Clinical Features
• Majority: within 60 min• biphasic phenomenon
– 3-20% of patients– caused by a second phase of mediator
release, peaking 8-11 hours after the initial exposure and exhibiting itself clinically 3-4 hours after the initial clinical manifestations have cleared
![Page 39: Nl ii '59 emergency](https://reader034.fdocuments.in/reader034/viewer/2022052418/587a2b6a1a28abbd388b635f/html5/thumbnails/39.jpg)
Clinical Features
• late-phase allergic reaction– primarily mediated by the release of newly
generated cysteinyl leukotrienes, the former slow-reacting substance of anaphylaxis
![Page 40: Nl ii '59 emergency](https://reader034.fdocuments.in/reader034/viewer/2022052418/587a2b6a1a28abbd388b635f/html5/thumbnails/40.jpg)
Diagnosis

Diagnosis
• routinely order acetaminophen levels in all overdose patients regardless of the history of substances ingested
![Page 148: Nl ii '59 emergency](https://reader034.fdocuments.in/reader034/viewer/2022052418/587a2b6a1a28abbd388b635f/html5/thumbnails/148.jpg)
Diagnosis
Specific levels1. acute overdose: 4-hour postingestion
acetaminophen level – Obtain a second level at 8 hours if the 4-hour
value is borderline or if delayed absorption is anticipated.
2. The nomogram should not be used to assess chronic or repeated ingestions.
