care to the teamspatients on ward 106 (typically 13) andoccasional outlying patients on ward 203.To attend and present patients at consultant andregistrar ward rounds, carry out instructions,maintain the patients’ medical records, generatedischarge summaries, liaise with patients andrelatives and GPs, make referrals as requested,attend the weekly Multi-Disciplinary Teammeeting.When on emergency take to clerk and sortinvestigations for emergency admissions.

Placement F1 Infectious diseases & HIVThe department There is 1 Consultant who works in Infectious
diseases, including HIV & general medicine, and2 consultants in immunology & HIV (onecurrently on maternity leave). Junior staffinclude 1 FY1 trainee, 1 CMT, and one SpR inGU medicine. We are responsible for the careof medical patients on the K ward, the ID/ HIVunit, and K ward Annexe, with 11 generalmedical patients
The type of work to expect andlearning opportunities
As an F1 in ID/ HIV, you will be expected todeliver the daily medical care for the inpatients onthe wards.
Inpatient Care▪You will often attend Consultant or Registrar ward rounds, although sometimes you will beexpected to conduct your own ward round▪Typically each day you will be responsible for the care of 10 -22 inpatients.▪ID & HIV medicine will give you the opportunity tomanage a wide variety of infectious & tropicaldiseases, these are not rare, but you may not seemore than one or 2 of the same condition in your3 months with us.▪Sometimes patients have complicated and prolonged admissions, which will require carefuldischarge documentation, and telephonecommunication with GP. You will get theopportunity to learn about the management ofHIV, including issues of confidentiality, disclosure,testing, new diagnoses, and longtermcomplications. You will also learn themanagement of TB in the context of HIV and non-HIV infected patients. .
Clerking▪You will have ample opportunity to assess and manage acutely unwell patients with a variety ofconditions on take, and directly from clinic▪You will be expected to present these patients to the on call Consultant on the post take wardround, the ID consultant, or the on call HIVconsultant.

Placement F1 - General Adult Inpatient PsychiatryThe department Lime unit is a 23 bed acute psychiatric inpatient
unit. It primarily caters for patients from theSouth of Bristol. The patient care is broadlymixed, involving all acute and major psychiatricdisorders as well as dual diagnosis, borderlinelearning disabilities, physical morbidity andcomplex formulations involving personalitydisorder diagnosis. The acute inpatient unit isplaced within the care pathway of the functionalmodel hence very close collaboration with allother psychiatric teams takes place on a regularbasis e.g. through the CPA and ward roundprocess. We work particularly closely with theBristol Intensive Team (BIT) which provide crisisresolution and home treatment and partlyoperates from the ward, including facilitation ofearly discharge. Placement F1 Stroke medicine – Frenchay HospitalThe department Wards 18/19 have acute stroke beds
Patients with a diagnosis of stroke (cerebralinfarction, Haemorrhage or TIA are admittedto the wards directly from ED. If due to bedpressures patients are on a different wardthey are still looked after by this team. Allpatients who have had thrombolysis will beadmitted directly the neuroscience HDU andafter 24 hours transferred to the wards18/19. Supervised experience of seeingpatients in HDU will be available
There will also be some other Neurologypatients on the wards 18/19. These patientsremain under the admitting Neurology /Medical consultant.
Our target is that 90% of patients with acutestroke should spend 90% of their stay on astroke ward.

Opportunities for skills training such a lumbarpuncture, iv, ia puncture. Management of strokerelated complications
Multi-Disciplinary Meeting (MDM) ~EveryWeek
The details of discussions are noted down onthe yellow sheets in section 4 of the notes.
A representative from all teams tries to bepresent so that all aspects of each patientscare can be discussed. Action plans,discharge planning and estimated dischargedates (EDD) are agreed for each patient.The MDM is chaired by the consultant,registrar or senior nurse depending on
availability. All ward staff present help withdocumentation in the MDM to maintainspeed and efficiency
Progress Meetings
Patients with potentially complex dischargeplans may require a progress meeting, sothat the team can discuss with the patient,family and other carers the progress thepatient has made so far, and set goalstowards discharge planning. It is important tohave the 1st meeting within 2 weeks ofadmission to hospital.You may be asked to give an overview of thepatient’s medical condition and prognosis.Patients and their relatives may alsoapproach you at separate times to discussissues. The consultants are all happy todiscuss with relatives if necessary,particularly for those who have a poorerprognosis - please organise this through thesecretaries.Supervised training in generic skills e.g.Breaking bad news. Prescribing and drugsmanagement.Care planning meetingsLed by the social worker to plan placement/packages of care.DischargesPatients TTAs need to be written and inpharmacy ideally in the morning but at verylatest by 3pm on the day of (or prior to ifappropriate) to discharge. Compliance aids(Dossett boxes etc) require at least 24 hoursnotice.
It is important to ensure that secondaryprevention is addressed as an inpatient, andpatients are medically ready for discharge(investigations done, warfarin etc) in line withthe expected discharge date.
We do not provide routine follow up- onlythose with outstanding medical issues. Allother patients should be directed to their GP.It is important that the discharge summary isexplicit in any follow up requirements.
Most patients on wards 18/19 will requiretherapy input (OT, PT, SLT etc) prior todischarge. Please check the notes fortherapy discharge plans.
The Multi-Disciplinary TeamOne of the key points to remember on theunit is that we function as a team. You mayfrequently find that the therapists who workclosely with the patients are often the bestsource of information about progress etc andcan help your management of the patient.
Equally, they may come to you for advice asto the best way to manage a patient. It isespecially important to remember thatpatients may not be fit for discharge justbecause all their medical problems havebeen addressed. The team comes to aconsensus on issues such as these, whichare discussed regularly at the multi-disciplinary meeting (MDM).

Main duties of the placement The F2 is an integral member of the ICU team.They take an active role in the ward round,examining patients, completing the ICUpaperwork and understanding the managementplans for the day. They perform the ward workduties such as ordering investigations andensuring the results are followed up and writtenin the notes. They carry the cardiac arrest bleepand attend cardiac arrests supported, wheneverpossible, by the anaesthetic registrar. Theyreview critically ill patients on the ward and learnabout appropriateness of admission to ICU,stabilisation and transportation of critically illpatients. They have the opportunity to gainexperience in practical procedures such asinsertion of invasive monitoring lines, vas cathsand airway management, both on the ICU and intheatres. During their time on ICU they will learnabout the principles of organ monitoring andorgan support. They will attend dailymicrobiology rounds, weekly radiology meetingsand weekly formal ICU teaching.

The GP is the first point of contact for most medicalservices. Placement MAU – F2 in Acute MedicineThe department There are 3 Consultants who work in Acute
Medicine. We are responsible for the care ofmedical patients on the two AAUs (Wards 105and 107). This includes inpatients as well asambulatory care patients in the lounge/seatedassessment area.
The AAUs are where the vast majority ofpatients are admitted on the medical take.Patients may stay here for anything from a fewhours to a few days.
The lounge has day attenders who often willneed a medical review.

As an F2 in Acute Medicine, you will beexpected to deliver the daily medical care for theinpatients on the AAUs, as well as providingmedical reviews for lounge patients. For oneweek in six you will spend your days clerkingpatients.
Inpatient Care▪You will often attend Consultant or Registrar ward rounds, although sometimes you will beexpected to conduct your own ward round▪Typically each day you will be responsible for the care of 14 inpatients. Placement F2- NeurosurgeryThe department The department of neurosurgery is a tertiary
referral unit serving the counties ofGloucestershire, Somerset and Wiltshire inaddition to the city of Bristol. It is one of thelargest neurosurgical units in the country. Over2600 emergency and elective neurosurgicalprocedures are performed each year. There are13 consultant neurosurgeons and all the majorneurosurgical sub-specialties are represented.The department consists of two male and twofemale adult neurosurgical wards with a total of67 beds, 12 of which comprise the Neuro HighDependency Unit. There are four neurosurgicaltheatres; one is dedicated to emergencyprocedures.
The department is located in close proximity tothe Emergency Department, two Intensive CareUnits and the Neuroradiology department withaccess two CT scanners, two angiogram suitesand three MRI scanners, including a new 3Tesla scanner.
Emergency referrals are dealt with by an ‘on call’team consisting of a consultant neurosurgeon, aspecialist registrar (who is the first line ofcontact) and a senior house officer grade doctorproviding ward cover.
The type of work to expect andlearning opportunities The work is primarily ward-based, although there
will be opportunities to attend theatre and out-patient clinics. Within a team structure led byconsultants and registrars, F2 doctors will havethe opportunity to deliver medical care toneurosurgical patients, pre- and post-operatively. Most of the work is defined in astructured daily morning ward round. Mostelective patients are pre-clerked in theneurosurgical pre-admission clinics. Patientstransferred to the unit on an emergency basiswill require review and initial evaluation onadmission, under the supervision of the on callregistrar and consultant.
When on call, neurosurgical F2’s also cover thein-patient neurology service.

consultant nephrologists, all of whom participatein a specialist renal on call service. Renalmedicine is a tertiary referral specialty. Progress MeetingsPatients with potentially complex dischargeplans may require a progress meeting, so thatthe team can discuss with the patient, family andother carers the progress the patient has madeso far, and set goals towards dischargeplanning. It is important to have the 1st meetingwithin 2 weeks of admission to hospital.You may be asked to give an overview of thepatient’s medical condition and prognosis.Patients and their relatives may also approachyou at separate times to discuss issues. Theconsultants are all happy to discuss withrelatives if necessary, particularly for those whohave a poorer prognosis - please organise thisthrough the secretaries.Supervised training in generic skills e.g.Breaking bad news. Prescribing and drugsmanagement.Care planning meetingsLed by the social worker to plan placement/packages of care.DischargesPatients TTAs need to be written and inpharmacy ideally in the morning but at verylatest by 3pm on the day of (or prior to ifappropriate) to discharge. Compliance aids(Dossett boxes etc) require at least 24 hoursnotice.It is important to ensure that secondaryprevention is addressed as an inpatient, andpatients are medically ready for discharge(investigations done, warfarin etc) in line with theexpected discharge date.We do not provide routine follow up- only thosewith outstanding medical issues. All otherpatients should be directed to their GP. It isimportant that the discharge summary is explicitin any follow up requirements.Most patients on wards 18/19 will requiretherapy input (OT, PT, SLT etc) prior todischarge. Please check the notes for therapydischarge plans.The Multi-Disciplinary TeamOne of the key points to remember on the unit isthat we function as a team. You may frequentlyfind that the therapists who work closely with thepatients are often the best source of informationabout progress etc and can help yourmanagement of the patient. Equally, they maycome to you for advice as to the best way tomanage a patient. It is especially important toremember that patients may not be fit fordischarge just because all their medicalproblems have been addressed. The department The Department of Orthopaedic Surgery at theAvon Orthopaedic Centre, Southmead Hospitalcomprises 30 Consultants the majority of whomare North Bristol NHS Trust (Southmead andFranchay Hospitals) based but also consultantsbased at the Bristol Royal Infirmary who havetheir elective operating lists and pre-operativeassessment clinics at the AOC. Consultant sub-specialist interests include young adult/adolescent, hands, shoulder & elbow, spine, hip,knee and foot & ankle. Main duties of the placement Each F2 doctor will be attached to 2 or 3consultants who attend the AOC on their pre-opassessment and operating days. Eachconsultant has a registrar +/- fellow who arepermanently in the department and may work for1 or 2 consultants. There is always a Registraron call covering the unit during the day until 9pmafter which time any queries can be directed tothe on call registrar at Frenchay Hospital. The F2will be involved in the day-to-day ward care ofpatients and the contemporaneous medical notekeeping during daily business ward rounds andweekly consultant ward rounds. Whilst on callthe F2 doctor will cover the AOC wards and referup to the on call registrar or liaise with medicalcolleagues as required. They will rotate throughweeks of night shifts during which time they willbe part of the Hospital at Night team.
