O Medical Forms – make Dr. appointments now PIAA forms on-line - NHSD Athletics
Physical must be signed by Dr. & dated after June 1st
Complete online registration & deliver physical form to Meg Part at the HS Office prior to August 14th
Current ImPACT test must be on file to start pre-season Testing opportunities will be scheduled in June
