Hi once again,I have reviewed the debrief for the release on 20 August and attached my responses to the issues that were raised.
1. Noted.2. Noted3. Noted. It is important for staff training and media releases that GoPro vision is taken for all releases. This includes
underwater vision of the entangled animal.4. I support the implementation of issues 4,5,&6 and recommended the IRB’s have Fisheries painted on them. I have
discussed them with Ross Quinn A/Director. He will discuss these issues with senior management later this week. Iwill advise you of the outcomes in due course.

Marine Animal Release TeamLarge Whale Disentanglement – Debrief of Release
DEBRIEF OF RELEASE CONDUCTED ON 20 / 08 / 2016
DATE OF DEBRIEF 22 / 08 / 2016 TIME 1420 hrs
PERSONS PRESENT FOR DEBRIEF:
NAME ORGANISATION NAME ORGANISATIONRob McDonald QBFP Ryan Stove QBFPMark Saul QBFP Neil Pye QBFPNicole Burow QBFP Fay Rohweder QBFPPhilippa Corbett QBFP
Shark Contractor advised MS and RM of entrapment at approximately 6.55am.Response time was exceptionally good and QBFP staff were first on scene. Whalewas on the surface and looked healthy. A risk assessment was conducted.
Communication was good, radios worked well.
RS stated he thought there was rope still on the dorsal, when Sea World started cutting the rope at the tail. Not confirmed.
NB stated there was really good team work between QBFP staff.
One medium pole and a large shoehorn knife were lost. More equipment is in stock.
RM reminded staff to follow QBFP procedures and do risk assessments and not be pressured by Sea World.
ISSUES RAISED
1. RS expressed concern about travelling through Currumbin Creek bar with waves and the large amount of boating traffic.
Action: Travel with caution when crossing the bar.
2. RM stated there were no zip ties on ‘Mantis’.
Action: MS has put a couple of packets of zip ties on ‘Mantis’. More to be purchased.
3. Extension Pole for Gopro was not taken to release.
Action: Extension pole to be taken to next release.

4. RM recommended ‘Fisheries’ be put onto the back of wet suits instead of ‘QBFP’ with a grab handle.
Action: Request permission from Manager SCP to stitch a panel onto the back of wet suits with the words ‘Fisheries’ and a grab handle.
5. NB asked if permission could be given for media to be contacted so staff can do media interviews on the beach when the MART vessel returns to shore.
Action: Request permission from Manager SCP to organise for media to be waiting on the beach and do interviews when MART vessel returns to shore.
6. PC stated public ring Sea World first instead of Shark Hotline and suggested a media release at the start of the season.
Action: Recommend to Manager SCP that a media release be put out at the start of the season advising the public of the hotline number to call when a whale is entrapped.

Date: 20/8/16 Time Finished: 1130Debrief
Issues raised
1. Mobilisation of MART staff quick and effective. Calls to staff made at 7am and all crew at HQ within 30 minutes.2. Communications following previous release significantly improved through use of hand held radios and mobile telephones.3. Once release vessel onsite good liaison between crew aboard and support vessel Mantis who had been onsite for 20 minutes. Cutting crew advised of current situation and plan on actions to be completed – order of cutting4. Communication between cutting crew good. Cutter focused on job at hand and not able to pick up on signs displayed by animal – limited vision due to proximity to animal. Spotter watch for visual signs displayed by animal and was able to advised driver on when to approach and retreat.5. Zip ties need to be kept on Mantis – packet in each of the inflatables but if on Mantis they could have started the snodding whilst waiting for the inflatable to arrive6. Pole for Go-pro needs to be taken in order to capture underwater footage of animal and release.7. New medium pole and large shoe horn cutter needs to be purchased as one set was lost during release. 8. Addition large shoe horn to be carried in case this happens again (cutting crew believe this is best tool for job – easiest to cut with)9. All Fisheries officers to continue to do risk assessment prior to any release and stick with formulated plan regardless of external pressures.10. Raised that wetsuits need handle on the back so kegger/cutter can be held/supported whilst working. Also wetsuits to be rebranded as Fisheries instead of QBFP.11. Further education to SLSC prior to whale season regarding appropriate number to call when encountering a trapped animal raised. Often SLSC members still call Sea World instead of Fisheries.12. Better communications with media following a release. Currently one sided picture is being presented by Sea World to further their own agenda.

Thanks Jeff,All points noted – regarding 1. Both mobile phone and VHF failed to be effective between Seaway and site some 4NM east ofSeaway.I have had staff conduct further checks with the VHF handhelds where they appear to be transmitting fine. The issue mayhave been that Mantis had their radio on scan and wasn’t locking on to our operating channel, although it was dialled into thescanning range.Mobile phone signal seems to be unreliable at the best of times and this was carried as a secondary measure.Item 4. – certainly room for investigation will discuss with Mark S.Regards

1. Communications – Comms are essential to ensure the safety of staff and that important information is conveyedbetween crews/vessels. I suggest trialling the recommendation before the next whale release so an alternativecommunications system can be investigated if this proves to be unsuccessful.
2. Kegging – One of the positive outcomes of these debriefs is the review of our release tactics. Where practical and withconsideration to staff safety we should endeavour to try different approaches in releasing whales. I support replacingthe existing quick release clips with better quality ones. The new clips should be distributed to all release teams andthe old style clips discarded. A box of 7 red A3 buoys was delivered to the GC on 14 July. The box was marked for theattention of Mark Saul. It may have inadvertently been placed in the contractors shed. I also request the DPI&Fstencilling on the A5 buoys be immediately removed and replaced with Queensland Government. The 1800 numbermust be on the buoys as well.

Marine Animal Release TeamLarge Whale Disentanglement – Debrief of Release
DEBRIEF OF RELEASE CONDUCTED ON 10 / 07 / 2016
DATE OF DEBRIEF 13 / 07 / 2016 TIME 10:30 hrs
PERSONS PRESENT FOR DEBRIEF:
NAME ORGANISATION NAME ORGANISATIONRob McDonald QBFP Ryan Stove QBFPMark Saul QBFP Neil Pye QBFPDavid Kahler QBFP
RM stated call came in at 1037 hours. DK & PC were on duty. RS, MS and RM called in. Water Police stayed with whale. All MART equipment was ready to go. A quick response enabled crew to be onsite within the hour.
ISSUES RAISED
1. Communication - mobile phones poor reception and radios not being answered at sea until close range.
Action: NP to check all radios.MS to add portable GPS to MART gear.
Outcome: DK stated when radio is on scan it keeps getting stuck.RM recommended taking radios off scan and using Channel17direct in future or channel 16 and 17 should work on dual watch.
2. Kegging – issue putting A5 buoy directly onto net rather than using the 30 metre line.
Action: MS to make up 6 x 3 metre extensions.MS to talk to J. Krause about replacing clips with rated 12mm clipsafter buoy clip opened up.
Outcome: MS stated staff are taught in training to do complete kegging process from start to finish. At times it might be necessary to shorten the process to achieve a better outcome.RM stated every release is different but a risk assessment process must not be circumvented.DK suggested rope be 2.5 metres.DK stated process attaching directly onto net works well.RM recommended carrying both A4 and A5 sized buoys in Mantis.MS stated a box of A4 buoys is being sent by J. Krause.

3. Float close to tail - RM expressed concerns about vessel being close to tail.
Action: Whale behaviour monitored. It was not aggressive.
Outcome: MS reminded staff to be conscious of side way swipe of tail.
4. Handle on wetsuit – RS had to grab NP a few times to ensure he did not fall overboard.
Action: WHS aspect to be researched first.
Outcome: It was suggested a flat grab handle be sewn into back of wetsuits.
5. MART Sustenance – muesli bars in stock were out of date.
Action: MS to check food and drinks in stock and advise AO what is required.
Outcome: Purchase food and drinks to be ready for MART release.
6. Go Pro – fell off helmet.
Action: NP to check fitting on helmet.
Outcome: To ensure secured properly.
7. Telemetry Gear – none available.
Action: JK has ordered new telemetry gear.
Outcome: New telemetry gear will arrive shortly.
NP stated whale behaviour training was beneficial and key triggers were noticed when watching the whale
